Teams compete in the Madison Area Youth Soccer League during the fall and spring. The Mt. Horeb Soccer Club is a parent run organization and is not affiliated with the Mt. HOW DO I REGISTER?

Registration is being accepted now either online, by mail, drop box, or in person.

Financial assistance applications are available at the Recreation Department.

REGISTRATION INFORMATION

1) Pay by credit card online at http://activenet.active.com/ mhrd or complete the registration form on page 19. 2) Payment must accompany your registration form. You can write your credit card information on the form or include cash or check. Make checks payable to the Village of Mount Horeb. Credit cards (VISA, MASTERCARD, AMERICAN EXPRESS) are accepted at the Recreation Department. 3) Send to or drop off at the Mount Horeb Recreation Department, 222 E. Front St., Mount Horeb, WI 53572. A drop box is located outside the front door for your convenience.

AMERICANS WITH DISABILITIES ACT We welcome all individuals and families from our community to use our parks and recreation programs regardless of race, religion, sex, national origin, age, or disability. Those individuals with special needs are encouraged to notify us at least two weeks in advance to help us accommodate your request.

ON-SITE REGISTRATIONS Instructors will NOT be allowed to accept registrations or payments at class locations. All registration transactions must be completed at the Recreation Department office or online prior to class start date.

RESIDENT/NON-RESIDENT A resident is anyone who resides within the Mount Horeb Village limits. All others are considered non-residents, and will be charged an additional $10.00 to the resident rate. This charge is assessed per person per program. This additional non-resident fee helps with staffing costs and equipment expenses for our programs.

WEATHER CANCELLATIONS If you are wondering if a class or program is cancelled due to weather, you can call our cancellation line at 437-3400 ext. 9.

REGISTRATION DEADLINES/LATE FEES A $5.00 late fee will be added to your payment if postmarked after the registration deadline dates listed. Please plan ahead.

REFUND POLICY A refund will be issued when a program is cancelled by the Recreation Department. Cancellations made two working days or more prior to the scheduled start date of an activity will have a $2.00 surcharge. Refunds or credits will not be issued if you cancel out of an activity less than two working days from the program start date. A credit (minus a $2.00 surcharge + fees for classes already attended) will be issued in the case of illness or injury. Credits can be used for any recreation program in the future. Please note that online convenience fees are non-refundable.

CONFIRMATION OF ENROLLMENT Once registered, plan on attending at the listed dates and times. Please feel free to call (608) 437-3400 to verify your registration and/or ask any questions you may have. EMPLOYMENT INFORMATION The Mount Horeb Recreation Department will be taking applications for summer positions from January 3rd through February 28th. Interviews will take place in March. Positions include lifeguards and softball umpires. If you are interested, please contact the Mount Horeb Recreation Department for an application. Applicants must be at least 15 years of age.

NON-SUFFICIENT FUNDS POLICY A $20.00 service charge will be assessed on all checks returned due to Non-Sufficient Funds (NSF). Once an NSF check occurs, all future registrations must be paid in cash or money order only.

PUBLICITY The Mount Horeb Recreation Department reserves the right to utilize photos and names of participants for publicity purposes. Participants not wanting their names and/or photo used, must notify the Recreation Department in writing at the time of registration.

PROGRAM CANCELLATIONS Programs may be cancelled if minimum registrations are not met or facility issues occur. You will be notified before the start date if an activity is cancelled.

SHELTER RESERVATIONS Reservations for shelters begins at 7:15a.m. on Tuesday, January 3 for the 2017 year. Phone calls for reservations will not be accepted. You must stop in to the Recreation Department to fill out the shelter form and make payment. Reservations are made on a first-come, first-serve basis. Shelter fees vary by park. You may call the Recreation Department at 437-3400 to check on shelter availability. Shelters include: Boeck’s Park Shelter, Grundahl Park Enclosed Shelter, Grundahl Park Open Picnic Shelter, Liberty Park Shelter, Sunrise Park Shelter and Waltz Park Shelter.

WAIVER OF LIABILITY The Village of Mount Horeb is not responsible for any injuries sustained as a result of participation in any department sponsored activities, or while using departmental facilities. Session 2 - ‘LOVE’ to Create! Each child will be working on a multi-media project on a 11 x 14 canvas to create a fun Valentine’s Day project. We will use acrylic paint, scrap booking paper, music sheets, stamps, markers and letter stencils and will coat our projects with modge podge to give them a finished look. These projects turn out beautiful and each child will be very proud of their art! Session 3 - My Painting is Blooming! Each child will be painting their own flower garden. They will be using acrylic paint on a 16 x 20 canvas. Throughout the painting will be talking about primary, secondary and complimentary colors. We will also talk about bold, bright, and colorful graphics. These paintings turn out colorful and whimsical and are so much fun to do!

TOT TIME Dates: Fridays, 1/6 – 3/24 Location: New Hope Evangelical Free Church Gymnasium Ages: 12mo - 4 Time: 9:00-10:00a.m. Fee: $1.00 at the door Join us for toddler open play at New Hope Evangelical Free Church. There will be some equipment provided for children to use. The Recreation Department will bring music to sing and dance to, and give the opportunity to run around the gym and play freely. Session 1 - Mr./Mrs. Snowman Each child will be creating a painting of a cute Mr. or Mrs. Snowman. They will be using acrylic paint on a 16 x 20 canvas. Throughout this painting, we will be working of shading and blending along with background, middle ground and foreground. ICE SKATING INFORMATION Recreational ice skating is available at Garfield Park. Ice hockey is allowed only on Wednesday nights, Saturday mornings and Sunday mornings. Garfield Park warming house hours*: Monday - Thursday 4:00-8:00p.m. (Wednesday is hockey night)

The warming house will open (except for 9:00a.m.-12:00p.m. on Saturdays and Sundays) if the condition of the ice is safe and ready to skate on. To reserve a shelter or ball field: 1) C  all (608) 437-3400 to check availability. PLEASE NOTE: We do not take reservations over the phone. 2) O  nce availability is determined, you must stop in to the Recreation Department to fill out the reservation form and make payment. Shelter fees are: Level 1 shelters without kitchenette (Boecks, Grundahl Open Picnic, and Waltz) are $50 (residents) and $70 (non-residents). Level 2 shelters with kitchenette (Gundahl Enclosed, Liberty and Sunrise) are $60 (residents) and $80 (non-residents). Ball field fees are $50 for single weekday use, $75 for single weekend use, and $125 for whole weekend use. 3) If your shelter requires a key, we will let you know when you can pick it up. Keys must be returned to the Recreation Department within one week after your rental. 4) A confirmation of your reservation will be given to you. LIBERTY PARK (1201 COX DRIVE) Amenities at this park include a shelter with kitchenette, soccer/football field, playground equipment, picnic tables, concession area, water fountain and restrooms.

SUNRISE PARK (201 TELEMARK PARKWAY) Amenities at this park include a shelter with kitchenette, soccer field, skate park, baseball diamond, playground equipment, picnic tables and restrooms.
