At Your Fingertips For an additional fee, take advantage of on-site services: Resident Dining Options – available through the Irem Clubhouse Restaurant and Pub. Our staff can assist in custom menu planning for special dietary requirements (more details on the following page). Housekeeping – available through local reputable companies Scheduled transportation – arrange to use the campus shuttle at your convenience Professional staff and management – personnel are available on-site to address questions and concerns, including an after-hours on-call program

Resident Dining Options Choose from one of the Masonic Village Resident Dining Options, each offering unique benefits. Enjoy the menu Ă la carte, too. Meet with neighbors and friends for lunch, dinner or Sunday brunch. With seating available in the restaurant, pub, patio and terrace, choose the best spot to suit your mood. Dine on American-inspired cuisine including weekly and nightly chef specials.

Misericordia University Explore collegiate life at Misericordia University, conveniently located next to Masonic Village. Newberry Estate Country Club At nearby Newberry Estate Country Club, enjoy a swim or lounge at the outdoor pool, play a game of bocce or a round of shuffleboard, take a tennis lesson or try your hand at pickleball.

CIP Rehabilitation Inc. Located in the Irem Clubhouse, CIP Rehabilitation’s professional staff provide outpatient rehabilitation services including physical, occupational and speech therapy. They also offer a weekly Senior Fit Club class and massage and personal training services.

Options You’re ready to move. What’s next? We offer one- and two-bedroom apartments where you can take in a sunset from the rooftop balcony or meet with neighbors in common areas. Villa layouts include two-bedroom and two-bedroom/den plans with one or twocar garages.

Payment Options Enjoy a private, spacious home while we manage day-to-day services and maintenance for you. We’ll work with you to find the option best suited for you and your budget.

Entrance fee options include: Guaranteed Appreciation Plan – Under this plan, you would be guaranteed 90% of the entrance fee paid within the first year of residency. Each year thereafter that you reside in your villa, you would receive an additional 1% of the original entrance fee paid toward a future refund to you or your estate. Eight-Year Declining Balance Plan – This option is a lower financial investment with a refund balance that declines gradually over an eightyear period. The length of stay determines refunds to you or your estate. The amount of refund declines at a rate of 5% the first month and 1% each of the following months. After eight years, there is no remaining refund.

The apartment rental fee option includes: Maintenance and all of the services and amenities listed on page 5 covered under one monthly fee. There is no entrance fee.

Care & Support Care Coordination Our experienced and professional team is here to help. On-site staff can provide resources and guidance with any transition of care, whether in your home or with a health care provider in the community.

Nursing & Personal Care Rest assured knowing you have priority access to quality health care, if needed, through our extensive network of community partnerships.

CARE & SUPPORT

Our preferred health care partners include Meadows Nursing and Rehabilitation Center and the Mercy Center, located adjacent to Masonic Village on the Misericordia University campus. Masonic Village locations throughout Pennsylvania are also an option for health care services.

Rehabilitation Services Find the assistance you need on your road to recovery after a hospitalization with rehabilitation services through our preferred community partners. Available on-site, CIP Rehabilitation offers outpatient services, including physical, occupational and speech therapy.

In Your Home Our staff can help you coordinate services such as Meals on Wheels, transportation, pharmacy deliveries, home care or home health care through our community partners. To ensure you find a good match, our approved provider list includes only private duty service companies that have met certain requirements.

Financial Assistance Generous donations enable Masonic Village to provide services to most eligible individuals who remain within Masonic Village’s care, regardless of financial means. It is expected your financial resources will be preserved for the cost of your care and services, and our staff will help you apply for all available financial assistance when needed. 13


5 Things to Know 1. Anyone age 60 and older may apply. For couples, one person needs to be age 60 and the other at least age 55. Anyone age 55 and older may sign up for the priority waiting list. For additional requirements, please contact the Sales & Marketing Office. 2. Move-ins average approximately three months, and Masonic Village has various resources to assist you. 3. We know your pets are part of your family, and you may bring up to two into your new home. We’ll provide specific guidelines based on your preferred living option. 4. Masonic Village provides support to most eligible individuals who may no longer be able to afford their services at some point in the future for reasons beyond their control. Our staff can provide guidelines. 5. All residents receive preferred access to personal care and nursing services on a fee-for-service basis (more details on the previous page).

The F.M. Kirby Center features concerts, orchestras, plays, Broadway touring shows and comedic acts.

The region’s mountains, streams, lakes and valleys connect you with the great outdoors. Frances Slocum State Park is just minutes away and offers hiking, picnicking, boating and fishing.

Mohegan Sun at Pocono Downs hosts a casino and live harness racing. Mohegan Sun Arena at Casey Plaza is home to the Wilkes-Barre/ Scranton Penguins, Pittsburgh’s farm team.

Nearby Wilkes-Barre boasts shopping, theaters and more than a dozen parks with hiking trails, tennis courts, shuffleboard areas, volleyball courts and ice skating rinks.

The New York Yankees Minor League Baseball team plays in nearby Moosic.

Application & Priority List Process STEP 1: Contact the Sales & Marketing Office to schedule a personal visit. STEP 2: We will send you an application to complete and return within 14 days. STEP 3: Within seven days of receiving your application, we’ll follow up with any additional questions and/or recommendations. STEP 4: We will notify you of your approval and send a copy of the resident agreement for you to review, sign and return within two weeks. Entrance fee plan - Upon signing the resident agreement, 10% of the entrance fee (minus the application deposit) is payable on your reserved accommodation. If you decide not to continue with the application process, following a 7-day rescission period, we will refund your 10% payment (minus the application deposit). Within 90 days of signing the resident agreement, or upon occupancy, whichever is sooner, the entrance fee is due. Within 60 days, or upon occupancy, whichever is sooner, the first monthly service fee is due. Rental plan - The balance of your first month’s rent is due when you return the resident agreement.

Priority List Applications If you’d like to plan for the future, you can submit an application for a priority list. Entrance fee priority list - Include a $1,000 deposit to place your name on this list. Rental plan priority list - Include a $200 deposit to place your name on this list. This deposit will be credited toward your application deposit when you apply for an accommodation.

Directions to Masonic Village From the Williamsport area: Take 118 East to Memorial Highway (415). Turn right onto Memorial Highway, and follow through two street lights. At the second street light, make a left onto Center Hill Road. At stop sign, turn left onto Lake Street. Continue on Lake Street, passing Misericordia University. Make the first right after university/cemetery onto Country Club Road. Look for signs for Masonic Village. From the West: Take I-80 East. Merge onto I-81 North via Exit 260B on the left toward Wilkes-Barre. Merge onto 309 North via exit 170B toward Wilkes-Barre. Follow 309 North for approximately 9.5 miles to Dallas, and watch for the “Y” where routes 415 North and 309 North split (Dallas Shopping Center on right). Keep right on 309 North, and continue straight to the first traffic light. At traffic light, make left onto Center Hill Road. At first stop sign, turn right and follow to Irem Road. Go approximately 1 mile, and turn left onto Country Club Road. Travel approximately ¼ mile, and look for signs to Masonic Village. From the Harrisburg area: Take I-283 North to I-83 North. Merge onto I-81 North via Exit 51B toward Hazleton/ Allentown. Merge onto 309 North via Exit 170B toward Wilkes-Barre. Follow 309 North for approximately 9.5 miles to Dallas, and watch for “Y” where routes 415 North and 309 North split (Dallas Shopping Center on right). Keep right on 309 North, and continue straight to the first traffic light. At traffic light make left onto Center Hill Road. At first stop sign, turn right and follow to Irem Road. Follow 309 North for approximately 9.5 miles to Dallas, and watch for the “Y” where routes 415 North and 309 North split (Dallas Shopping Center on right). Keep right on 309 North, and continue straight to the first traffic light. At traffic light make left onto Center Hill Road. At first stop sign, turn right and follow to Irem Road.
