WHAT IS BIO-IDENTICAL PELLET HORMONE THERAPY AND HOW DOES IT WORK/DIFFER FROM TRADITIONAL HORMONE REPLACEMENT THERAPY? DR. NAEL DAGSTANI: Pellet hormone therapy is designed to reconstruct, as close as possible, the physiology that a woman or a man would possess in their late 30’s or early 40’s. The structure of the hormones used is identical to what the human body produces, hence the term “bio-identical”. Utilizing pellets as the hormone delivery method allows the body to control the release of the hormone, which cannot be reproduced by pill, cream, patch or injection. SINCE THE HORMONE PELLET IS INSERTED UNDERNEATH THE SKIN, IS THE RELEASE PROCESS MORE BENEFICIAL TO THE BODY? DR. D: Yes, the pellet method tends to be the most effective. When you’re using other forms – pills, creams, gels, patches, etc., you tend to get more of a rollercoaster effect. Even with the patch, you get 50% of the hormone in the first 24 hours. With pellets, hormone release is controlled by heart rate and the amount of blood that’s pumping. The analogy often used is a child licking a lollipop, the child being your body and the pellet being the lollipop. If you’re exercising, your heart rate goes up; the kid is licking the lollipop more vigorously. When the body is resting, less of the lollipop is used. The importance is that you get the right amount of hormone at the right time, continuously. Other forms of delivery aren’t able to do that.

WHAT ARE THE MOST COMMON SYMPTOMS THAT BRING PATIENTS IN TO SEE YOU? DR. D: Many patients I see are having trouble with symptoms such as irritability, anxiety, fatigue, low libido, brain fog, muscle loss, night sweats, and hot flashes. Many of those symptoms are related to a testosterone deficiency, which tends to be undervalued and underutilized for women. Women of course don’t require nearly the amount of testosterone that a man does, but the role that it plays in a woman’s body is just as significant. HOW SOON WILL A PATIENT START FEELING BETTER AND HOW LONG DO THE PELLETS LAST? DR. D: It depends on the individual, but patients often start feeling better anywhere from a few days to several weeks. Night sweats and hot flashes tend to be the first issues to improve and that is often within a few days; improvement in energy, mental clarity, mood, and libido tend to be more testosterone related, which usually takes a few weeks for effect. Depending on the individual’s activity and stress level the pellets can last anywhere from 3 to 5 months. WHEN PATIENTS COME BACK FOR THEIR CHECK-UPS, WHAT IS THEIR REACTION AND YOURS? DR. D: The reactions from my patients range from “I wish I’d done this sooner” to “my husband/wife thanks you”. Bio-identical hormone therapy is one of the most profoundly beneficial treatments I’ve found, it is restoring health at a foundational level, and it is a natural treatment, which all fit with my goals and philosophy as a naturopathic doctor.
