POPULARITY OF LARCS IS ON THE RISE (WHEN COST IS NOT A FACTOR)

While the total number of contraceptive units dropped slightly from 2015 to 2016, the number of distributed long-acting reversible contraceptives (LARCs) rose significantly, as did weeks of protection.

Upon further research, we discovered the increase in LARCs occurred in the four Planned Parenthood San Antonio clinics that receive Title X funding (a federal family planning contract managed by the Women’s Health and Family Planning Association of Texas). As a condition of this funding, clinics do not charge a fee to clients who have incomes at or below 100% of the Federal Poverty Level. The funding also allows the purchase of pharmaceuticals at discount pricing for clinics serving populations with low-incomes, increasing 6 | 2016 SERVICE DELIVERY REPORT

access to these highly effective forms of birth control, which otherwise can be prohibitively expensive. Many patients requested LARCs after the 2016 presidential election citing concern over not being able to obtain LARCs if the new administration dismantled the Affordable Care Act as they have repeatedly said they intend to do. The most popular LARC was the single-rod implant (Nexplanon®). Other noteworthy trends: • Oral contraceptives remain the most popular form of birth control among our clients. • Demand for emergency contraception at PPST has been declining since EC became available over-the-counter at numerous retail pharmacies. 2016 SERVICE DELIVERY REPORT | 7

abortion care In 2016, Planned Parenthood provided 740 abortion procedures (in-clinic and abortion pill) at our South Texas Medical Center. During part of the year, abortion care was paused, which likely accounts for the decrease in abortion care when compared to previous years (in 2015 we provided 1,498 abortion procedures). The type of abortion care also experienced a dramatic shift as more patients chose the abortion pill compared to 2015 (see pie chart on opposite page). In March of 2016, the U.S. Food and Drug Administration (FDA) updated its prescribing guidelines to reflect medical best practices. This change allows medication abortion to the prescribed a few weeks later into pregnancy (through 10 weeks gestation) and it is likely the reason for the dramatic shift. In June of 2016, after almost three years of litigation, the U.S. Supreme Court struck down key provisions of HB2, Texas’ onerous abortion law. Nevertheless, since the law passed in 2013, some of these provisions were in effect at different times in different regions, causing public confusion about whether abortion was legal and where people could get an abortion when they needed it. Planned Parenthood continues to work hard to correct the damage to abortion access caused by HB2. Information provided both through community outreach and in our clinics dispel abortion myths and provide the most comprehensive information about pregnancy options. 8 | 2016 SERVICE DELIVERY REPORT
