TROOP 257 SUMMER CAMP 2012

This year we are attending session 4 Week of July 22nd – 28th Camp Wanocksett Dublin, NH OUR CAMPSITE IS ARROWHEAD. THE COST OF A WEEK AT CAMP PER SCOUT WILL BE EARLY BIRD $345.00 REGULAR COST $370.00 IF YOU ARE GOING TO PAY THE $345.00 WE NEED THE MONEY NO LATER THAN MAY 10, 2012 SO A CHECK CAN BE MAIL TO THE CAMP. THE COST OF TROOP PICTURES HAS NOT BEEN POSTED YET. AS I GET THE INFORMATION I WILL PASS IT ALONG PLAN ON AT LEAST $10.00. CAMP WEB SITE IS WWW.CAMPWANOCKSETT.ORG FEEL FREE TO LOOK AROUND. AS ALWAYS YOUR SCOUT WILL NEED TO FILL OUT AN UPTO DATE MEDICAL FORM.

DO NOT WAIT TILL THE LAST MINUTE TO FILL THIS OUT! I will need med forms ABOLUTELY NO LATER THAN JULY 10, 2012, as they will need to be turned into camp for pre-camp check-in. We cannot wait until the last minute this year.

MEDICAL FORM INSTRUCTIONS Accurate medical records for campers are required by BSA standards and state law. They are also critical to ensure timely, effective care should you or your Scout become sick or injured while at camp. All campers, adult leaders and staff MUST complete the BSA Annual Health and Medical Record form annually. Forms expire after 12 months. Scouts MAY NOT participate in many camp activities including (but not limited to) swimming, boating, climbing, COPE, and sports, and may not remain in camp longer than 72 hours without a completed medical form. Read the medical form carefully. This page highlights areas that are commonly incomplete. All portions of the form must be completed for ALL summer camp programs. Common mistakes include: • Missing emergency contact information (Part A) • Incomplete medication information (Part A) • Missing medical insurance card (Part A) • Missing immunization record (Part A) • Missing parent/guardian signature (Part A& B) • Missing physician signature (Part A & C) • Physical exam more than 12 months ago (Part C) NOTE: State regulations require that a copy of your complete immunization record be attached to your medical form. MEDICAL FORMS ARE NOT RETURNED AT THE END OF CAMP. Always submit a COPY of your medical form. Keep the original for use at other Scouting activities.
