IMPORTANT: Documentation Log Submission When your students turn in their documentation logs to you by the requested date, please check for a few things before you submit the logs to the OFE. 1. Must be an ORIGINAL copy; photocopies of the documentation log will not be accepted. 2. Must have an ORIGINAL signature for each entry/date. 3. Must include ALL ten hours of field experience. Please count up the hours to make certain. **If a student/students do not meet these three main criteria, please return to log back to them to make certain that they either a) give you the original, b) get the needed signatures, and/or c) go and observe for the remaining of the ten hours of field experience.

Name Badges •

Field experience name badges will be printed out for each student.

The Office of Field Experience (OFE) will print out and distribute these name badges to the instructors. Expect the name tags by the 17th class day. o The OFE will keep the name badges of those students who have not applied to the Ed Prep Program. These students are not allowed to complete field experience until they have applied to the Ed Prep Program.

Students who receive name badges without a picture must go the Bearkat One Office (Estill Building, Suite 330) for a picture to be taken. The Bearkat One Office may also be reached at: (936)294-2273. Once a picture has been taken, the OFE will print the student a new name badge.

Students are required to wear this name badge during all their field experiences. o Example of name badge on the following page.

Lanyards may be purchased by the students at the University Book Store for under $2.00. o Students may also use lanyards from conferences, etc.

Black and white copies of name badges will also be distributed to the instructors for reference.
