The Rockford Park District welcomes and supports the participation of individuals with disabilities in all programs and services. • Consultation on activity modification • Disability awareness training • Behavior modification support • Observation and evaluation • Visual systems and sensory equipment/toys • Sign language interpreters • Adapted equipment (sleds, skate walkers, hand cranked bicycles, sport wheelchairs) • Varying levels of disability-trained staff support

popular programs in which individuals with disabilities have been included: • Lessons (swimming, hockey, ice skating, horseback riding, tennis, golf) • Day and overnight summer camps (Summer Blast, Nature Quest, Camp Lone Oak, Pony Camp) • Neighborhood programs (Playgrounds, Lewis Lemon and Washington Park community centers) • Environmental education and recreation • Youth sports • Rec nights • After school programs

Inclusion Services has created successful inclusion opportunities by: • Implementing a positive support plan for a child with a conduct disorder in a full day summer camp setting. • Providing an all-terrain wheelchair for a student with a physical disability for full trail access at Atwood Center. • Creating and training staff on an enhanced visual system for a camper with a visual impairment. • Trained staff supporting a camper with an attention deficit disorder by providing additional supports such as redirection and positive reinforcements at soccer camp. • Providing a sign language interpreter for a teen at horseback riding lessons.

A parent’s guide to inclusion Inclusion Services roles 1. Contact family/guardian to complete full assessment. 2. Obtain and organize any additional info, equipment, and additional support staff (if applicable).

How Inclusion Support works: 1. Register for desired Rockford Park District program at Webbs Norman Center, Carlson Ice Arena, or rockfordparkdistrict.org. 2. Check "yes" where the registration form states, "…does the participant have a disability or medical conditions that may require assistance or accommodation?" 3. A team member from Inclusion Services will contact you within 48 hours to assess needs, level of support, and an appropriate start date.

3. Notify program staff of participants’ needs and the staff’s role in supporting them in their program. 4. Communicate with family any updates, changes, or any information that they need to made aware of before the start of the program. 5. Maintain communication with parents/ guardians, support staff, and program staff, throughout the course of the program. 6. Recommendations will be addressed based on the participant’s progress or level of success in the program.

Parent/Guardian roles 1. Verify the participant meets minimum eligibility requirements of program (i.e., age, registration, and level of participation). 2. Keep in mind, recreation programs and activities are voluntary in nature. Participation will be encouraged and aided, but not forced. 3. Communicate openly with Inclusion Services staff, providing pertinent information on participant’s needs for successful inclusion. 4. Communicate absences to inclusion and/or program staff in advance. 5. Provide constructive feedback on participant’s progress and response to inclusion effort. Indoor opportunities will also keep you from getting bored when the temperature dips. Youth Group Golf Lessons

Have fun and make new friends while learning the basic fundamentals of golf (with an emphasis on FUN). Classes will cover putting, chipping, pitching, sand shots, and full swing with irons and woods. Lessons are offered at two locations. Golf course etiquette and care will also be emphasized throughout the entire program. Instructed by PGA members and apprentices. This is a class for golfers of all skill levels. The class will cover all the usual and most common rules, and will finish with the fun but sometimes confusing rules. The session ends with an open forum. Instructed by PGA members and PGA apprentices. Y18

Learn To Swim–Rolling Green Elementary School

Participants learn the fundamentals of swimming and ultimately, how to swim by themselves. On the first day of class, students are placed in the most appropriate of five levels. Children must be able to enter the water and participate on their own. Each level has a specific set of objectives to achieve before moving to the next level. Classes are taught in the main pool in at least four feet of water. All levels are taught in a positive and fun atmosphere!

Skill level doesn’t matter when it comes to NFL Flag Football. All participants ages 5 to 14 will receive an official NFL Flag Football jersey, a set of flags, trophy, NFL certificate, and field trip to an NIU football game. n the court, right in the middle of the action, is the best way for youth to learn the game of basketball. This program emphasizes

skill development, sportsmanship, and team spirit. Every participant plays at least half of every 50-minute game per week, with the first quarter of every game being practice. There is a 20-minute practice prior to the game. Pre-K –2nd grade players use junior sized basketballs and 8-foot standard hoops; 3rd–4th graders use intermediate basketballs and 10-foot standard hoops. • No full-court press is allowed. • We do not accept registration by complete teams. • We will send a confirmation e-mail the week before games start, which will list your coach and team information. • The first week of practice includes "Meet the Coach" and an introduction to the season. Games begin the following week. • No games will be played during Thanksgiving weekend and Rockford Public Schools’ winter break.

About our program:

• O ur programs use the nationally recognized US Figure Skating Basic Skills Program. Each participant receives two free passes for Public Skate, or 30-minute freestyle sessions. • Classes meet once a week, and skates are provided for all levels. • The minimum class size is six skaters. We reserve the right to combine classes with fewer than two skaters. • Skaters progress at their own rate, moving on to the next level only after required elements are mastered. All levels may be repeated until the requirements are met. The Rockford Park District reserves the right to move any skater between levels. • If you need to miss your regular class, you can make it up if it is offered another time during the week. If not, you can use one of your public or freestyle session passes to practice.

he Learn to Skate program provides a safe and exciting experience for the beginner as well as the more advanced skater. The program

allows participants the ability to achieve the skills necessary to either skate for fun the rest of their lives, or participate competitively. We offer a variety of program options, including: lessons, skill development, performances, and competitions. Easy to Register! Our beginner, intermediate, and advanced level structure makes our system easy for parents and participants to understand. Advanced Skate Lessons

This is a 15-minute practice followed by a 30-minute lesson for advanced skaters who are ready to put it all together and want to be introduced to all the elements in skating and possibly begin a competitive career. Basic 7, 8, and Freeskate 1-6 skill levels. Skill Focus: • Backward crossovers • Spiral sequences • Advanced jumps • Advanced spins • Combination jumps • Turns

Moving Beyond: Advanced Training in Private Lessons

The Bridge to Private Lessons At any time during the Basic or Freestyle levels of the Learn to Skate program, skaters may choose to implement private lessons in addition to group lessons, or to switch from group instruction entirely to one-on-one instruction with a coach. Private lesson fees are set by each individual coach. All private lessons take place during freestyle skating sessions, which is an additional cost. I f a participant has never skated before, what class should he/she sign up for? Our programs are designed to accommodate any person, of any age, who would like to learn to skate. Each participant is different, but we recommend the beginner start at the Chair Hockey or Rink Rats level. Our instructor can then determine whether he/she should be in a different level.

C an parents go out on the ice with their participant? Except for Chair Hockey, only students and instructors are allowed on the ice.  hat equipment will participants W need to play hockey? The Junior IceHogs lesson program provides free helmets and skates for participants to use. Chair Hockey and Rink Rats require protective elbow and knee pads. Ter-mites 1–2 and Learn to Play Hockey require hockey-style shin pads, elbow pads, and hockey gloves. Learn to Play Hockey participants also need their own stick. Hockey pants, or breezers, are recommended, but not required. S hould participants purchase pads and hockey gloves even if they are just learning? Hockey pads and gloves are recommended, but not required. We’ve found the more protected participants are, the less it will hurt when they fall, leading to increased confidence and best of all, more fun!  ow should participants dress? H It is recommended that participants wear sweatpants or athletic pants (shin pads go underneath them), a warm sweatshirt, and winter or hockey gloves. I f participants miss a session, can it be made up in one of the other classes? We do our best to accommodate. Simply talk to the instructor to give them a heads up! How many instructors per participant? We strive to keep our student/teacher ratio at 6:1. Several instructors are on the ice to help participants. Who are the instructors? What are their qualifications? Our instructors are all highly experienced kid-oriented, hockey-minded coaches.

Monday Madness

Don’t wait to sign up - limited spots are available! This one-of-a-kind camp includes a tour of the BMO Harris Bank Center including the IceHogs locker room, ticket to the game, and lunch. A light off-ice workout will be followed by an on-ice practice, all at the Center. Drop-off and pick-up will be at the box office at the BMO Centre. Native Americans were the first people to inhabit this area. Join us as we take a walk back in time to experience a bit of what life was like. Know what to bring on a hike? Know what to do if you get lost in the woods? This program will teach you basic survival skill that could keep you alive! Payment options Payment is required at the time of registration. We accept cash, local personal checks, credit cards (Visa, MasterCard, American Express, and Discover Card only) and debit cards on which the VISA or MasterCard logo appears. Please make checks payable to ROCKFORD PARK DISTRICT. If your check is returned for insufficient (NSF) or uncollected funds, your account may be electronically charged or drafted for the face amount of the check, plus the additional $25 service fee. The service fee added to all returned items is $25 or the maximum allowed by state law. Your payment by check is agreement to this policy and terms. You have the right to revoke this transaction. Automated payment plans are available for Rockford Park District programs. CURRENT CUSTOMERS Any customer who has previously registered for a Rockford Park District program can register for programs by completing one simple form! Participants no longer need to provide all of their basic household information every time they register for a program, and will be able to register more than one participant on the same Program Registration form (as long as that participant’s basic information is on file from a previous registration). NEW CUSTOMERS If you have never registered for a Rockford Park District program, you will be asked to fill out the Household Information form, as well as the Program Registration form. Once your Household Information form is on file, you can register for programs by simply filling out the Program Registration form. ALL CUSTOMERS If you register in person at one of our Customer Service locations (see left), a copy of your Household Information currently on file will be printed and presented for you to review. If everything is correct, you can proceed to register; if any of your information has changed, simply write the new information on the form and a Customer Service representative will update your file after completing your program registration. TITLE VI As the Rockford Park District has received financial assistance in the form of Land and Water Conservation assistance, the US Department of the Interior, under Title VI of the Civil Rights Act of 1964, prohibits discrimination on the basis of race, color, national origin, age, or disability. How do I know if I’m registered?

If you registered online, you will receive e-mail confirmation as soon as you have been registered in the program. If you submitted a paper registration form, you will receive a confirmation receipt through the mail listing all the programs for your family, and whether they have been enrolled in the class or placed on the waiting list. The confirmation receipt does not guarantee enrollment if the class does not fill and is cancelled. In the event your program is cancelled, you will be notified and your program fee refunded.

Waiting list policy

Some programs are very popular and fill quickly. You may opt to register for a filled program and be placed on the waiting list. To be placed on a waiting list, the program registration process must be fully completed, including payment. Your position on the waiting list is based upon when the registration is processed by the District. If the District cannot accommodate your enrollment by the second meeting of the class, your registration fee will be refunded in full. All waiting lists have a limited number of spots, so it is possible that the waiting list may already be filled at the time you attempt to register.

How do I know if I’m a resident?

Rockford Park District programs are open to both residents and non-residents of the Rockford Park District. People who are residents are paying property taxes that support the Rockford Park District, and they receive a discount on most Rockford Park District services. To determine if you are a Rockford Park District resident, you can review your personal property tax assessment to see if you are assessed by the Rockford Park District, or you can view a copy of the tax bill for your address at wingis.org. You can also call our office at 815-987-8800 during business hours to have us look up your address.

May I register for a friend?

In order to register for a friend, you must bring a completed registration form on which the Waiver and Release of All Claims and Assumption of Risk has been signed by an adult who is legally responsible for the participant.

Unless otherwise noted, recreation programs require participants to reach the minimum age by Jan 1, and not reach the maximum age before the conclusion of the program. Lesson programs require that participants be within the age limits of the class by the first day of the program.

Transfer policy

Transfers are allowed up until the beginning of the first class. Transfers after a program has started must be requested through the manager of the specific program. If the transfer is to a higher priced program, the additional fee must be paid prior to the transfer. If the transfer is to a lower priced program, a credit will be issued to the household.

What is the cancellation and refund policy?

Full refunds will be granted for any registration cancelled by the participant two weeks prior to the first day of the program. If the program you’ve registered for is cancelled, a full refund will be issued. If you feel that the program you or your child participated in did not deliver the service that it was supposed to, you are entitled to a Quality Assurance refund. All we ask is that you speak with the program manager to help them understand your dissatisfaction. Partial refunds may be issued for programs that you are unable to complete due to illness or injury (it is your responsibility to notify the District of your situation before the program ends) or that the District has to cancel due to inclement weather or conditions. Some programs reserve the right to issue promotional day passes or schedule make-up classes in lieu of cash refunds for weather-related cancellations. No refunds will be issued to participants who withdraw from a program less than two weeks prior to the first day of the program. These regulations apply to recreational and instructional programs throughout the Park District. They do not apply to daily admissions, memberships, permits, leagues and rentals. These services have specific policies as part of the information provided to the participant at the point of registration.

Refunds will be issued in the form of a credit to your household account. You may redeem your credit towards any future Park District registrations for any member of your household. If your household credit is more than $10.00, you may request a refund by calling Customer Service. Refunds will be applied to any outstanding balance owed by your household for programs, activities, memberships, leagues, or rentals. If there is no outstanding balance on your account, the refund will be issued in the form of a check or applied to your credit card, based upon the method of payment used at the time of registration. Allow up to two weeks to receive your check or for the credit to be posted.

Recreation Satisfaction Guaranteed!

Our goal is to provide our customers and citizens with recreational programs, events, and activities of the highest quality. When a customer is not satisfied, we sincerely request their comments, suggestions, or ideas for improvements. If a customer is not completely satisfied with a recreational program, event, or activity the Park District directly provides, the customer may tell us and we will arrange for one of the following options of their choice: 1) Cash refund on site (when possible, limited up to $25) 2) Refund by mail (within 2-3 weeks) 3) Refund to credit card (if payment was made by credit card) 4) Refund to household account Refund applications (Quality Assurance Forms) may be filled out in writing at the facility where the program, event, or activity is delivered, or at the Customer Service Office during business hours. Season pass refunds may be pro-rated, based on the length of use during the season of operations.

o child should be denied access to learning an important ability, such as swimming, or developing skills for a lifetime of enjoyment, due to their inability to pay. The Rockford Park District offers a fee assistance program called Fun For All Kids (FFAK).

FFAK Guidelines

If you require financial assistance and live within the Rockford Park District boundaries, your children may qualify for the Fun For All Kids financial assistance program. Qualified applicants can receive up to 75% off registration fees for Rockford Park District programs. 2012 scholarship funds may be redeemed now through January 31, 2013. After scholarship funds have been applied, participants are responsible to pay their portion of program costs at the time of registration. NOTE: Daily admissions, private lessons, and season passes are NOT available through Fun for All Kids.

Fee Assistance

Customers of all ages with disabilities who meet fee assistance guidelines and are residents of the Park District may be eligible for the fee assistance program.

FFAK Ticket Packets

Solutions for your Child Care Needs

The YWCA of Rockford administers a Child Care Assistance Program funded by IDHS. This program exists to help families of Boone, Jo Daviess, Stephenson, and Winnebago counties find child care and/ or pay a portion of the cost of their child care for children under the age of 13. Families must meet established guidelines. For questions regarding the Child Care Assistance Program, or to request an application for assistance, please call 888-225-7072 or 815-484-9448.

Qualifying FFAK applicants can purchase Fun For All Kids Ticket Packets at a discounted rate of $25 per packet. FFAK Ticket Packets provide individual admissions to a wide variety of Park District programs and facilities. Only one ticket packet may be purchased per child per year. Each ticket packet contains $190 in value to do these fun recreational activities: • 2–One-day Magic Waters passes* • 2–One-day Nicholas Conservatory & Gardens admissions* • 8– One-day pool passes to Alpine Pool or Sand Park Pool* • 4– Rounds of golf at Ingersoll* • 4–Small buckets of golf balls at Elliot or Learning Links * • 4– Ice Skating passes* • 1– Trial Ride pass* • 4– Pony or Hay ride passes* • 4–Forest Queen hourly-ride passes* * Children under age 10 must be accompanied by someone 16 or older Each ticket packet is assigned to the qualified child to use as admission to these Park District facilities. The District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. The program will combine the expertise of a Titleist Performance Institute Certified Fitness Instructor with the knowledge and skill of a PGA Professional. The classes are certain to equip the serious golfer with the training they need to achieve optimal physical performance, as well as gain valuable tips to improve their golf game. Seth B. Atwood Park is a diverse 334 acres complete with prairie, woodland, river, and wetlands. Each of these areas needs special attention, and you can help! Join Environmental Recreation and Education (ERE) staff on select Saturdays and learn a little about one of these areas.
