REFUND POLICY

1. Online – https://apm.activecommunities.com/mhrd. Register online 24 hours per day/7 days a week with a Visa, MasterCard or American Express. A convenience fee is charged for use of this service.

A full refund will be issued when a program is cancelled by the Recreation Department. Cancellations made five working days or more prior to the scheduled start date of an activity will have a $5.00 administrative charge deducted from your refund. Refunds or credits will not be issued if you cancel out of an activity less than five working days from the program start date. No refunds will be given once a class or program has started. Please note that online convenience fees are non-refundable.

Mail-In – Fill out the registration form on page 19 (or download a form from mounthorebwi.info) and return with payment to: Mount Horeb Recreation Department, 222 East Front Street, Mount Horeb, WI 53572. *Make checks payable to Village of Mount Horeb. 3. W  alk-In – Register in person at the Recreation Department during normal business hours, 7:15 AM-4:30 PM Monday through Thursday and 7:00 AM-12:00PM on Friday. Office hours are subject to change. A drop box is located outside near the front door for your convenience.

REASONABLE ACCOMODATION REQUEST If a reasonable accommodation is necessary to participate in a program, please contact the Recreation Department at 608-437-3400. Every attempt will be made to accommodate the request.

ON-SITE REGISTRATIONS Instructors/coaches will NOT be allowed to accept registrations or payments at program locations. All registration transactions must be completed at the Recreation Department office or online prior to class start date.

RESIDENT/NON-RESIDENT A resident is anyone who resides within the Mount Horeb Village limits. All others are considered non-residents, and will be charged an additional $10.00 to the resident rate. This charge is assessed per person, per program. This additional non-resident fee helps with equipment expenses and staffing costs for our programs.

WEATHER CANCELLATIONS

NSF POLICY A $20.00 service charge will be assessed on all checks returned due to non-sufficient funds (NSF). Once an NSF check occurs, all future payments must be made with cash only.

SAY CHEESE The Mount Horeb Recreation Department reserves the right to utilize photos of participants for publicity purposes. Participants not wanting their photo used, must notify the Recreation Department in writing at the time of registration.

WAIVER OF LIABILITY The Village of Mount Horeb is not responsible for any injuries sustained as a result of participation in any department sponsored activities. We encourage you to obtain your own insurance prior to, and for the duration of, the activities you participate in.

FINANCIAL ASSISTANCE Financial assistance applications are available at the Recreation Department. You will be required to bring income and tax documentation with you.

EMPLOYMENT INFORMATION We will be taking applications for ice rink warming house attendants through December 7th. Our ice rink season is typically late December through February, but weather can play a factor.

In the event of inclement weather, the department will use multiple factors to determine if a program/activity will be cancelled. These factors include national heat index guidelines, field conditions, forecasted weather, etc. Morning activities may be cancelled up to 30 minutes before a program begins. A decision regarding evening programs/ activities will be made no earlier than 2:00 PM on the day of the activity. Participants will receive notification of cancellations via email and information will be placed on our Facebook page. Cancellations will also be posted on our Recreation Department website (http://www.mounthorebwi.info/ departments-services/mount-horeb-recreation-department)

We will be taking applications for our summer positions from January 2nd through February 28th. Interviews will take place in March. Positions include lifeguards, admissions/concessions supervisor, admissions/concessions assistant supervisor and admissions/concessions pool staff. Please contact the Mount Horeb Recreation Department for an application. Applicants must be at least 15 years of age (a/c supervisor and a/c assistant supervisor must be 18 or older).

The skills are applicable to the home and restaurant dining experience. Is this my water glass or yours? Elbows off of the table! Let’s go swimming to rid all of that indoor energy! Participants will have dinner shortly after arrival, make a small craft to let the food settle, then play some fun swimming games in the pool. Learning about aerospace, chemical, and electrical engineering, we will build rockets, playful polymers, electro magnets, and so much more! For each lesson, students focus on developing problem solving, critical thinking and peer collaboration skills. These life skills provide students with the confidence to approach challenges in their lives and realize that they can succeed. They will be using acrylic paint on a 16 x 20 canvas. Throughout this painting, participants will be working on shading and blending along with background, middle ground and foreground. The instructor will walk them through step by step, but each child will put their own personal touch on their project. These paintings turn out beautiful and would make a wonderful holiday gift! The Mount Horeb Senior Center is a facility whose goal is to provide Case Management and Nutrition services and resources for people 60+. We serve seniors in the Villages of Mt. Horeb and Blue Mounds and the Towns of Blue Mounds, Perry, Primrose and Springdale. Congregate and homedelivered meals are served Monday through Friday at 11:15 AM. We offer many social programs, health related services and transportation. Are you looking for a place to hold your next special event, meeting, birthday or graduation party? We have shelters available to rent, by following the steps below:

choose a park and pick a date on the calendar to rent, or use the “find time widget” to pick a date and space type.

1. Go to www.gymdandy.io to create an account. After you create an account, an email will be sent to you. you will need to confirm your account by clicking on the email and then “confirm email”.

3. Input your event details, expected attendance, and any other information pertaining to your reservation. you will then need to enter a payment method and card details. For reservations made with cash or check, you will need to visit the Mt. Horeb Recreation Department.
