WHO WE ARE The Riverside Community Health Foundation (RCHF) is the lead agency of the Riverside Community Diabetes Collaborative (RCDC). RCDC is a group of over 40 diabetes-focused organizations and businesses that work together to strengthen community programs and services for the prevention and management of diabetes in Riverside and surrounding areas. RCDC is your local resource for diabetes and chronic disease prevention information, events and more.

DIABETES TIPS Spring is right around the corner! This is the perfect time to spring into action with your diabetes care plan. One important component to managing your diabetes is physical activity. Take advantage of the spring time sunshine and get moving outdoors. Try new ways to get active! Take a walk around the neighborhood, spend time in your garden, or enjoy activities in the park with friends and family.

EATING HEALTHY FOR LIFE Learn how chronic disease can be prevented through making healthier food choices. Topics include nutrition label reading, fast food alternatives, portion control, food groups, and healthy substitutions. Eastside Health Center April 6 (Thursday), 9am - 10am

A FREE PROGRAM FOR PEOPLE WITH DIABETES AND THEIR CAREGIVERS.

DIABETES MANAGEMENT EDUCATION The Diabetes Wellness Program offers diabetes management education using the Conversation Map® education program. These interactive, small group sessions are designed to help you discover what you want and need to know about managing your diabetes or that of a loved one. These 2-hour sessions are taught by a licensed healthcare professional.

This FREE program is designed for people with diabetes. *To participate in any of the program’s classes, please RSVP by calling (951)788.3471.

DIABETES WELLNESS PROGRAM *DIABETES COOKING CLASSES These cooking classes are led by a registered dietitian and focus on how to cook healthy meals for someone with diabetes. We offer a Recipe of the Month card at every class so that you can build a cookbook throughout the year. Educational topics at each class include nutrition label reading, healthy substitutions and serving sizes. DIABETES MINI-SERIES: Diabetes Care on the Go Traveling, vacations and emergency situations can affect your diabetes management plan. Want to learn how to be prepared outside of your normal routine? Join us at our Mini-Series where you will learn to prepare a diabetes disaster plan, travel guide and how to manage your diabetes on the go. Registration and free health screenings start at 5:15 pm. The “Together We Can!” Mini-Series and Annual Seminar are quarterly seminars that discuss diabetes management tips from healthcare professionals. Save the date for the next Mini-Series on Wednesday, August 23. The 7th Annual Diabetes Seminar held at the Riverside Convention Center will be on Monday, November 6.

GROCERY STORE TOURS

Diabetes Grocery Store Tours: Join us as we explore smart grocery shopping tips for people with diabetes. We’ll learn all about carbohydrates, carbohydrate counting, and how to build a healthy, diabetic-friendly plate. April 5 (Wednesday), 10:00 am – 11:00 am at Stater Bros. Heart Healthy Grocery Store Tours: This grocery store tour is all about heart health! We’ll leave you with tips to help you become heart-smart at the grocery store and choose good-for-you foods.
