A Brief Introduction to Instrument-Assisted Soft Tissue Mobilization (IASTM)

What is soft tissue?

The term “soft tissue” refers to the fascia (connective tissue): muscles, tendons, and ligaments that make up the body. Injuries to these areas are a common cause of pain and dysfunction that causes patients to seek the assistance of a physical or occupational therapist. There are a variety of techniques used to treat these conditions, including exercise modalities and manual therapies. Instrument-assisted soft tissue mobilization (IASTM) is one of the manual techniques a physical or occupational therapist can use to treat these injuries.

What is IASTM, and why is it used?

When soft tissue injuries occur, the natural processes of the body lay down new cells to heal the injury. These cells are often laid down in a disorganized pattern and they can bind to the underlying tissues, ultimately restricting the mobility of these structures. IASTM involves the use of tools to mobilize the soft tissue structures of the body. These tools come in many shapes and sizes, and are made of a variety of materials, such as stainless steel, plastic, jade, and ceramic. While the tools may differ in shape and material, they share the common purpose of assisting the clinician with mobilizing the injured soft tissue in order to reduce adhesions (scar tissue), increase blood flow to the injured area, and encourage healing of the damaged tissue.

What injuries are most commonly treated with IASTM?

The different shapes and edges of the tools allow the clinician to use them around all of the different contours of the body, which makes them useful for most conditions seen in the clinic. These tools can be used to break down scar tissue after surgery, decrease pain caused by sprains and strains to ligaments and muscles, reduce swelling/edema, and alleviate nerve restrictions. Common conditions that warrant use of IASTM treatment are joint replacements, rotator cuff tears and surgical repairs, plantar fasciitis, muscle strains, and tendonitis.

Therefore, temporary redness, heat sensation, and sometimes bruising are normal immediately following treatment, and there may be some soreness or discomfort. It is important to not be alarmed by these symptoms, and recognize that these are temporary symptoms being closely monitored by your therapist. After several treatments, there should be a noticeable change in mobility and function. IASTM, combined with your exercise program, can help you return to your normal daily activities and hobbies.

Olivia graduated with a Bachelor’s Degree in Public Health and a Minor in Gerontology from The Richard Stockton College of New Jersey in 2012. She continued her education at the University of Medicine and Dentistry of NJ in Newark, where she received her Doctorate of Physical Therapy in 2015.

While in graduate school, Olivia and her fellow colleagues conducted a study entitled “For individuals with lower extremity musculoskeletal injuries, do off-the- shelf video games, compared to standard of care, improve balance, function, and patient satisfaction?” The abstract was accepted and Olivia presented the poster at the National Combined Section Meeting in 2015.

Since graduating in 2015, Olivia’s primary professional focus has been in the orthopedic outpatient setting, although she has also worked and completed clinical experiences in both acute care and acute rehabilitation. Olivia is a certified LSVT Big clinician, and she has been trained in Instrument Assisted Soft Tissue Mobilization. Olivia is interested in non-operative and operative orthopedic conditions, sports rehabilitation, balance and gait dysfunction, and manual therapy. Her future professional plans include becoming a Board Certified Orthopedic Clinical Specialist.

If you don’t already have one, consider establishing a budget or a spending plan. Creating a budget requires you to: • Identify your current monthly income and expenses • Evaluate your spending habits • Monitor your overall spending To develop a budget, you’ll need to identify your current monthly income and expenses. Start out by adding up all of your income. In addition to your regular salary and wages, be sure to include other types of income, such as dividends, interest, and child support.

Next, add up all of your expenses. If it makes it easier, you can divide your expenses into two categories: fixed and discretionary.

Fixed expenses include things that are necessities, such as housing, food, transportation, and clothing. Discretionary expenses include things like entertainment, vacations, and hobbies. You’ll want to be sure to include out-of-pattern expenses (e.g., holiday gifts, car maintenance) in your budget as well.

To help you stay on track with your budget: • Get in the habit of saving—try to make budgeting a part of your daily routine • Build occasional rewards into your budget • Examine your budget regularly and adjust/make changes as needed Setting and prioritizing financial goals The second step to creating and implementing a financial plan is to set and prioritize financial goals. Start out by making a list of things that you would like to achieve. It may help to separate the list into two parts: short-term financial goals and long-term financial goals.

Short-term goals may include making sure that your cash reserve is adequately funded or paying off outstanding credit card debt. As for long-term goals, you can ask yourself: Would you like to purchase a new home? Do you want to retire early? Would you like to start saving for your child’s college education?

Once you have established your financial goals, you’ll want to prioritize them. Setting priorities is important, since it may not be possible for you to pursue all of your goals at once. You will have to decide which of your financial goals are most important to you (e.g., sending your child to college) and which goals you may have to place on the back burner (e.g., the beachfront vacation home you’ve always wanted).

Implementing saving and investment strategies After you have determined your financial goals, you’ll want to know how much it will take to fund each goal. And if you’ve already started saving towards a goal, you’ll want to know how much further you’ll need to go.

Next, you can focus on implementing appropriate investment strategies. To help determine which investments are suitable for your financial goals, you should ask yourself the following questions: • What is my time horizon?

• What is my emotional and financial tolerance for investment risk?

• What are my liquidity needs?

Once you’ve answered these questions, you’ll be able to tailor your investments to help you target specific financial goals, such as retirement, education, a large purchase (e.g., home or car), starting a business, or increasing your net worth.

Managing your debt and credit Whether it is debt from student loans, a mortgage, or credit cards, it is important to avoid the financial pitfalls that can sometimes go hand in hand with borrowing. Any sound financial plan should effectively manage both debt and credit. The following are some tips to help you manage your debt/credit: • Make sure that you know exactly how much you owe by keeping track of balances and interest rates • Develop a short-term plan to manage your payments and avoid late fees • Optimize your repayments by paying off high-interest debt first or take advantage of debt consolidation/refinancing Understanding what’s on your credit report An important part of managing debt and credit is to understand the information contained in your credit report. Not only does a credit report contain information about past and present credit transactions, but it is also used by potential lenders to evaluate your creditworthiness.

What information are lenders typically looking for in a credit report? For the most part, a lender will assume that you can be trusted to make timely monthly payments against your debts in the future if you have always done so in the past. As a result, a history of late payments or bad debts will hurt your credit. Based on your track record, if your credit report indicates that you are a poor risk, a new lender is likely to turn you down for credit or extend it to you at a higher interest rate. In addition, too many inquiries on your credit report in a short time period can make lenders suspicious.

Today, good credit is even sometimes viewed by potential employers as a prerequisite for employment—something to think about if you’re in the market for a new job or plan on changing jobs in the near future.

Because a credit report affects so many different aspects of one’s financial situation, it’s important to establish and maintain a good credit history in your own name. You should review your credit report regularly and be sure to correct any errors on it. Business Spotlight Dr. Kathy Banks received her undergraduate degree from Rutgers University and received her DMD from Temple University School of Dentistry. After dental school, she completed an Advanced Education General Dentistry program and then completed her 4-year specialty training in Oral and Maxillofacial Surgery at Rutgers School of Dental Medicine.

She was later awarded her Board Certification by the American Board of Oral and Maxillofacial Surgeons and the American Board of Anesthesiology. She is currently the first female Chair of the Oral and Maxillofacial Surgery Foundation, and organization dedicated to improving the quality and safety of patient care by advancing innovation in oral and maxillofacial surgery research and education.

In addition to caring for her patients, Dr. Kathy Banks is highly focused on perfecting her team in the office. Although it did take some time, she has now found the ideal fit.

Team members Monica, Danielle, Diana, Amanda, Jessica and Sonya work exceptionally well together and enable her to provide the excellent patient care that she is known for. As an Oral and Maxillofacial Surgeon, she has had numerous papers published and is a frequent presenter for continuing education classes.

With more than 15 years of experience treating patients of all ages, she is able to treat most of them comfortably in her office. However, when circumstances dictate, Dr. Banks holds hospital privileges in three hospitals: Jersey Shore University Hospital, Southern Ocean Medical Center, and Community Medical Center. She treats patients on an outpatient basis when necessary.

Another enhancement to her practice has been starting ongoing education in various areas of expertise. “Before opening this office, I witnessed staff members elsewhere going through the motions of their jobs. I did not want this to be my team. As a life- long learner, I insist on investing in the professional development of all of my team members, including myself.

New technology plays a big part in the office of Dr. Banks. “Hands down, the number one development in oral and maxillofacial surgery has been integrating 3D imaging into patient care. Cone Beam CT scanning is indispensable in diagnosing common oral conditions such a cysts, tumors, and impacted teeth ” she says.

She also uses 3D images for dental implant surgery and can perform virtual surgical implant surgery using either of the two software programs.

She explains, “From this work up, a surgical guide can be fabricated and used in the surgery procedure so that the implants are placed in the exact angulation and depth as planned. This is called “guided surgery” and I use this routinely.”

Another use for the CBCT scan is jaw reconstruction. “I have used CBCT scans to plan orthognathic surgery procedures where the jaws are cut and repositioned so the teeth match up. The virtual surgery plan done on the computer is used to generate surgical guides that help me perform the jaw surgery with enhanced precision,” she says.

“Why Did I Get Cancer, Doc?�

Whether it is Mark, who was diagnosed with leukemia at age 45, or Marge, who was diagnosed with colon cancer at age 80, or Mary, who was diagnosed with breast cancer in her thirties, they all have the same questions, ultimately: “Why me? Why did I get cancer? How long did I have it? What could I have done to prevent it?�

The answer is not simple. Scientists have been researching for decades to and the answer to this question. In fact, there is no one simple explanation to why one person developed cancer, while another person in the same environment and genetic background may not develop it in his or her lifetime. But scientists have found links between certain things and the development of cancer.

It is known that cancer is caused by changes in one’s DNA. DNA is a component of the nuclei in the cells. When the DNA is damaged somehow, it makes the cells multiply in abundant numbers. Usually, abnormal cells that are made in the body are destroyed by the body’s own defense mechanism, a process called ‘apoptosis’ or programmed cell death. When there is a defect in apoptosis, cells accumulate and become tumors.

The damage to the DNA can be caused by several factors. ere are some changes or mutations that are inherited and are present from birth, which can later make a person susceptible for cancer. Only a small percentage of cancers develop this way. In the majority of cases, certain environmental factors play a major role.

Some factors that play a role in the development of cancer are: ‡ /LIHVW\OH IDFWRUV (nutrition, tobacco use, physical activity, etc.) ‡ 1DWXUDOO\ RFFXUULQJ H[SRVXUHV (ultraviolet light, radon gas, infectious BHFOUT FUD t .FEJDBM USFBUNFOUT DIFNPUIFSBQZ SBEJBUJPO JNNVOF system-suppressing drugs, etc.) ‡ :RUNSODFH H[SRVXUHV ‡ +RXVHKROG H[SRVXUHV ‡ 3ROOXWLRQ Substances and exposures that can lead to cancer are called carcinogens. Some carcinogens do not aect DNA directly, but lead to cancer in other ways. For example, they may cause cells to divide at a faster than normal rate, which could increase the chances that DNA changes will occur.

Carcinogens do not cause cancer in every case, all the time. Substances labeled as carcinogens may have dierent levels of cancer-causing potential.

Some may cause cancer only after prolonged, high levels of exposure. And for any particular person, the risk of developing cancer depends on many factors, including how they are exposed to a carcinogen, the length and intensity of the exposure, and the person’s genetic makeup.

There are several substances that are proven to be carcinogens. Still others are felt to be involved with a high risk for cancer, but their role is not substantially proven.

The International Agency for Research on Cancer (IARC) is a part of the World Health Organization (WHO) that works on identifying causes of cancer. IARC classies potential carcinogens into ve dierent groups, depending on the level of evidence that links each to the cancer. Some of the known Group 1 carcinogens are: alcohol, aatoxins, benzene, coal tar, hepatitis C virus, radioactive materials, tobacco smoking, etc. A detailed list of the carcinogens is available on the American Cancer Society’s website, cancer.org.

Even though exposure to carcinogens increases the chance of developing cancer, a variety of other factors also play a role. While most of it is beyond our control, there are some things which a person can do to reduce their chance of getting cancer.

HPV In Oropharyngeal Cancer The human papilloma virus or HPV is the most common sexually transmitted viral infection in the United States. There are many strains of the HPV virus and several have been shown to cause cancer.

It has been estimated that approximately 12,000 individuals are infected with HPV every day. In the majority of individuals, the immune system can clear the virus, but some people can become chronically infected.

HPV is known to cause cancer of the cervix in women and squamous cell cancer of oropharynx including the tonsils and the base of the tongue in both men and women. Risk factors include sexual activity at an early age, a greater number of sexual partners, and weakened immune systems. In the past, oropharyngeal cancer was usually associated with a history of smoking or drinking alcohol. Today the number of patients with oropharyngeal-related HPV cancer is increasing and it is now the most common cause. In patients infected with HPV, oropharyngeal cancer can develop in people in

their 30s to 50s even in the absence of a smoking or alcohol history. It may present as an ulcer, growth or pain in the mouth/throat or a mass in the neck.

Patients should undergo a complete head and neck examination by an Otolaryngologist, which may include using a fiber-optic camera to evaluate the tonsils and back of the tongue. In the event of suspicious findings, a biopsy or fine needle aspiration will be done to confirm the pathology and assess the presence of HPV. Once diagnosed, the treatment may include resection of the tumor, often using robotic technology, and removing the lymph nodes from the neck. Radiation therapy and chemotherapy may also be utilized as a primary treatment modality or in addition to surgery.

HPV-related cancer can be prevented. Young men and women can be vaccinated against HPV. It is recommend that this be performed beginning at age 11-12 before they become sexually active.

The vaccination can include HPV type 6, 11, 16 and 18. HPV 18 type is the most common etiology for oropharyngeal cancer. I would encourage parents to have their children vaccinated against HPV and potentially save them from these types of cancers.

Raising a child with special needs can be a rich experience. It’s also a costly one that comes with unique challenges for your financial future and estate plans. There is no one-size-fits-all recommendation, but here are some things to consider: 1. Your future security: Protect your children

by securing your own financial future. Start saving for retirement as early as possible to take advantage of compound interest.

2. Life insurance: Consider investing in enough

life insurance to care for your child’s future financial needs. Government benefits can cover basic costs, but extra funds can fund additional expenses, such as hiring an additional caregiver after you’re gone. Talk to an advisor about beneficiary designations and consider a special needs trust. Consider disability insurance, too, to protect your family financially if you are living but no longer able to work.

3. Special needs trust: Assets held in a child’s name are held against them and could disqualify them from government benefits. One way around that is to set up a special needs trust. The intent is that a special needs trust can supplement a disabled person’s income in order to pay for extra wants and needs not covered by government benefits.

4. Educate your family: If you think your

parents or another family member might leave a legacy to your child, make sure they understand

the implications of a cash gift and the benefits of a special needs trust.

5. 529 ABLE (529A): Like a 529 education

plan, a 529A is a tax-advantaged way to save for your child’s special needs education and certain disability expenses. Generally speaking, parents should consider maxing out their own retirement accounts before funding supplemental savings tools like these. Be aware that once the account exceeds $100,000, your child will no longer be eligible for Supplemental Security Income (SSI) payments.

6. Adult guardianship: If your child will be

unable to make medical and financial decisions after he or she turns 18, talk to a lawyer about a power of attorney and health care proxy or becoming your adult child’s legal guardian. Don’t assume you’ll automatically be able to make decisions for them just because they’re disabled.

If you or your family member is in the hospital for an acute illness, surgery or an injury, you may be told that the next step is “rehab,” short for rehabilitation services.

Going from the hospital to a short-term rehab facility can be daunting.

Here are 5 important points to remember:

1. The goal of rehab is to help patients be independent – to do as much for themselves as possible.

2. Rehab is done with a patient, not to a patient. You or your family member must be willing to work with the rehab team in the facility and later at home to continue a home exercise program.

3. The patient’s chronic (long-term) health conditions, such as high blood pressure or diabetes, are treated during the rehab stay.

4. The length of stay in a rehab facility typically last for a few weeks.

5. Medicare, Medicaid and most private insurances cover rehab when ordered by the doctor.

When going from the hospital to an inpatient rehab setting, be prepared for a quick move. A discharge planner in the hospital (a nurse or a social

Rehab includes treatment to help patients get back all or some of the movement and function they lost because of the current health problem.

For example, many people need rehab after hip or knee replacements.

It’s never too early in a rehab stay to start thinking about discharge. You or your loved one should never leave the facility until there is a safe and adequate discharge plan, whether it be to a long-term setting (such as a nursing home or assisted living) or to your own home.

If you or your loved one are planning to return home, here is what you need to consider:

• Home health care services – Medicare will pay for short-term rehab at home, but you may need additional caregiver help, such as a home health aide • Medication management – make sure you understand your medication regimen before leaving the facility • Primary doctor follow-up – arrange a follow-up appointment as soon as possible after discharge • Financial accounting – review your rehab bills carefully • Equipment needed – make sure you arrange for any needed equipment, such as a hospital bed, wheelchair or walker, before leaving for home

Rehab is an important step in achieving the most independence and function possible. Remember that you are the most important part of the rehab team. Use this time well to learn and practice. You or your loved one will benefit from this step in care.

1 garlic clove minced 1/2 cup red wine vinegar 1/4 cup capers, drained 2 T sugar 3 large tomatoes, cut into bite size chunks 4 1/2 oz pimento stuffed olives, drain & cut in half

In a large saucepan over high heat, cook eggplant, zucchini, mushrooms, onions, celery and garlic in the oil for 10 minutes, stirring occasionally.

Stir in the vinegar, capers, sugar, salt and pepper to taste.

Reduce heat to low, cover and simmer for 10 minutes or until vegetables are fork tender.

Stir in tomatoes and olives and bring to a boil over high heat. Spoon mixture into a bowl, cover and refrigerate at least 3 hours or until well chilled.

Over 90 million Americans suffer from a variety of chronic sleep disorders, resulting in poor quality and a limited quantity of sleep that significantly interferes with health and functioning.

Obstructive sleep apnea is one of the most undiagnosed medical conditions and it is estimated that about 20 million Americans suffer from this condition. Patients with OSA stop breathing many times throughout the night. This is caused by a physical blockage or collapse of the upper airway while sleeping, resulting in a chronic lack of oxygen to the brain and body. Over time, it takes a toll on your body and causes or contributes to other very serious medical problems.

How does it affect our health?

It is no surprise that obstructive sleep apnea is referred to as the silent killer. Sleep apnea is a very serious condition and should not be taken lightly. If left untreated it damages your cardiovascular system and can contribute to high blood pressure, heart disease including arrhythmias, rapid heart rate/palpitations, heart attacks and strokes.

Other conditions related to sleep apnea include depression, mood disorders, memory loss, low sex drive, restless leg syndrome and acid reflux or GERD. OSA contributes significantly to premature death, and studies now show that in some cases a person can decrease their lifespan by as much as a decade if they have untreated sleep apnea.

What symptoms do people with sleep apnea have?

There are a wide variety of symptoms that patients may have, depending on the severity of their condition. Some patients may also show no signs and symptoms if their condition is mild; nonetheless, it needs to be treated. The most frequent sign is loud snoring and sometimes gasping for air while sleeping. Other signs include waking frequently at night, frequent leg movement while sleeping, irritability, lack of focus, loss of memory, morning headaches, daytime sleepiness, low sex drive, obesity, chronic exhaustion and lack of energy.

How is obstructive sleep apnea treated?

The most commonly prescribed therapy for OSA is continuous positive airway pressure or CPAP therapy. This is where a patient wears a mask and is hooked up to a

machine blowing air continuously to keep the airway open while they are asleep. While this is very effective, patients find this treatment very uncomfortable and most patients do not use their CPAP. Studies show that more than 60% of patients prescribed a CPAP will not use it. This puts them at severe risk for serious health problems.

How can you help patients?

I have been in practice for over 14 years, and during this time I have treated many sleep apnea patients with oral appliance therapy, which is an alternative to CPAP. For patients with mild and moderate sleep apnea, this treatment is just as effective as using a CPAP. Patients tolerate oral appliances better than CPAP, and because they are comfortable, the compliance is much better. For most patients with severe sleep apnea, we use oral appliances along with CPAP in combination therapy.

What is oral appliance therapy?

Oral appliances are mouthpieces custom-made for a patient. There are many types of devices available and the appropriate one for a patient is dependent on a number of factors. They work by moving the lower jaw forward and keeping the airway open while sleeping. The bottom line is they are comfortable and non-intrusive.

What should patients do if they suspect they have sleep apnea or they snore?

Any person who snores loudly (or knows someone who does) should be screened for this serious medical condition. Committing to ‘Memory’ –

Designer Turns Her Baking Hobby Into a Full-Blown Career AnnMarie Cooney did not grow up dreaming of becoming a baker. Visions of sugar plums and intricately decorated cakes were not dancing through her head as a young girl.

By trade, Cooney was a kitchen designer, but retired early to raise her daughters. “With three girls, my job was to raise good women,” Cooney says. It was a special request from her daughter that would eventually turn her into a AnnMarie Cooney highly sought-after cake designer and baker.

“It all started because my daughter wanted a cake,” Cooney says with a smile. Cooney’s middle girl, was born with health issues that resulted in special needs. So when she wanted a circus cake for her fifth birthday, Cooney made her the best circus cake around. That was 14 years ago, and her girls quickly caught on to their mom’s special talent.

At every birthday, they requested a special cake. “It became, ‘what can we do next’ and ‘how can we top that,’ Cooney says. Then she began making cakes for friends. Cooney’s husband had to build an additional kitchen at her house just to accommodate all of the special cake requests. When Cooney’s youngest daughter entered kindergarten six years ago, Cooney decided she wanted to go back to work. “But, I needed to figure out what I was going to do,” Cooney says.

Sadly, Cooney’s grandmother, with whom she was very close, passed away during this time. “I was sitting at home, just very sad, and my cousin called me to meet her at the park with her kids,” Cooney says. However, in the midst of grief, she didn’t feel up to it.

“My cousin sad, ‘come on grandma would want us to.’ So I went, Cooney says. On the way to the park, she saw the real estate sign. She checked it out and immediately knew that it would be the perfect location for her bakery, A Sweet Memory Cake Shoppe & Tea Room. “My grandmother left each of her grandchildren a small inheritance and it was just enough for me to open my business,” Cooney explains.

Cooney spent about five years at that location before moving to another larger space six months ago just two miles down the road in Waretown. A beautiful picture of her grandmother has watched over her since the day she opened. It’s difficult to simply call Cooney’s creations “cakes.” Some designs are so intricate that they look like art. Cooney and her team created a three-dimensional depiction of Van Gogh’s “Starry Night” that captured the essence of this masterpiece with sweet, swirly curves and lines of vividly hued icing.

Then there’s the adorable panda birthday cake that could easily be mistaken for a cuddly toy.

The list of unique, edible masterpieces goes on and on, That’s because they average a minimum of 30 specialty cake orders per week, mostly by working with different catering and special event venues in and around Long Beach Island. However, they also have a long list of loyal customers.

“Some of our customers started with a gender reveal and now we’re doing their fifth birthday cake,” Cooney says. Customer Kathy Helm of Waretown got her grandson’s birthday cake from Cooney close to five years ago. “It was perfect, I brought a couple pictures of what I wanted and they just did their thing,” Helm says. And she has been coming here ever since. “I’m in sales, so I bring my clients treats from here all the time,” Helm says. She’s also brought her granddaughter to Sweet Memory’s princess tea parties.

“Our clients really do become our friends,” adds Cooney. In addition to the special-order cakes, A Sweet Memory, has a selection of seasonal cakes, mini desserts, cookie sandwiches and 521 Route 9 • other baked goods readily

available daily. Other creative creations include their cake pies; cakes topped with pie filling and decorated to look exactly like a pie. “Our baking is very seasonal,” Cooney explains.

Winter flavors includes a tasted snowball cupcake, which is a chocolate cake filled with marshmallow and chocolate fudge, frosted with coconut buttercream, rolled in toasted coconut, drizzled with chocolate and topped with marshmallows. Or their gingerbread scone with maple cream. Or the orange, cranberry Christmas scone with orange cream.

“My grandmother used to always have her ‘lady friends’, as she called them, over for tea,” Cooney explains, adding that she wanted to honor her grandmother by adding a high tea to her bakery. “We squeezed one table in our previous location and began hosting afternoon teas.” They were all so well-received Cooney has expanded her operation to include a full-time tea room. Decorated with a lovely mix of china and flowers, the tea room serves tea, lunch and snacks from 11 a.m. to 4 p.m. Wednesdays through Sundays. They also offer a high tea, but reservations are required.

“Our clients have really created the charm of this place,” says Cooney, who adds almost of her china, platters and place setting have been donated by her customers. High tea ($27.95) is served on tiered platters and begins with an individual pot of tea, chosen from their selection of more than 50 teas.

Followed by seasonally inspired salad and homemade soup, four varieties or tea sandwiches, freshly baked scones served with their specialty creams, and completed with a selection of their miniature desserts. “Everything from our meats to spreads, our dressings and chutneys, our soups are all made fresh in-house,” Cooney says.

Cooney also places an emphasis on healthy fare. “We source organic whenever possible and offer a large selection or vegetarian friendly dishes,” she says. With the extra space the tea room has added to A Sweet Memory Cake Shoppe and Tea Room, Cooney is able to host special events. “We’ve done cake-decorating classes and other ladies events,” Cooney says.

They’ve also hosted princess teas, American Girl teas, unicorn teas and more. They even carry a line of tea geared towards little ones. “We even have a dress up section that ladies and little ladies love,” Cooney says.

A little help from above has turned Cooney’s one-time hobby into a business that brings customers joys bite by bite.

The business that Cooney started out of her home has grown into a full-time operation with a staff of eight. “We work hard and we work a lot, but we really do love each other,” Cooney says.

She says they have become a great team and she couldn’t do all of this without them. Cooney’s brother-in-law, Paul Cooney, recently joined the team as chef and baker. There’s also Sarah Rod, the head decorator, and Cindi Reider, head fondant who works in fine detail. “Sarah and Cindi are so talented, they really are the best around, they’re artists,” Cooney says. Together, what they create can easily be considered art, but at the end of the day-or party or wedding or afternoon tea- and after the last bite, their masterpiece has been turned into a sweet memory.

Fiduci-what?

All trust companies, including Garden State, are held to the “fiduciary standard,” but we don’t hold the monopoly on fiduciary services.

In May, the Securities and Exchange Commission (SEC) removed the word “fiduciary” from the mandated language registered investment advisors (RIAs) use in their customer relationship summary, concluding the word is “legal jargon.” Based on survey feedback, they decided the required RIA disclosure language to be: “When we act as your investment advisor, we have to act in your best interest and not put our interest ahead of yours. At the same time, the way we make money creates some conflicts with your interests. You should understand and ask us about these conflicts because they can affect the investment advice we provide you.”

The first sentence of the new disclaimer refers to the first and foremost requirement if your advisor is a fiduciary, the duty of loyalty, or putting the client’s interests above their own.

Trust Companies act not only as a fiduciary, but also as a trustee, which means that in order to fulfill our duty of loyalty to our clients, we have many other specific duties we are legally obligated to perform on behalf of our clients.

Here are some of the more common duties a trustee owes its beneficiaries: • Duty to administer a trust by its terms. Every trust agreement should make plain the purposes of the trust, as they provide the critical benchmarks for evaluating the trustee’s actions.

• Duty of skill and care. A high standard of performance is required, even if an amateur is named who has no prior experience as a trustee.

• Duty to give notices. Notices may concern legal rights of the trust beneficiaries, such as a power to make withdrawals, or they may cover such ministerial matters as designating a successor trustee or an agent to assist in trust administration.

• Duty to account. A written accounting of the assets, liabilities, receipts and disbursements of the trust must be provided to the beneficiaries regularly.

• Duty of impartiality. The trustee must not favor one beneficiary over another, unless the trust document directs that providing for a particular beneficiary is a principal purpose of the trust.

• Duty to invest. Trust assets must not be left idle. In addition to making the trust investments, the trustee has a duty to diversify the investments and develop an asset allocation plan. This is a job for professional investors or corporate fiduciaries.

• Duty of confidentiality. Normally, the terms of a trust, the identity of its beneficiaries and their respective interests, and the nature of the trust assets cannot be disclosed to anyone except the beneficiaries and those who need such information in order to be able to administer the trust.

The word “fiduciary” may be legal jargon; however, we take pride as a trustee in our duty of loyalty to our clients and being legally bound by it. The new disclosure language switch doesn’t preclude RIAs from using the term “fiduciary,” which we believe many will continue using to differentiate themselves, as it is the highest legal standard that a financial service provider can be held to.

An Expert in Offering Concentrated Expertise in All Areas of Colon and Rectal Surgery As a board-certified general surgeon and proctologist, Dr. Nina Paonessa believes in patient-centered care and preserving each patient’s independence, well-being and dignity. Specializing in colon and rectal surgery, Dr. Paonessa has been in practice for over 15 years. She completed her undergraduate education at Siena College in Loudonville, NY with a Bachelor of Science in Biology, Magna Cum Laude, and a member of the honor society, Alpha Kappa Alpha. After completing her undergraduate studies, Dr. Paonessa completed medical school at Lake Erie College of Osteopathic Medicine in Erie, PA and completed her colon and rectal surgery fellowship at Lehigh Valley Hospital in Allentown, PA.

CW: When did you first decide to become a surgeon?

Dr. Paonessa: I discovered my love for the specialty during my second year of general surgery residency. I began to notice how patient outcomes varied based upon the training/skill of the surgeon. Colorectal surgery is a field in which specialized training truly impacts patient outcome.

For example, in certain cases, the difference between patients having a permanent colostomy versus bowel continuity after colorectal resection is dependent upon the skill/training of the surgeon. Additionally, I had the privilege of doing a one-month rotation on the colorectal surgery service of Dr. Douglas Wong at Memorial Sloan Kettering Cancer Center in New York City. It was here that I truly fell in love with the specialty of colorectal surgery.

CW: What do you like the most about being a colorectal surgeon?

Dr. Paonessa: For me, the most rewarding part is when I can make a positive impact on a patient’s quality of life and well-being, whether it be curing a patient of colon or rectal cancer and restoring bowel continuity, to ridding a patient of their hemorrhoid symptoms, to freeing a patient from the social isolation caused by their symptoms of fecal incontinence.

A fringe benefit of my specialty is that I have the privilege of meeting so many wonderful and unique individuals. It is truly rewarding working with each of my patients.

cases in which the cancer can be cured, dealing with a diagnosis of cancer is stressful and at times, life altering. I truly feel for the patients and try to make the difficult road ahead of them a bit easier.

CW: How would you describe your practice’s philosophy?

Dr. Paonessa: In my practice, my staff and I share the same philosophy of “the patient comes first.” No matter whether it is benign or malignant disease for which a patient is coming to the office, we understand that it is a stressful and oftentimes an embarrassing situation for them. In my office, each patient’s privacy and dignity is preserved and respected to the utmost degree. We realize that just because we take care of colorectal problems every day, does not mean that patients are used to dealing with these issues.

CW: What are the most common reasons why a patient would schedule an appointment at your practice?

Dr. Paonessa: Men and women of all ages schedule appointments at my office for many reasons. These include hemorrhoids, fissures, colorectal cancer screenings via colonoscopy, treatment of colorectal cancer, squamous cell carcinoma of the anus, fecal incontinence and pelvic floor dysfunction.

CW: Tell us a success story that you have experienced over the years.

Dr. Paonessa: We have had many success stories over the last 15 years. One of my favorite success stories is that of a woman who was hospitalized for perforated sigmoid diverticulitis with a large intraabdominal abscess. I was requested to see her as a second opinion as the first surgeon proposed immediate surgery consisting of sigmoid colon resection and end colostomy. After careful history and physical exam and review of her CT scans, I proposed a plan of bowel rest, IV antibiotics and percutaneous drainage of the abscess. This allowed for the abscess to reduce in size to merely a small cavity. The patient was eventually taken to surgery, during which she had a sigmoid colon resection and colorectal anastomosis, without even a temporary stoma (bag). She recovered uneventfully and was able to return to work.

CW: describe some of your favorite leisure activities.

Dr. Paonessa: Outside of being a surgeon, I enjoy spending time with my family. I also enjoy cooking and trying new recipes, reading, dancing and spending time at the beach.

CW: What is the most difficult part of being a colorectal surgeon?

Dr. Paonessa: Explaining to a patient about their diagnosis of cancer is the most difficult part of being a colorectal surgeon. Even in many

My job is all about giving patients the best care and helping them feel as comfortable as possible. Part of my routine is taking their vitals and overall health history. I also assist Dr. Paonessa with different procedures and help to ensure our patients have the best overall experience while visiting the office.

CW: What is the most rewarding part of your job?

I really enjoy interacting with the patients and meeting new people and getting to know them. It feels great to be able to assist them with their questions and concerns while visiting Dr. Paonessa. At our office, patients are considered family and we go above and beyond as we would for our own families, and that is the most rewarding feeling to me.

Allison Bambino, Certified Medical Assistant (CMA) CW: What do you find most challenging about your position?

I really enjoy all aspects of my position. I look forward to gaining new responsibilities as I learn and become more experienced.

CW: What is a typical day like for you?

The first thing I do when I arrive is check the schedule for the day. I set up the patient’s chart and exam room depending upon what they are coming in to see Dr. Paonessa for. This could be preparing paperwork for an outpatient procedure or setting up the exam room for a minor procedure in the office. I make sure that every instrument Dr. Paonessa needs is ready and I assist her with the procedure in the office. Other days, I am entering the patient’s vital signs, health history, and medications into their chart.

CW: What is the age of the typical patient?

The age varies depending upon the reason for their visit. The typical age for a colonoscopy is between 50 and 75 years old. The typical age for a patient with incontinence is usually 60 and older. There really isn’t a typical age for patients with hemorrhoids, pilonidal cyst/abscess, fissures, fistulas, or thrombosed hemorrhoids. We’ve had patients as young as teenagers and as old as 90 years old come to our office.

The reason I chose this profession was to help others and be of service in my small place in the world. The most rewarding part of my job is interacting with and helping the patients who enter our doors. Another huge bonus is that I get to work with inspiring women everyday who encourage and help each other. We have created an amazing team and a beautiful little family.

CW: What sets Dr. Paonessa apart from other colon and rectal facilities?

Although I cannot speak of other practices, I can write of the amazing woman I work for. “She burned too bright for this world.” These are words written by Emily Bronte. They are words that I have read hundreds of times, and they come to mind when I think of Dr. Paonessa. Her compassion and kindness for others knows no bounds. It is her intelligence and fierce determination that continue to astound me. Finally, her heart of gold does not seem of this modern world or of this hurried time.

Myths, Darn Myths, and Mixed Messages

“T

he great enemy of the truth is very often not the lie, deliberate, contrived and dishonest, but the myth, persistent, persuasive and unrealistic.” Truth be known, when it comes to our health, it is important to study the facts to understand how behaviors or change of behaviors may benefit us and those we love.

The healthcare industry is constantly making new breakthroughs and reporting important facts on studies. However, the Internet, testimonials by friends and neighbors, and marketing gimmicks have put out a ton of misinformation about exercise. And it may be difficult to discern. We all want to be in our best possible health and shape, and when we read something that “makes sense” or seems possible, we are likely to believe it even if there is no science to back up the claims. Let’s take a few moments to debunk some commonly held exercise myths.

Dr. Nina’s What You Need to Know: To Debunk Exercise Myths Myth: I can exercise my way out of a bad diet From a practical perspective, exercise, while critical to a healthy lifestyle, is never going to be an effective way of losing weight, unless you have the training schedule—and the willpower—of an Olympic athlete (note: the vast majority of Olympic athletes do not follow a bad diet).

In order to burn 500 calories, a 160-pound adult would need to walk at 3.5 miles/hour for 80 minutes; jog at 6.5 miles/hour for 40 minutes; cycle in spin class 39 minutes; mow the lawn 60 minutes; swim laps 53 minutes; or Zumba 50 minutes. It is an awful lot more than most people realize! And brings some credence to my saying that a moment on your lips can mean an eternity on the treadmill or bike.

Myth: Spot training will give me that six-pack Doing a thousand crunches a day is likely to burn calories and make your abdominal muscles sore and toned, but it does not necessarily translate to seeing those “rock-hard abs” we so desire. Although it seems logical, spot training, or exercising one part of our body, with the hopes it will look amazing may leave us disappointed. Unless the fat that lies above those muscles disappears, we will not be able to see those toned muscles. In order to get that “six-pack,” we need to burn fat throughout the body via cardiovascular exercise and a balanced diet.

It should be noted that heavy sweating can cause you to become dehydrated and, as a result, force your kidneys to conserve water and hang onto toxins. So the next time you know you will be breaking out a sweat, take a bottle of water with you.

Myth and Truth: You must stretch before you exercise

There is no scientific evidence that static stretching—defined as stretching while you stay in place such as bending over to touch your toes or pulling your ankles towards your hips—before a workout can prevent injury or decrease delayed-onset muscle soreness. In fact, some studies have shown that static stretches lasting longer than one minute can strain your muscles and slow you down.

On the other hand, dynamic stretching, such as swinging your arms or walking lunges, increase your range of movement and blood and oxygen flow to your muscles prior to exertion. This may help not only improve performance, but decrease your risk for injury.

And the truth be told, it can be challenging to navigate through all the information, tidbits, and advice that is thrown our way. Separating fact from fiction is key to finding the best workout regimen for our lifestyle and body type.

Myth: You sweat toxins out of your body Our body has effective, built-in toxin elimination mechanisms via our liver, kidneys, and gut—not by sweating. The primary function of perspiration is to regulate body temperature. When our body is hot, we sweat and the moisture makes our skin feel cooler; when it evaporates into the environment, it takes with it the heat. Sweat is mostly comprised of water (99%) and a small amount of salt, proteins, carbohydrates, and urea. “Trace,” or minuscule, amounts of toxins may be detected in sweat, but they get rid of less than 1% of the body’s total toxin content.

She has been a registered dental hygienist since graduating Hostos Community College with an A.A.S. Corrine loves educating and interacting with her patients. The most rewarding aspect of her job is teaching her patients how to improve their oral health and, in turn, their overall health. Girija Surya, MD

Girija Surya is an American citizen by naturalization, was born in India and currently resides in Monmouth County. Dr. Surya attended Stella Maris College and Madras Medical College, where she received her medical degree in 1975. In Medical school she received awards and merits in Anatomy, Physiology, Organic Chemistry, Internal Medicine and Preventative Medicine. In the USA, she started her residency in surgery with a desire to ultimately train in Obstetrics and Gynecology. Her exposure during training, particularly in Cardiac and Vascular surgery, gave her a new passion. She went on to complete an additional 3 year fellowship in cardiovascular and thoracic surgery and got certified by the American Board of General Surgery and Cardiovascular and Thoracic surgery.

She has been practicing in Monmouth County since 1994. She has served on the faculty of cardiac surgery in Robert Wood Johnson University Hospital from 1987 - 1990 and subsequently with Thoracic and Cardiovascular group of N.J. at St. Michael’s Medical Center in Newark, N.J. She has two successful Vein Centers, where she treats Vascular disease (both arterial and venous), and also educates women on other connected problems such as diabetes and hyperlipidemia.

Dr. Surya has a daughter and a son.

She loves biking, dancing, tennis, and watercolors.

We found our new home this summer at Bethel AME Church, located at 3 Waterworks Road, Freehold Boro. We tried it out this summer and Reverend Sparks invited us to stay and we couldn’t be happier. Seniors in Monmouth County can join us every Tuesday from 10:30AM-2:30PM at the best place in the neighborhood for seniors age 60+ for Socialization, Fun, Friends, Conversation, and Lunch & Coffee. We will be expanding our program to include speakers, crafts, movies and exercise. Don’t miss these upcoming special programs that will also take place at Bethel AME Church. (And an added bonus, there is plenty of parking!)

Women’s History Mary Whiton Calkins

The early stages of psychology are most associated with the name Sigmund Freud, who is known as the founder of psychoanalysis, otherwise known as therapy. However, approximately 20 years ahead of Freud, Mary Whiton Calkins was Feministvoices.com. born to Charlotte and Wolcott Calkins. Mary was the oldest of five children and raised in a religious, disciplined household. Mary was raised between Buffalo, New York and Newton, Massachusetts.

Mary was educated and entered into college not as a starting “freshman” but a sophomore. Mary began her undergraduate studies at Smith College. As most young adults who begin their college education, we are influenced by our environment or experiences. Mary was no different.

Mary had a younger sister who passed away in the spring of Mary’s first year of college. It is reported that the death of Mary’s sister had a significant impact on Mary, which led her to study the field of psychology.

After taking a year away from college, Mary returned to Smith College and graduated with degrees in Classics and Philosophy. Mary’s family took an 18-month trip through Europe. When Empathysites.com the family returned to the United States, at the guidance of Mary’s father, Mary interviewed for a professor position at Wellesley College. It was at Wellesley where Mary also began her teaching in the field of psychology on the condition that Mary needed to obtain the appropriate training in the field. Although Mary had received offers from high-level institutions, she wanted to remain close to her family, which led Mary to Harvard University. At the time, the institution did not recognize women as students; however, they could partake in what was known as “graduate seminars.” Mary was recognized as “a faculty member from a college seeking post-graduate instruction.” Mary began her “seminars” with the foundations of psychology and also experimental psychology, which was a field of interest for Mary. Mary is most notable for her research related to the study of dreaming. After one year attending seminars at Harvard, Mary returned to Wellesley and is known to have created the first experimental psychology laboratory at the all-female college. Mary again returned to Harvard and petitioned to be recognized as a student, which she was denied but was accepted to return again as a faculty member participating in seminars. At Harvard, Mary had the ability to work with the newly appointed head of psychology, Hugo Munsterberg, who recognized Mary’s intellect, and she made an impression on him. Hugo approached the head of Ubysses.ca

Harvard to request that Mary be accepted into the PhD program at Harvard as she was the most impressive student and most qualified to support him with his work in experimental psychology. Suffice to say Mary was once again denied. Mary continued to work with professors Archive.org at Harvard and even participated in a doctoral examination, which she passed, and which the university refused to acknowledge.

Archive.org It is reported that in 1902, Radcliffe College, which was an undergraduate college associated with Harvard University, attempted to offer Mary and three other women doctorate degrees and Mary refused. Then again, 25 years later, a pool of influential alumni again petitioned Harvard to grant Mary her doctoral degree, which again was denied. Even in 2019 it is reported that Mary has not been granted her official doctorate from Harvard University. It is known that there have been petitions created by educational institutions in with students attempting to advocate for Mary’s hard work and contribution to the field of psychology and have not had success.

Mary is known to have developed the concept of self-psychology, which is the study of understanding the “self ” and conscious awareness of experiences, drives and behaviors. Mary has published many textbooks and writings associated in the subject areas of psychology and philosophy. Mary never let the rejection of Harvard University hold her back. Mary worked hard in the fields of philosophy and psychology, continuing to study behavior, dreams and the environmental effects on behavior.

Mary’s contributions to the field of psychology made a significant impact during her tenure as a “seminar” attendee at Harvard University. So much so that her instructors and fellow students respected her work in the field, which led them to reflect on their own self-awareness and determined that she should have been considered as an equal and attempted to advocate for her right to receive a doctorate degree.

Mary remained close to home and resided with her family until her death in 1930.

I am sure that it is not often that we take a step back and truly reflect on our own self-awareness and how our environment and experiences may shape our behaviors.

As we begin our journey toward the end of another year, I encourage everyone to take a moment and contemplate their own self-awareness and to enjoy some muchneeded self-care!

Rita King, LCSW is the Associate Director of Justice Involved Services at Jewish Family Service. In this role, Rita currently supervises a Re-Entry program for incarcerated individuals as well as a community based diversion program for consumers experiencing mental illness and involvement with the criminal justice system.

Rita is a “De-escalation for Individuals with Special Needs” training facilitator and has trained approximately 1,200 law enforcement professionals to date. Rita is also a CIT trained mental health professional. Rita has a strong passion for women’s history and in her free time she continues to raise awareness of the importance of women’s history through lectures and community speaking engagements.
