After nearly 11 years of service in the Flexible Benefits division of Risk Management, I am very excited to join the SDPEBA team! I have always enjoyed serving City employees and retirees and am honored to be able to continue to do so as the Benefits Manager for SDPEBA. This year, open enrollment will be very different from any other open enrollment we have ever experienced. With the limitations of social distancing in place due to the COVID-19 pandemic, all scheduled benefits fairs have been cancelled. Our goal is to help employees get the information and support they need to make important decisions regarding their benefits- and this year is no different. SDPEBA has been working extensively to find ways to provide an Open Enrollment experience that will keep everyone safe while still providing the high-quality support and service you deserve. And as a result, we are very excited to share with you what you can expect for FY21: • SDPEBA will be hosting a Virtual Benefits Fair (VBF) which will allow you to gather information and interact with the benefit providers safely from your home or office. • During the VBF there will be educational sessions provided from each medical plan (Sharp, Kaiser, and CIGNA), as well as other plans and programs. • A NEW identity protection program call Identity Guard is being offered to protect you and your family from identity theft, cyber bullying, safeguard your devices and more! • To help streamline and improve accuracy of enrollments, you can now enroll for MetLife Dental & Legal, Identity Guard and VSP online via https://enroll.sdpeba.org. Please know that SDPEBA’s commitment to serving goes well beyond open enrollment. Our team will follow-up with every new employee who signs up for a SDPEBA plan to verify that their cards have been issued/received, make sure they know how to access services, as well as let them know that “We got you” if they need any additional assistance or support with their benefits throughout the year. IMPORTANT ENROLLMENT INFORMATION HEALTH

SDPOA members can enroll in SDPEBA’s Sharp Health Plan, ALADS, Kaiser, or CIGNA plans through the flexible benefits program. All health plan enrollments are completed through the City’s SAP Online Enrollment System. Enrollment through the City’s system will start June 8th and end June 26th.

DENTAL & VISION

To enroll in the City’s Delta Dental plan and/or VSP plan, you must use the City’s SAP Online Enrollment System. Enrollment through the City’s system will start June 8th and end June 26th. If you would like to enroll in SDPEBA’s MetLife dental or VSP vision plan, you must enroll using the SDPEBA online enrollment system by June 30th. To enroll, please visit https://enroll.sdpeba.org by June 30th.

NOTE – If you select SDPEBA’s dental or vision plan, your premiums will be deducted through a post-tax payroll deduction. Enrollment in SDPEBA’s plan does not automatically cancel your City dental or vision plan, you must use the City’s online enrollment system to cancel those deductions.

AFLAC

Aflac plans are only available during open enrollment through a post-tax payroll deduction. If you are currently enrolled your benefits will automatically roll over, you do not need to re-enroll. If you want to add/change/term Aflac coverage, please contact our Aflac team at info@sdpeba.org.

SDPEBA offers SDPOA members access to our MetLife Legal Plan (Formerly Hyatt Legal) and Identity Guard our new identity protection plan. These plans are only available through a post-tax payroll deduction. MetLife Legal is only available during open enrollment, but Identity guard is open all year. WHAT IS SDPEBA? SDPEBA stands for the San Diego Public Employee Benefit Association and is a partnership between the San Diego Police Officers Association (SDPOA), AFSCME 127 (127), City of San Diego Retired Employees’ Association (REA), the Retired Fire and Police Association (RFPA), and the San Diego Municipal Employees Association (SDMEA). The goal of SDPEBA is to reduce costs, improve benefits, enhance support and stabilize premiums. SDPEBA is here to help! We are committed to providing quality customer service all year round to any employee or retiree who enrolls in any of our sponsored plans. Below is a list of the services we provide.

ENROLLMENT

PROVIDER REVIEW

Signing up for benefits can be confusing, but our team is here to help employees and retirees with enrollment questions via phone, email, text, or in person. We hold onsite benefit fairs throughout the City to bring materials and information to the workplace. We also work with the City to process the enrollment data to make sure everyone is properly in the system on their effective date.

Throughout the year we track questions and issues about our providers and obtain feedback from employees and retirees about how well our plans are performing. We then review this information regularly with our providers to identify trends or specific problems. This collaboration allows us to proactively work to make improvements to our benefit programs and the customer service experience throughout the year.

CASE MANAGEMENT

EDUCATION

In the event you experience a problem with one of SDPEBA’s sponsored benefits, our team will advocate on your behalf to address your issue. We will work diligently to make sure the insurance providers are following our contracts and providing the best possible service to our enrollees.

Signing up for benefits is just the start, getting the most out of them is key. Throughout the year we publish articles on the SDPEBA website that help educate employees and retirees about their benefits and how to use them. We also work to educate people on wellness and encourage them to participate in our Best Health Wellness program.

SUPPORT STAFF

24/7 SELF SUPPORT

One of the most important things we do at SDPEBA is providing support for the benefits we offer. We handle thousands of calls and emails throughout the year from retirees who need assistance with their benefits. It can be a simple call about an ID card, or a complex call about assistance with a referral. Every call is important to us!

SDPEBA has a 24/7 Self Support site at https://support.sdpeba.org. This site is built to allow retirees to access information through an automated system outside of our normal office hours of Monday through Friday, 9am to 5pm. The site is updated regularly with answers to many of the questions asked by retirees. If the answer isn’t there, the site allows you to submit a question and our team will respond the next business day.

You have the flexibility to choose any licensed dentist, in or out of the network, and still receive benefits. Your costs may be higher when you go to an out-ofnetwork dentist.

This plan requires you to pre-select a dentist who participates in the network in order to receive benefits. Each enrolled family member may select a different participating dentist, and has the ability to change dentists up to one time each month.

Specialty Care

No referral needed for specialty care.

Your selected participating dentist will provide you with the name of a network specialist. Just call that specialist to schedule your appointment. No pre-authorization is required.6 Any co-payment for services is listed on your plan’s Schedule of Benefits. This co-payment applies whether the services are provided by your selected participating dentist or by a participating specialist.

Access to Dentists when Traveling

When traveling within the U.S., you have access to one of the largest networks in the industry You may also use an out-of-network dentist.4

Because dental HMO / managed care programs are based on the use of defined networks, general dental care is not accessible while traveling. The only exception is in an emergency situation when you are unable to receive care from your selected participating dentist. Please refer to your plan coverage documents provided in your enrollment booklet for details.

Network Discounts

All participating dentists have agreed to accept negotiated fees as payment in full for covered services. These fees typically range from 30% – 45% less than the average charges in the same community.5

This plan provides access to hundreds of dental services that may be considerably lower than your cost would be without this plan.2

All participating dentists have to go through a rigorous upfront and ongoing selection and review process.7

This plan typically includes a yearly deductible and an annual benefits maximum. Please refer to the Plan Summary included in your enrollment booklet for benefits specific to your plan offering.

This plan is not subject to deductibles. Out-of-pocket costs for covered services are typically calculated based on co-payments as listed in the plan’s Schedule of Benefits. Please refer to your enrollment booklet for information specific to your plan offering.

Understanding Your Options: Many members have questions regarding the voluntary group life insurance that the City provides and how to purchase additional coverage. The following information is intended to help you better understand the difference between Group and Individual Life Insurance plans. Group Term Life Insurance: The rates for the City’s Group Life Insurance plan are based solely on age. Regardless of your gender or health, you are going to pay a rate that automatically increases every five years anywhere from 20 to 40% and can take place without notifying the policy holder. Based on this age criteria, a relatively healthy person is going to pay the same as an unhealthy person who is in the same age bracket. Individual Life Insurance: Individual Life Insurance plans are the most cost effective option for relatively healthy employees. In fact, individual plans can provide the employee with more coverage for less and save them large amounts of money. For example, a 40 year old with $200,000 group policy pays $22 a month with the rate increasing to $30 a month at age 45, $50 a month at age 50 and $90 a month at age 55. The same 40 year old can obtain a $500,000 individual life insurance policy for a fixed level 20 year rate of $31 a month. Not only will the healthy person get $300,000 more coverage, they will also save over $7,000 during this time.

5. Can I sign up anytime? No, the only time you can sign up for the Group Aflac plans is during open enrollment which is April1st - June 30th. New employees have 30 days from hire date to sign up mid-year. 6. Can I make coverage changes anytime? You can cancel anytime, but in order to make any changes mid-year, a qualifying event must occur. The most common example is marriage. 7. Can I keep my plans into retirement? Yes, you may keep them by using a pension check deduction. Contact SDPEBA for more info. 8. If I leave the City of San Diego, can I keep my plans? Yes, Aflac Group plans are portable. You can pay Aflac directly. Please contact SDPEBA for more info. 9. When I retire will my rates go up? Rates do not increase when you retire, but they do change from bi-weekly to monthly or annual deductions. 10. What happens to my plans when I am on unpaid leave? When you are on unpaid leave, your premiums go unpaid. Aflac does not send out invoices for missed premiums, and your plans can be terminated for non-payment. If you plan to go out on leave, or have missed any deductions, please contact SDPEBA to set up a payment plan. 11. If I change my address with the City, does it automatically get updated with Aflac? No, you must contact SDPEBA to get a change of address form. This is important so claim checks are not delayed due to an incorrect mailing address.

1. How do I file a claim? The best way to file a claim is to email Claims@SDPEBA.org. Our team will personally help you process your claim and will follow up with you until the claim has been reviewed and paid. If you do not get an immediate response, please contact us by phone. Our emailed response may have been stopped by your spam folder. 2. Is there a deadline to file claims? No, there is no deadline to file a claim. 3. Do I have a time frame to seek treatment for my injuries? Yes, you do but it depends on the type of plan you have. For example, the New Accident plan allows you to wait 7 days, but the old plan only allowed 3 days. We encourage you to seek treatment right away for all injuries even if it is minor. The minimum payout for this visit is $50. 4. How do I file a claim for an annual exam? Claims@SDPEBA.org: • • • •

5. What is needed to file a claim under the Accident Plan (injury claim)? This will vary depending on the type of injury and treatment. Simply email Claims@SDPEBA.org with the following info: • • • • • •

Name and DOB of person injured Date and how injury happened Diagnosis of injury (fracture, cut, burn, etc.) Date of first visit for injury If work related, did you go to Concentra or Sharp occupational? If you are with Kaiser, please provide Kaiser medical record number

Once you submit this information, we will organize your claim so the process is as simple as possible. 6. What is needed to file a claim under the Hospital Plan (illness claim)? This will vary depending on the type of illness. Simply email Claims@SDPEBA.org with the following info: • • • • • •

Name and DOB of patient Date symptoms started Diagnosis List of all dates related to illness Surgery? Medication? Hospitalization? If yes, please provide applicable dates. If you are with Kaiser, please provide their Kaiser medical record.

Once you submit this information, we will organize your claim for you so that the process can be as simple as possible. 7. What if I do not know what is covered or I have questions? We encourage all policyholders to email Claims@SDPEBA.org any time you see a doctor. Simploy say “I just went to the doctor yesterday for XXXX and wanted to see if I could file a claim.” We never want you to assume that something is not covered. It never hurts to ask, and we are always happy to assist you. Like most group benefit plans, it is important for you to understand the exclusions, limitations and the schedule of benefits. All of this info can be obtained by visiting SDPEBA.org or by calling 888-315-8027. 8. Can I get my payments direct deposited to my Account? Yes, direct deposit is available for all claims as long as you have direct deposit set up prior to your claim being disbursed. Please contact SDPEBA for a direct deposit authorization form. If you have already submitted the form, it will stay on file with Aflac until you ask them to remove it from their system. If you have a change in your banking information, you will need to send in a new direct deposit authorization form.

SDPEBA is launching a new online enrollment system this year to improve the enrollment experience, lower paper waste, and reduce errors. Employees can enroll online for SDPEBA’s dental, vision, legal, and identity protection plans. To get started, visit:

https://enroll.sdpeba.org You will need to register at this site and create an account. Once you have your account started you can begin your enrollment process. You will be able to add/term/change your SDPEBA legal and identity protection plans.

To access this new enrollment process you must have already had benefits with SDPEBA. If you have not enrolled in any of our benefits, you may not be in our new enrollment system. If while at https://enroll.sdpeba.org the system is unable to locate your account, please contact SDPEBA at 888-315-8027 or email support@sdpeba.org and our friendly staff can quickly add you over the phone, and grant you access immediately.

FREQUENTLY ASKED QUESTIONS The SDPEBA team gets thousands of calls, emails and other inquiries over the year. To help answer some of these questions we have developed a list of the top five most frequently asked questions.

1. IF I AM NOT MAKING ANY CHANGES, DO I NEED TO RE-ENROLL FOR MY FLEX BENEFITS?

It depends. If you are currently participating in the City’s Flexible Spending Account plan (Dental/ Medical/ Vision, or Dependent Care), then “YES” you will need to re-enroll to continue. For all other plans, “NO” you do not need to re-enroll. Although you are not required to re-enroll, employees are encouraged to review their benefits and verify dependents and beneficiaries (especially since the City is transitioning to a new Benefits Application).

2. WHEN ARE MY BENEFITS EFFECTIVE?

Open Enrollment changes for SDPEBA and City health plans become effective as of August 1st, and premium changes will be reflected in your 7/24/2020 pay (pay period ending 7/10/2020).

3. HOW DO I ENROLL IN SDPEBA’S METLIFE DENTAL OR VSP VISION PLANS?

You must enroll online via https://enroll.sdpeba.org. Please know that you cannot enroll into SDPEBA's dental or vision plans through the SAP. Important: These plans are only available to you through post-tax payroll deductions. C

4. IF I AM CURRENTLY ENROLLED IN THE CITY'S DENTAL AND/OR VISION PLAN, DO I NEED TO LET THE CITY KNOW I AM SWITCHING TO SDPEBA?

Yes- If you choose to enroll into SDPEBA’s dental/vision plan(s), be sure to cancel your City dental/vision plan(s) through SAP. Unfortunately, SDPEBA cannot cancel benefits on your behalf. If you need assistance with the City's SAP online system, please contact either your payroll specialist or Risk Management at 619 236-5924.

5. WHEN WILL I GET MY ID CARDS?

SHARP HEALTH PLAN - ID cards will arrive near 8/1/20. If you have not received your card by 8/15/20 please contact us to request a new card. METLIFE DENTAL PPO - No cards are sent out for this plan. You will get an email with instructions on how to use the plan on 8/1/20. METLIFE DENTAL HMO - Only new enrollees who have selected a primary care dentist will get an ID card around 8/1/20. If you have not received your card by 8/15/20 please contact us to request a new one. VSP VISION - No cards are sent out for this plan. You will get an email with instructions on how to use the plan on 8/1/20. METLIFE LEGAL - No cards are sent out for this plan. You will get an email with instructions on how to use the plan on 8/1/20.

6. HOW DO I FIND AN IN-NETWORK PROVIDER LISTING (FOR ALL PLANS)?
