Facts from Human Resource Policies What you MUST know!

Vacation Policy:  Request for Time Off Form must be filled out & sent to the office  Approval or denial of requests lies with your Client Care Coordinator and Client Care Manager  Maximum vacation allowance at a time is 3 weeks  Must give 6 weeks’ notice when requesting time off

Sick Leave Policy:  Doctor note must be provided if you are off work for more than 3 days in a row  Abuse of sick leave of absence will not be tolerated What should be in a doctor’s note?  Start and end dates of your medical time off from work  Any restrictions and limitations – if they are temporary or permanent PHCS has the right to request restrictions and limitations in order to determine if an employee is totally disabled from their own occupation or any occupation. Employees have a corresponding duty to facilitate this request under the Human Rights Code by providing a Physician the necessary consent to complete the required medical documentation.

Remember: Sick leave is eligibility, and is not deemed an entitlement!
