our Leadership At Medical Home Plus, Inc. (MHP), we educate caregivers as they navigate complex systems on behalf of students with disabilities. Our trained advisors work directly with families and the schools to collaboratively identify appropriate educational and behavioral supports, thus making a child’s academic progress more accessible. MHP’s expert educational advisory staff participates in IEP (Individualized Education Plan) and 504 meetings to ensure student needs are clearly understood and addressed, and when the time is right, we plan for adulthood by supporting career exploration and independence. In 2025, MHP worked with over 230 school teams throughout Virginia to strengthen caregiver voices and honor the individuality of students receiving special education services. This impact is possible because of partners who believe strong communities require access, equity, and informed advocacy. Supporting MHP aligns your business with practical solutions and measurable outcomes for students with disabilities. Our theme for this 15th Annual Uncork Hope Fundraiser, MHP Gives Opportunities, reflects the doors that open when children receive individualized guidance geared towards long term success.
