PruittHealth @ Home allows patients to return home and continue to heal with nurses and support teams monitoring their well-being. This team of expert caregivers supports the recovery process from illness, injury, or surgery in the comfort of patients’ homes.

Our home health teams plan regular in-home visits and use advanced connection technologies to ensure comprehensive monitoring and consistent treatments and therapies to maintain optimal health and wellness.

PruittHealth Hospice & Palliative Care teams provide specialized, expert care to help make patients’ final days comfortable and connective through family-centered care delivered in-home or at a PruittHealth skilled nursing, assisted, or independent living location.

HOSPICE CARE:

PruittHealth Hospice is our family-focused approach to end-of-life care that helps patients live their final days well cared for, pain-free, and surrounded by their loved ones.

Hospice care does not hasten or postpone death, caring for the person rather than the disease, with a focus on quality rather than length of life. Hospice care is always familycentered, involving the patient and family in the decision-making process. PruittHealth delivers hospice care 24 hours a day, 7 days a week in the setting tailored to each patient’s individual needs. Hospice is not a place; it is a specialized service approach to end-of-life care.

PALLIATIVE CARE:

PruittHealth Palliative Care provides comfort, ongoing treatment, and spiritual support to ensure the best possible quality of life.

Palliative care is specialized care for those with an acute or chronic illness and is focused on symptom management and stress relief. Patients may have palliative care at any stage of their illness and may receive curative therapy at the same time. The goal is to reduce a patient’s suffering, provide ongoing treatment, and promote the best quality of life for those with serious illnesses.

PHARMACY

PruittHealth Pharmacy Services ensures our customers receive their medications in the most convenient and secure way possible and provides individualized, patient-centered care with the highest standards of excellence.

We ensure peace of mind for patients and their families by closely monitoring all medications from the time the order is received and work to eliminate any unnecessary medications. A team of experts is available to handle our customers’ medication needs to protect their well-being and safety.

RX@HOME PROGRAM:

As our customers transition home from a health care center, we provide peace of mind to them and their families through appropriate medication management. With the Rx@Home program, customers return home with prescription medications to support them during their transition.

INFUSION THERAPY:

PruittHealth provides patients with everything they need to receive quality infusion services in the comfort of their home. Our team of experts verifies benefits, determines out-of-pocket costs, and presents any available options to minimize costs for our patients.

We are committed to upholding the highest standards of care, offering advanced technologies, and delivering timely and accurate supplies to patients’ homes. We offer:

Patients are assigned their own clinical expert who communicates at least weekly to ensure a patient is responding to IV therapy and to answer any questions.

PruittHealth Diagnostic Services offers a range of laboratory tests to deliver accurate, reliable, and convenient testing solutions for our patients. Our in-house, state-of-the-art laboratory makes testing for COVID-19, Flu A/B, and RSV, to name a few, easy and convenient. This accessible testing solution ensures patients receive the correct diagnosis quickly. With PruittHealth Diagnostic Services, our patients receive the enhanced, highquality care they’ve come to know and expect from our family of providers.

CARE MANAGEMENT

PruittHealth offers in-home services to eligible elderly and disabled people receiving Supplemental Security Income (SSI), active full Medicaid, or pending Medicaid coverage. Our program is designed to ensure members’ safety and improve their overall health. We offer adult day health, emergency response, home meal delivery, support services, and much more to meet and exceed individual health care needs and expectations.

OUR PROGRAM REQUIREMENTS:

• 65 years old or older and/or disabled

• Receiving Medicaid and SSI

• Medical conditions, such as diabetes, high blood pressure, lung disease, arthritis, Alzheimer’s disease, dementia, heart disease, or stroke that require monitoring and regular doctor’s care

PHYSICIANS SERVICES

PruittHealth Physicians Services is a medical practice division within our family of providers, consisting of physicians and nurse practitioners who provide services to enhance patient care.

We provide better coordination and communication with hospitals, referral sources, and payers, which are critical to care transitions and reduce rehospitalizations. By ensuring physician access and engagement, we offer a level of communication, trust, and personal connection with patients and their families.

CONSULTING SERVICES

PruittHealth Consulting Services provides contracted health care centers and medical providers with clinical guidance and support in nursing, nutrition, social services, rehabilitation, medication therapy management, spiritual care, and more. We provide teams with tools to improve compliance, develop best practices, and create policies and procedures.

AQUATIC THERAPY

Aquatic therapy helps patients through state-of-the-art programs featuring HydroWorx® therapy pools with adjustable depth, resistance jets with massage features, underwater cameras and viewing monitors, and computer documentation.

BALANCE PROGRAM

Our balance program uses validated assessment tools and appropriate intervention to help patients improve balance, reduce fall risks, and treat associated problems, including incontinence, pain, muscle weakness, and hypotension. Our program creates personalized care plans and home exercise programs that work to improve balance, strength, range of motion, endurance, walking, daily activities, and overall quality of life.

CARDIAC CARE

Cardiac care works with a patient’s team of health care providers, including primary care physicians, cardiologists, clinical nurse specialists, dietitians, mental health professionals, case managers, and pharmacists to help improve overall health and quality of life.

FITNESS THERAPY

Fitness therapy utilizes physical and occupational therapy, nursing, and exercisemonitoring tools to help patients restore lost function and regain independence.

LIFE ENRICHMENT

Life enrichment programs help patients stay physically and mentally fit by providing extensive resources to expand knowledge and fulfill spiritual needs.

MEMORY SUPPORT

Memory support is designed to assist Alzheimer’s and dementia patients by attempting to slow memory loss and other cognitive changes. Memory support provides life enrichment activities that stimulate the mind and promote overall health. The program provides family support and education to help caregivers keep loved ones engaged.

PAIN MANAGEMENT

Pain management helps patients gain control over chronic pain to enhance enjoyment of life and limit lost productivity and wages. Our pain management team addresses pain and its underlying causes with therapy and treatment plans that include physical or occupational therapy, medication assessment and management, education, injections, or surgery.

RESPIRATORY THERAPY

Respiratory therapy offers high-quality care and independence to patients who have had a tracheostomy, experience an exacerbation of respiratory symptoms, or require BiPAP/CAP or ventilator support. Our team of clinical experts develops comprehensive care plans designed to increase long-term health and overall well-being.

STROKE REHABILITATION

Stroke rehabilitation works with patients and loved ones to improve physical, cognitive, and lifestyle functioning after minor and major strokes. Each plan is personalized to ensure the patient’s unique needs, priorities, and preferences are met.

WOUND CARE

Wound care offers comprehensive, advanced treatment options to alleviate suffering and mitigate infection, hospitalization, and limb loss. Our policies and procedures are grounded in research, consultation, and the expertise of certified Wound, Ostomy, and Continence Nurses (WOCNs).

ADMINISTRATOR-IN-TRAINING PROGRAM

PruittHealth is dedicated to creating opportunities for individuals that are interested in pursuing a career as a health care administrator. Through our Administrator-in-Training program, an individual receives classroom instruction, hands-on training, and mentorship from experienced administrators. The goal of the Administrator-in-Training program is to develop strong leaders throughout our health care centers, resulting in increased patient care and partner retention.

THE PRUITTHEALTH NURSE ADVANCEMENT PROGRAM pays the tuition (up to $15,000) of 20 caregivers, including CNAs, Senior CNAs, and LPNs, who wish to become registered nurses.

PRUITTHEALTH UNIVERSITY

We recognize that quality people thrive in an environment that promotes lifelong learning, so years ago we established PruittHealth University, an online learning resource that is updated with new and innovative material each month. Video education programs guide partners through topics, such as human resources, benefits awareness, leadership and customer service skills, as well as clinical-specific programs.
