SUMMARY OF MOTIONS MINUTES OF MEETING CORY LAKES COMMUNITY DEVELOPMENT DISTRICT

A Regular Meeting of the Cory Lakes Community Development District’s Board of Supervisors was held on Thursday, November 17, 2016 at 6:00 p.m., at the Cory Lake Beach Club, 10441 Cory Lake Drive, Tampa, Florida 33647. On MOTION by Dr. Spiro and seconded by Mr. Castillo, with all in favor, the October 20, 2016 Regular Meeting, as amended, and the October 28, 2016 LAF Committee Meeting and October 24 – November 15, 2016 Sunshine Board Meeting Minutes, as presented, were approved. On MOTION by Dr. Spiro and seconded by Mr. Castillo, with all in favor, the Universal Engineering Sciences proposal, to provide geotechnical services for the reconstructed stormwater structures, in a not-to-exceed amount of $3,240, to be paid out of Capital Outlay, was approved.

Board of Supervisors Staff Directives Facilities Manager • Review recommendations from the LAF Committee and determine which ones are temporary versus permanent issues. Bring any concerns about LMP to the Board. Notify LMP of areas that need mulch. • Clean the letters on the Cross Creek Boulevard sign. • Secure proposals for the kitchen refurbishment. Contact Kamir to provide an assessment and recommend contractors. • Coordinate with Mr. Shah to secure quotes for a gym, gym plus a basketball court and gym plus a full court gym and classes, to compare to the gym expansion proposal. Provide information to the Sunshine Board. • Send an e-blast to residents about the resident tree pruning. • Speak to Envera about installing an override button, versus moving the entire visitor gate. • Map all sidewalks that have a change in elevation of .5” and mark the seam at the closest address with orange paint. Use the unit amounts in RB Concrete’s proposal and provide a comprehensive list of sidewalk repairs. Negotiate with RB Concrete on the St. Croix and Cachet prices. • Speak to Kolter and Ryan Homes about paying for lighting the Capri Isle monument. Office Manager • During office hours, when a private event was held, post a “Private Event – Community Office Matters Only” sign. • Send an e-blast on the Christmas holiday lighting contest information before Thanksgiving. Email the back page of the newsletter that had the prize information. • Allow for the ability for individuals to create topics on the Sunshine Board. District Engineer • Examine speed tables in Tampa Palms, Compton Park and USF. District Manager • Place consideration of open seat and presentation from candidates on December agenda. • Do not put appraisal for vacant land in future agenda packages. • Work with Mr. Hall and A.J. Forbes on lines of communication with the Security Committee. • Secure proposals for the gym expansion and kitchen refurbishment. • Send a letter to the 25 owners in Reclinata tree maintenance program and request a response. • Invite Mr. Jason Hamilton, of Alert to the next meeting. • Mark a map of areas that need tree replacements and provide to a nursery supplier or wholesaler. • Include summary motions and staff directives in all agenda packages under Minutes. • Speak with District Counsel about Mr. Hall’s incident with the homeowner to see what legal action the Board could take. • Provide a current P&L to Dr. Spiro. • Research solar lights.

CDD News STREET PALM TREE MAINTENANCE The CDD would like to inform you the process has begun to take over maintenance of the palm trees along the roadways between the curb and the sidewalk throughout the community. As part of this process, LMP will be removing the grass around the base of the trees and installing a ring of mulch to help with the fertilization application. The actual trimming of the trees will begin in January after the holidays are past us. Trees will be trimmed in January and August as part of the bi-annual schedule and then again as necessary to keep in-line with the POA Policy. This tree trimming and fertilization service is at no cost to the homeowner. The only time the homeowner may incur a cost is if the tree dies. Then the resident may choose a tree from the 3 approved palm trees that will have the best chance of survival depending on the cause of the original trees demise. In some cases, the resident may opt not to (or it may not be viable to) place a new tree in the location. If you have any questions or concerns regarding this, please contact the Beach Club (813-986-1031). MORRIS BRIDGE EXIT SWING ARM NOW OPERATIONAL We would also like to notify you that the swing arm on the exit at Morris Bridge is finally operational. The swing arm is in place to try and stop visitors from entering the community through opened exit gates after someone leaves. The operation of the swing arm will only slow down those who like to squeeze through while the iron gate is still opening. This practice of 'squeezing through' opens the person up to vehicle damage and damage to the community gates. This is just another step the CDD is taking to keep the residents of Cory Lake safe. BARCODE TRANSITION We have been operating under dual barcode systems for gate access during the transitional security change. However, as of JANUARY 31, 2017, WE WILL ONLY BE OPERATING UNDER THE ENVERA SYSTEM. Although we have been working to correct and configure the barcodes to one system, there may be some that have been missed. IF YOUR BARCODE DOESN’T ALLOW GATE ACCESS, PLEASE CONTACT THE BEACH CLUB IMMEDIATELY WITH THE BARCODE NUMBER! We will review the records and update accordingly. FINGERPRINT SCAN/ BIOMETRIC SYSTEM Also starting in January is our NEW FINGERPRINT SCAN SYSTEM for gate and amenity (gym/pool) access. We are updating equipment specifically

for our new security company, Envera. All resident’s fingerprints MUST be rescanned! We are unable to transfer original prints to the new system. There is not any new paperwork to complete, just bring your driver license for resident verification; this is a very quick process. A new feature with Envera is a fingerprint reader/scanner at the Resident Gate entrance at the Morris Bridge AND Cross Creek gatehouses. This allows a resident to enter the community through the resident gate entrance using their fingerprint if you are in a different vehicle (rental car, limo service, friends/family car). It will eliminate the need for a long wait in the guest lane or several issued ‘temporary’ barcodes. CHECK YOUR EMAIL FOR WHEN THIS PROCESS IS EFFECTIVE. WE WILL HAVE EXTENDED OFFICE HOURS TO ACCOMMODATE RESIDENT WORK SCHEDULES. •

What is it? Neighborhood Watch is a group of volunteer citizens coming together in a community, communicating with each other and the police to reduce crime in the area in which they live. We are the citizens behind the “eyes and ears” of the police. How does it work? Neighbors watching out for neighbors. Just follow your normal routine. If you bicycle, walk, jog, keep an eye out on your neighborhood. If you see anything suspicious, call the TPD first, then notify your Neighborhood Watch Coordinator, Sheryl Springer. An alert will be cascaded throughout the NHW team, and to the office personnel. We have one of the safest neighborhoods in New Tampa. With your involvement and awareness, it will stay that way!
