REGISTRATION DATES: Monday, June 16, 2014 – 9:00 AM – Noon Tuesday, June 17, 2014 – 9:00 AM – Noon ***NO LATE REGISTRATION***

REGISTRATION PROCEDURE 1.

A mandatory letter on school letterhead from the school counselor stating the deficiencies and the courses in which the student is to enroll.

2.

Parent/Legal Guardian must be present at the time of registration. Full tuition ($175.00 per class) must be paid at the time of registration.

3.

Tuition must be paid in CASH, CERTIFIED CHECK OR MONEY ORDER. NO PERSONAL CHECKS.

Period 1 (7:30 AM – 10:15 AM) Pre-Algebra Essential Math Algebra I Geometry Biology Chemistry English I English II Early World History U.S. History Spanish I & II Theology I – IV World Studies Psychology Physics

Period 2 COURSES:

(10:30 AM – 1:15 PM) Algebra I Geometry English III English IV Modern World History U.S. History Physical Education Algebra II / Trig General Science Health Geography Sociology Environmental Science Earth Science Physical Science

Credit: Each course is worth .50 credit. St. Francis de Sales High School will notify your home school that you have passed the course. Your home school will then issue you credit for the course.

1 class - $175 2 classes - $350 3 classes - $525 4 classes - $700

Dismissals:

Classes will begin on Thursday, June 19, 2014 First Session – Semester I

June 19 to July 10

Second Session – Semester II

July 14 to August 1

Final Exam Date – July 10th

CLASS TIME:

Final Exam Date – August 1st

Absences and Tardies Absences Permitted 1 per class OR (NOT BOTH) Tardies Permitted 3 per class IF EXCEEDED: Consequence – Dropped from the course with NO REFUND

Students may be dismissed from Summer School with NO REFUND of tuition for unacceptable behavior, including but not limited to absenteeism, punctuality, alcohol, smoking, drugs, gang activity, fighting, disrespect to staff, and disruptive behavior.

Bus Transportation:

The CTA South Chicago Bus #30 stops in front of the school at 102 nd & Ewing.

Final Exams: Students must be present on July 10th and August 1st and take the final exam in order to receive credit. Students who are not present on July 10th and August 1st will receive a grade of F with no refund.
