Choosing a doctor Choosing the right doctor is a critical decision that requires some in-depth research. Very often individuals make this decision purely by which names turn up on a health insurance plan, while others choose the doctor who is closest to their homes. Although managed care has changed the way many look for and use doctors, this doesn’t mean a person should remain hands-off in the selection process. Narrow down potential doctors by their specialty, academic history, proximity to home, and whether the doctor is board-certified. A doctor who is board-certified has taken several extra years of training and passed a difficult board examination. To remain board-certified, he or she sometimes must complete continuing education and periodic recertification. After the list is made, you can select two or

This way there is the chance to gauge the doctors’ personality, how well the interaction goes, and how comfortable you would feel in his or her care. The doctor who best meets this criteria is likely going to make the best fit.

Great tips for better heart health It’s always important to pay attention to matters of the heart -- especially those that impact your heart health. As the leading killer of Americans, cardiovascular disease affects one in three people in the U.S. -- approximately 81 million people. And, the American Heart Association predicts that this number will increase to 116 million people, or 40.5 percent of Americans, by 2030. Thankfully, there are simple steps you can easily incorporate into your day-to-day life that can make a big difference, according to Susan J. Crockett, PhD, RD, FADA and leader of the General Mills Bell Institute of Health and Nutrition. “Genetics does play a role in cholesterol and overall heart health,” says Crockett. “While

being aware of one’s family history is certainly important, there are lifestyle changes you can make and foods you can eat as part of an overall healthy diet that can decrease the risk factors for heart disease and may help lower cholesterol.” There are several ways people can begin to make a difference in their personal heart health and cholesterol levels, according to Crockett. To help make your lifestyle more heart-healthy, try to remember Crockett’s “HEART” tips. • Have a list: Keep a running list of health to-dos, such as regular cholesterol and blood pressure screenings, as well as questions for your physician. Find out and jot down foods you can eat to lower cholesterol and other ways to take

care of your heart. • Eat more of the “good stuff”: Be conscious of what you are eating and make an effort to incorporate heart healthy foods into your diet. For example, fruits, vegetables, whole grain, and low-fat dairy are all good choices for a heart healthy diet. Eating more fish like salmon, which contains omega-3 fatty acids, may also help reduce the risk of heart disease. • Aim for more whole grain: When making food choices, look for whole grain oats or whole grain oat cereal that contains beta glucan, like Cheerios. Beta glucan is a natural soluble fiber found in oats that helps reduce bad cholesterol. To reduce the risk of heart disease, you need three grams of soluble fiber daily

from whole grain oat foods as part of a diet low in saturated fat and cholesterol. • Run, walk, skip, jump: However you choose to exercise, just make sure it is a part of your daily routine.   • Try healthy swaps: Healthier options are out there, so make the switch. For instance, instead of full fat mayonnaise, spread smashed avocado on your sandwich, which is high in healthy fats that help your heart. Also, when eating out, don’t be afraid to ask your server for healthier preparation methods, like steamed and broiled instead of fried, or ingredient substitutes like whole grain pasta. Taking a few moments each day to make heart healthy decisions can make a significant difference in your future.

What to do when beginning an exercise regimen There are precautions men and women should take before beginning a new exercise regimen. • Visit your physician. It’s best to get a full physical before beginning an exercise regimen. A full physical can reveal if you have any health problems that might limit what you should and shouldn’t be doing at the gym. If anything turns up, your physician can develop a plan of attack for you to address the issue. If nothing turns up, then your doctor will probably give you the green light to go forward with few, if any, limitations. * Conduct a self-assessment. Once you’ve visited the doctor and received the go-ahead to start working out, do an honest self-assessment to see where you are in terms of fitness. Walk a mile and time yourself. Do as many push-ups and sit-ups as possible, but be careful to stretch and not push yourself. This self-assessment should not be demanding. Instead, the goal is to gauge where you are and how your body feels when doing some simple exercises. • Establish your goals. The goal of most people beginning a new exercise regimen is to lose weight. However, there are other incentives as well. For example, some people might be starting to train for a marathon or another sporting event. Whatever the reason, know why you’re getting started, as such goals can help you monitor your progress as the year goes on. • Start slowly. Caution should reign supreme when beginning an exercise regimen. Diving into the deep end at the onset increases the risk of injury, which could limit activity for months to

12 | Medical Directory 2012

come. First get your body acclimated to exercise, then gradually challenge yourself as you see fit. • Leave time to recover. Though it might feel rejuvenating to get back to exercising, it’s important for everyone, but especially those who are just starting, to allow themselves some time to recover. Allow your muscles and joints to recover between workout sessions. Frequency of sessions can increase as your body gets acclimated, but at first allow a day or two between sessions so your body can recover. • Listen to your body. Exercising after a long hiatus from routine exercise won’t be easy, and your body is likely going to tell you that through certain aches and pains, if not nausea, dizziness or shortness of breath. If any of these symptoms appear, take a break. This could be your body telling you that you’re asking too much and you need to take your foot off the gas pedal for a little while. • Consider hiring a personal trainer. Many people are overwhelmed when entering a gym after a long time away. If you find yourself intimidated or simply don’t know where to begin, hire a personal trainer. Many charge by-the-session, so you can learn which machines to use and how to use them after a session or two and then continue working out on your own. If joining a gym as a new member, the gym might offer a couple of complementary personal training sessions. If so, take full advantage of this offer. When beginning a new exercise regimen, don’t forget to let caution reign until your body has adjusted to this healthy lifestyle.

Routine checkup schedule for women While there are many health screenings relevant to both men and women, there are also tests specific to each gender. Two of the more important routine screenings women should be aware of are regular mammograms and pap tests, which can both diagnose risk factors for certain types of cancer. Mammogram A mammogram consists of X-ray pictures of the breasts and is used to detect irregularities in breast tissue that may reveal the presence of cancerous tumors. Screenings also might detect microcalcifications that can sometimes indicate breast cancer as well. The Centers for Disease Control and Prevention says that 40,678 breast cancer deaths occurred in 2009 and that 26 out of every 100,000 females will die from breast cancer. Most organizations, including the National Cancer Institute, recommend that women age 40 and over have a mammogram done every one to two years to help catch

14 | Medical Directory 2012

cancer early, which improves a woman’s chance of survival. However, not every woman is routinely getting screened. In the United States, roughly 68 percent of women have gotten mammograms within the last two years, according to the CDC. Although mammograms require small doses of radiation, which can cause cancer, to take the images, the benefits nearly always outweigh the risks. It is recommended that women age 35 to 39 get a baseline mammogram so they have an image of the healthy breast to which future mammograms can be compared. Pap Tests A pap test, also known as a pap smear, is a diagnostic tool that checks for cellular changes in the cervix. The cervix serves as a barrier point between the vagina and the uterus. The pap test is usually done in a gynecologist’s office, although family practitioners also may administer it in their own offices. The doctor will insert

a speculum into the vagina to widen the passageway so he or she can see the cervix. He or she then will take a sample of cells from inside and outside the cervix and send them to a laboratory for testing. It is recommended that a woman refrain from sexual activity prior to her pap test. It’s also best if a woman does not use any lubricants, cleansers or medications that are used intravaginally before the test. Also, she should not use tampons. In fact, those who have their menstrual period will want to reschedule the pap because it can be difficult for a doctor to get a good collection of cells for testing. The frequency of pap tests will be determined by the woman’s age, results of prior pap tests, medical history, and history of the human papilloma virus, or HPV. Many women get annual pap tests starting at age 21 or when they first become sexually active. Those who have had normal results for at least three years in a row may

opt to get tested every two or three years. Women who are age 65 or older may be able to cease pap tests, but annual pelvic exams are still advised. For those who have had a hysterectomym which involves the removal of the uterus, pap tests are still necessary. However, after a total hysterectomy (removal of the uterus and cervix), pap tests may stop. Pap tests are not always 100 percent accurate, however. False positive and false negative results can happen. The doctor may require a different type of test if a pap test comes back with a positive result. To remain healthy, women are advised to keep up with routine physicals and tests that can help diagnose cervical and breast cancer in their early stages. If lack of medical insurance is preventing a woman from being tested, she can contact the National Breast and Cervical Cancer Early Detection Program to find programs that offer free or low-cost pap tests to women in need.

My pills are expired. Now what? It happens every day. You have a headache or another ailment and go to the medicine cabinet to find relief. After shaking a few pills out of the bottle, you happen to glance at the expiration date stamped on the side and realize those painrelief pills have expired. You wonder if you will get sick if you swallow them or if you can get away with it this time. Who isn’t trying to stretch a dollar a little further these days? And with prescription drug costs rising and some companies cutting back on health insurance coverage, there are thousands of people who may be between plans and cannot afford to continually restock their medicine cabinets with new drugs. Is it safe to take that anti-anxiety medication even if its expiration date has come and gone? Will those acid reducers make you even more sick? These are viable questions. For the most part, medical experts say that expired drugs are reasonably safe to take. According to information published in Pharmacology Today, the expiration date stamped on over-the-counter medication is a date at which the drug manufacturer can still guarantee full potency of the drug. The expiration date on your prescription medicine bottle may be the date that the prescription -- not the medicine -- expires, generally a year after the medication was filled. A law was passed in 1979 that required drug manufacturers to issue the expiration date as a means to giving consumers what they paid for, and likely to avoid litigation over drugs that are no longer effective. Medical authorities state that the majority of expired drugs are safe to take -- even medications that expired years ago. However, their potency may be

16 | Medical Directory 2012

reduced. Liquid medications, such as oral antibiotics, may lose their potency faster than pills. Tetracycline, a broadspectrum antibiotic, is one that causes some controversy regarding safety after expiration. So it’s best to discard tetracycline pills once they have expired. Others say that nitroglycerine and hydrocodone (Vicodin) may present some dangers after expiration, but this has not been proven in any large-scale study. If you need some more reassurance that those expired pills are fine to take, consider a study conducted by the US Food and Drug Administration at the request of the US military. The military was considering disposing of and replacing its drug store every few years because of expiration dates, which would have come at a considerable cost. After a lengthy analysis, the FDA determined that 90 percent of the more than 100 drugs they tested -- both prescription and OTC -- were still potent even 15 years after the expiration date. That doesn’t mean it is always safe or effective to take

an expired pill, especially if you are self-diagnosing a medical condition and subsequently self-medicating. Medications should always be used under the guidance of a doctor who can monitor dosing and progress. Also, medications should never be shared among different members of the family for whom they were not prescribed. Individuals who stockpile medications also run the risk of some other dangers. There’s the chance of grabbing the wrong bottle and taking a medication that is not needed, a problem common with the elderly that can result in illness. With narcotic and prescription drug abuse a rising epidemic among young people, having a cabinet full of drugs could prove tempting to adolescents thinking about getting high from drugs readily available in their own homes. The best advice regarding expired drugs is when in doubt, throw them out. But if you’ve just swallowed some expired ibuprofen and are worried about side effects, chances are there is nothing to worry about.

Vaccinations are crucial to protect young children Most parents know that one of the most important steps they can take to protect their children is to vaccinate them against dangerous diseases. But knowing what to vaccinate against and when to do so can be a complicated process. Don’t put off getting in touch with your pediatrician to have these questions answered, and to ensure you are up-todate with your child’s vaccinations. “When children are not immunized, the results can be devastating,” says Dr. Robert W. Block of the American Academy of Pediatrics (AAP). “Making sure your children are properly immunized will give them up to a 98 percent chance of avoiding many diseases.” The AAP is offering these vital tips for parents to help

sort out all the details: • Ask questions. Remember, your pediatrician is ready and willing to partner with you on getting your children immunized. Use your child’s doctor as a resource to help you sort out the endless supply of information -- some good and some questionable -- on this topic. So prepare questions in advance, and take a note pad with you to your visit so you can jot down important advice and critical dates. • Be on time. Your children’s immunization schedule is not an area of parenting where you should feel free to get creative. There are scientific reasons, based on knowledge of children’s immune systems for why shots are recommended at certain ages. Updating your records and keeping a calendar can help you keep it

all straight. More information on recommended schedules and on how immunizations work can be found at www. healthychildren.org. • Cocoon. One of the best ways to protect very young children -- who have not yet received the full range of vaccines for deadly diseases -- is to ensure that all family members and caregivers who come in close contact with the child are up-to-date on their own immunizations.  This protective measure is called “cocooning.” • Smile. Most children won’t enjoy getting vaccinated. You can make the process smoother by staying calm yourself, as young children can pick up on your feelings. Remember to cuddle with your child before, during and after the vaccination. Bringing

along a favorite stuffed animal or blanket is a great way to provide some much-needed comfort. If your child is experiencing pain, ask your doctor if a non-aspirin pain reliever is safe to administer. Consider your child fortunate to grow up in the U.S., where he or she will have easy access to lifesaving vaccines. Not all children around the world are equally protected, however. Parents can join one of several global campaigns, such as ShotatLife.org, to support the health of children all over the world. You have more control over your children’s wellness than you realize. If you’re unsure of whether your children’s vaccinations are up- to-date, do your little ones a favor and check with your pediatrician today.

Tips for men to stay healthy as they age With age comes wisdom. Unfortunately, it also comes with an increased threat of developing certain health problems. Prostate cancer and other diseases affect a disproportionately large amount of American men. According to the Prostate Cancer Foundation (PCF), more than 16 million men are affected by the disease globally, and American men represent nearly 2.5 million of that figure. Given these statistics, American men should be especially vigilant about their health. With a few lifestyle tweaks and attitude adjustments, you can help stay healthy as you age. Eat Right If you’ve eaten a particular way your whole life, you might find it difficult to change. But cutting out the junk in favor of fruits, vegetables and whole grains is well worth the effort. Not only will you look and feel better, certain foods have even been proven to reduce your risk of developing diseases like prostate cancer, coronary heart disease and diabetes. Opt for fish over red meat. Evidence from several studies suggests that fish can help protect against prostate cancer because it contains “good fat,” particularly omega-3 fatty acids. Choose olive oil over margarine. While monounsaturated fat found in olive oil is beneficial to health, trans-fatty acids contained in margarine contribute to clogged arteries, high cholesterol and an increased risk of stroke and heart attack. A free nutrition guide and

tasty recipes are available at www.pcf.org/nutrition . Stay Active A sedentary lifestyle contributes to your risk of obesity, heart disease and cancer. You don’t need to become a marathon runner, however, to experience benefits from a more active lifestyle. Start with what you can handle, like a 20 minute walk once a day. If you have bad joints, consider a low impact activity like swimming. Get Checked While it’s always important to be open with your physician about your health and your family’s health history, starting at age 40 it becomes crucial. The older you are, the more likely you are to be diagnosed with prostate cancer and other diseases. In fact, more than 65 percent of all prostate cancers are diagnosed in men over the age of 65, according to PCF. Your doctor can help you determine your risk of developing various diseases, and make recommendations accordingly. Regular doctor’s visits are also an important component of early diagnosis. While these tests may not be fun, they can save your life. If you have a history of prostate cancer in your family, consider a yearly rectal examination and a Prostate-Specific Antigen (PSA) test starting in your forties, or even earlier if your doctor recommends it. Broader awareness and understanding of the health risks associated with aging can save lives. So don’t shy away from talking to your friends and family about your health, and theirs.
