s Gene Audtry and more recently Aerosmith said I’m “back in the Saddle again!” It is wonderful to be back writing for Panache Vue Magazine. Disseminating accurate information on Orthodontics is really important. We have all heard the fake anecdotes about how Orthodontics is just for the “upper front teeth aka the “social six”. This is as far from the truth as it can get. I continually hear from patients, “I’m not concerned with my bite”, and my answer to that is “until you no longer have one”. One does not realize that without a reasonable bite eating can be very difficult. Depending on the problem we see patients that develop G.I. problems due to the inability to chew their food and get it ready for digestion. At certain times during a consult with certain types of problems I will ask the patient about how long they have had their G.I problem, without any questionnaire portion of their medical history asking about this potential problem. I usually get a look of “How did you know about that, I never told you”. Well it’s simple if we cannot chew our food properly the digestive process will be broken from the beginning. his exhibition on the legacy of George Inness, Jr. will include paintings from the Unitarian Universalist Church of Tarpon Springs. Additional works from the private collection of Cici and Hyatt Brown include paintings of the Anclote River, Lake Tarpon, Spring Bayou and the Sponge Docks. Few of these paintings have ever been exhibited in this area. A companion show, The Legacy Continues, will present a sampling of works by fine artists who’ve made Tarpon Springs their home, including Kevin Grass, Elizabeth Indianos, Mitch Kolbe, Christopher M. Still and Joseph Weinzettle. Also on view is Historic Tarpon Springs, a collection of prints by Tarpon Springs architect Edward C. Hoffman, Jr.

W

e go through quite a lot to get our skin to regenerate and rejuvenate collagen, from topical products to lasers to surgery and every cream imaginable. But new advancements in an age-old procedure make collagen stimulation easier than ever to achieve. “Collagen Induction Therapy, or microneedling, is a very old technology that goes back to ancient times,” says Dana Martinez, owner of Lecada Medical Artistry in South Tampa. “What brings it into the 21st century is the fineness of the needles and ability to control the depth of penetration.” Think of it like acupuncture, but for wrinkles and scars. As the small, handheld wand travels gently over the skin, it creates microscopic, pinpoint punctures into the dermis – the majority of which are simply pushing your pores open temporarily. This is perceived by the body as damage, which stimulates the release of growth factors that trigger the production of collagen and elastin. The human body reacts to any injury by initiating the healing process, but usually it will only heal as far as it must to keep you healthy – which is different to healing to a cosmetic level. By persistently triggering the healing process, you encourage your body to continue healing until the job is finished. The treatment is not painful. Typically, it is the length of the needle used which determines whether or not a numbing agent is used prior to treatment. The length of the needles also determines the results gained.

Essentially, the rule of thumb is longer the needle the better the results — although it’s not quite that simple as the depth of the skin in the area to be treated, its current condition, the area, the client’s lifestyle must all be taken into consideration. “The therapist has complete control, choosing to lightly prick the top of the skin to help treat fine lines and wrinkles, allowing topical products to seep in better, or to go deeper to treat stubborn scars and stretch marks. As the skin heals, these small punctures form new collagen for healthier-looking, smoother skin,” says Martinez. The large portions we consume are laden with preservatives, pesticides, hormones and antibiotics which are used by large food manufacturers to increase their yields. It’s no surprise that two thirds of our adult population and one third of our children are obese and overweight. Diabetes is being diagnosed now in younger individuals including children and the complications of the condition are at an all time high with cardiovascular problems, cancer, autoimmune diseases, strokes and dementia. Excess weight is an inflammatory condition, where the fat cells produce inflammatory cytokenes which cause damage and disease to every organ, including our brain. This inflammatory nature is directly caused by the disruption of the hormonal balance necessary to maintain health. The main culprit is the hormone Insulin. Through a mechanism called insulin resistance that disrupts our ability to heal and starves the systems of the body from getting the energy they need to function, instead storing it as fat.

A cascade of other hormones add to the disruption and progression of disease. The most common being thyroid which is responsible for the metabolism and physical and mental energy producing functions of the body.The key laboratory test is the level of Free T3 and depending on its range, your energy and metabolism will be reflected. Low thyroid function causes weight gain; and insulin resistance decreases your body’s ability to make Free T3, which in turn facilitates the non ending loop of making more fat. Another large culprit is cortisol, which is produced by our adrenal glands in response to physical or mental stress. Unfortunately, in our society we experience stress from the moment we awaken, we don’t generally appreciate the moments but instead are usually racing to achieve more. Elevated cortisol increases insulin resistance, makes us crave carbohydrates, which further increase insulin and hence makes more fat. SHBG (Sex Hormone Binding globulin) is associated with health and longevity. People with high levels enjoy healthier and longer lives, and those with low levels are generally sicker with cancers, diabetes, heart disease and live shorter lives. Insulin resistance decreases SHBG levels and when a person who has insulin resistance gets rid of it by releasing their excess weight and arriving at a healthy weight, their SBHG levels go up. Testosterone and estrogen are equally important in both men and women in our ability to achieve and sustain optimal health. They both directly decrease insulin resistance and adequate levels are necessary to decrease fat, especially abdominal fat which carries the highest cardiac risk due to its proximity

to the heart. They help improve cardiovascular function and decrease atherosclerosis. They help build and maintain lean body mass (muscles) and are necessary for the health of our bones, our mental health and our overall sense of wellbeing. Both of these hormones are not only affected by obesity, but their decreasing levels or lack of them, as it occurs naturally during the midlife years, directly causes the body to be off balance and the formation of fat accelerates. This is evident by the battle of the abdominal bulge that is common in both sexes as we reach our fifties. Indisputably, treating obesity is both complex and a chronic medical problem which means that once you are successful in releasing that excess weight and in reaching your healthy weight ; the potential to regain and accumulate sick fat cells persists. Dear Andrew Ashton, I been noticing online when looking at photos of Summer trends for makeup, they are showing a lot of eye makeup that look really glossy. Is this a technique or a certain product I can use? Teresa Tampa Dear Teresa, Yes, the glossy look of eye makeup is definitely huge this season. The looks were all over the runways for the spring/summer.

You can get this look by using a creme based shadow but instead try this technique. There’s a couple of different ways of applying. First you can use a silicone type product (something used as a winkle filler) and mix with you shadow to create a more creme based shadow and apply to eye lids. Although this can work I find the next technique will give you a much glossier look. Apply your desired shadow to the lash line area. Next use a clear lip gloss (the glossier the better) and apply to a qtip and work the shadow out to cover as much of the lid as desired. This is a great technique to achieve the glossy eye look. Dear Andrew Ashton, Everyone is wearing braids. What do I need to know about the braid trend right now? Haley Safety Harbor

Dear Haley, The braid trend has been going on a long time. The difference is the braids change and how they are placed on head. Right now braids are very loose and sometime very messy. I great technique to creating a messy but very cool looking braid. Rough dry your hair or let it dry naturally. Add a rePear Forming Paste to create a bit more texture to the hair. Section off hair ear to ear and clip out of the way. Take the back and braid very loosely and not perfect. Secure the bottom. Let the top down which you have clipped out of the way. Take small sections and loosely push the ends into the braid. Also, if you would like a bit more texture, braid small sections of the top and push those into the braid. Try not to use hairspray unless you really need to as you want the look to be messy and loose. And just have fun with the braids this summer.

Last year Americans spent $2.8 trillion on healthcare. That is around $8,000 per person. So does this mean that we are all healthy and living well? Absolutely NOT! In fact, we lead the world in heart disease, obesity, and cancer. Astonishingly, 5 out of 6 Americans will be diagnosed with either heart disease or cancer. If we were spending the most money in healthcare and had the healthiest people, maybe it would be worth it. But we are spending the most money AND have the unhealthiest people. P

anache: When and why did you decide to become an orthodontist? Paul: I attended the University of Detroit Dental School and had finished my undergraduate requirements in my 3rd year of training. Having this completed I decided that I would explore specialty options for after graduation. I had volunteered in the Orthodontic department and became interested in not only Orthodontics but the sub- specialty of surgical Orthodontics for the correction of Dentofacial deformities. Panache: What one memory stands out to you the most early on in your career and one more recently? Paul: The earliest memory that stands out was one of how this specialty could and does change people’s lives in a very positive way. You can have a patient who has a very poor self image of themselves and is very withdrawn due to their smile, and once we correct their smile they “come out of their shell” and become happy outgoing individuals. My most recent memory actually is the same as my first. The impact I can have on an individual is such a motivator to continually strive to improve my Orthodontic skills and knowledge. Panache: What is unique about your practice compared to the competition? Paul: I have a relatively small “boutique” practice. What I mean by that is I have only one location and I am the only Doctor in the practice. What this means is that I totally control and treat every patient. With a small dedicated and caring staff we pride ourselves on seeing our patients on time which allows them to plan their day around their appointments without a long wait to be seen. We see only a small number of patients daily which allows us to really get to know each and everyone on a personal basis. Panache: Are you involved with any charities and do you do any non-profit work to give back? Paul: The answer to those two questions is YES! My wife and I have been deeply involved with the Susan G. Komen for the cure foundation for many years and somewhat recently have turned our attention and giving to the Suncoast Hospice organization. We have done

a fund raiser during Halloween in which we totally transform the outside of our residence into a Haunted graveyard. “The Mansion of the Macabre” and ask for donations for Hospice. This elaborate graveyard takes 4 weeks to assemble and is open only 2 nights during the Halloween season. We have been featured on ABC news two years in a row as one of the “Top 10 most Outrageous Halloween Homes across America” and on the Hallmark Channel. I always strive to “give back” to my community with my Orthodontic talents and have treated many pro bono patients in my career. I support many school and private endeavors that my patients request thru contributions to these events. Panache: What do you do to keep up with the latest in Orthodontic technology and trends? Paul: I am constantly attending continuing education classes and subscribe to a literature review that allows me to read many articles that are being published. Doing this keeps me at the cutting edge of treatment options and technology. I am a member of numerous Orthodontic societies and also study clubs that allows for the sharing of ideas on complex mutli-disciplinary treatment plans for patients that have excep-

tionally difficult problems. Panache: What are your personal and professional mottos and how do you apply them to life and business? Paul: The biggest Motto is “treat People as you would like to be treated” and always “treat my patients as I would my own family”, and last but not least “you can never overeducate someone”. All the above play a large role in not only my private life, but also professional life. Panache: For those who have never been to an orthodontist but really should, what would be the most important advice you would give them? Paul: Since my initial examination is at no charge there is no reason not to get an evaluation. There is never any pressure to start treatment but only an education on the problems that may exist. I have seen numerous patients that a restorative approach is a much wiser treatment plan and It is my duty to inform them of ALL of their options and if that approach is better I will recommend they seek out that alternative to Orthodontics. Panache: You are considered to be one of the most successful orthodontists in

Tampa Bay. What do you contribute your success to? Paul: By being me and doing the absolute best for all that I treat. I never like to give up on a treatment and will continue if necessary to achieve the best that is possible in all patients care. Panache: Any new changes in your practice that you would like to share with our readers? Paul: Staying current with advances in Orthodontics is a biggie for me. This is not new but a constant part of what makes my practice what it is. Panache: What would you like to be remembered for most? Paul: Boy that’s a difficult question. I guess wanting to be remembered as a person who feels it is so important in putting others before me. I have been very fortunate not only in my private life but also my professional life. From being married to my “best friend” for forty years and having the greatest kids and grandkids I could ever ask for. I will not and cannot change who I am, no more than a tiger can change its’ stripes. As I mentioned I am a very fortunate individual and I would not change anything for the world.

CHANGING PRESCRIBING HABITS Is Hard to Do, But It Can Save Billions BY DAVID LIPSCHITZ

J

ust yesterday I saw a 70-year-old patient whom I diagnosed with high blood pressure. Like most physicians, I prescribed a generic drug called an ACE inhibitor as treatment. I did so because of information that this medication not only brings down high blood pressure but also reduces the risk of heart attacks and heart failure, major causes of death in older persons. Recently, the National Institutes of Health began a new program to persuade physicians that a water pill, called hydrochlorothiazide, is the initial treatment of choice for hypertension. Despite a large study showing this water pill was as effective in lowering blood pressure and reducing the risk of heart attacks and heart failure as

the more expensive medications, very few physicians have changed their prescribing habits. Currently, less than 20 percent of newly diagnosed high blood pressure patients are initially treated with hydrochlorothiazide that costs, on average, $36 to $96 annually. By contrast, the cost of an ACE inhibitor ranges from $90 to $468 annually. Other commonly used medications are calcium channel blockers, beta blockers and variants of the ACE inhibitor called ARBs, all of which range in price from $240 to more than $700 annually. There is one more wrinkle in the treatment of high blood pressure that I must mention. For many overweight patients with mildly elevated blood pressure, weight loss, exercise and restricting salt content can effectively bring the blood pressure into normal range. As a physician who believes passionately that cost should always be a consideration in medication choice, I am forced to dwell on why I am so reluctant to change, particularly as I am fully aware of the results of the large study demonstrating the benefits of a water pill in treating hypertension. My choice is related to force of habit, a comfort level with the use of an ACE inhibitor and the fact that the medication is effective with few side effects. And yet, until more information

appears to the contrary, I must change my prescribing habits and recommend lifestyle changes first and, if necessary, initiate treatment of hypertension with a water pill. Please remember, though, that an even more serious concern about high blood pressure is the fact that many patients with this disorder are either not treated at all or receive inadequate treatment. While patients with high blood pressure should be initially treated with hydrochlorothiazide, for many, the addition of another medication, such as a beta blocker or an ACE inhibitor, may be needed to bring the blood pressure down to the normal range. Because such a large fraction of the population has high blood pressure, a minor adjustment in the habits of a physician can save billions. And this is one of many examples. Zocor to treat elevated cholesterol costs about $40 annually, and now that Lipitor is generic, its cost is coming down. And billions are spent each year on the prescription drug Crestor, which is only needed in a small fraction of patients for whom lowering cholesterol is extremely difficult. There is an important lesson for you the consumer. It is absolutely critical that everyone become educated consumers of health care. The information available in our newspaper on almost a daily basis, on the Internet, from the media and newsletters provides a great deal of useful information that will allow you to ask your physician key questions about the proposed treatment plan. Always consider cost as well as effectiveness in choosing a medication, and always ask if strategies such as diet and exercise may have the same effect. Even if you are blessed with an insurance program that has a medication benefit, remember that the most expensive is not necessarily the most effective. Though I have used high blood pressure as an example, there are many other common ailments where the expensive medication may not be more effective than the cheap over-the-counter and generic drugs. If our health system is to survive in the future and flourish, our health care choices must become more rational and appropriate. Consumer and physician education are the key to holding down costs while assuring the highest possible quality of care.

MAMMOGRAMS STILL WARRANTED Despite Evidence Their Value Is Overrated BY DAVID LIPSCHITZ

E

very week, one of my patients tells me, “I hear I do not need to have a mammogram -- it causes more harm than good.� This impression comes from numerous studies, primarily from Europe, that have questioned the value of mammograms. The most radical proposal, published in The New England Journal of Medicine, recommends mammogram screening for breast cancer be abandoned. This report, proposed by the Swiss Medical Board, examined the conclusions of a number of research studies demonstrating the value of mammograms. It determined the conclusions might be erroneous. Based on its analysis, mammograms for breast cancer did not prolong life and often resulted in unnecessary surgery that led to a significant incidence of complications. At worst, the needless additional tests, biopsies and even therapy for cancer that would never cause harm may lead to increased

mortality and greater suferring. Their report states that at best, mammograms may save one life for every 1,000 mammograms performed over a 10-year period. Needless to say, this report has led to a storm of controversy in the medical community. Until now, studies have never suggested abandoning mammograms, although the United States Prevenatitive Services Task Force recommends beginning mammograms at age 50 and performing them every 2-3 years, rather than annually. What does a mammogram look for? An abnormality is identified in about 6-8 percent of women. In some cases, this abnormality is clearly benign, and in others, a finding may either diagnose a cancer or suspicious lesion that should be evaluated further or rescreened in 6 months. In many women who have a suspicious lesion, additional more-detailed imaging studies and biopsies are performed. Finally, and with a great deal of relief, most are told that no cancer was identified. In some, a tiny cancer is identified, leading to surgery, and on occasion, radiotherapy and chemotherapy. Then why this controversy? Opponents of mammograms have closely evaluated very large studies and are of the belief that many small cancers may resolve spontaneously and that treatment, therefore, is unnecessary. False positives, unnecessary biopsies and anxiety are all worth it if early detection of a cancer prolongs life. If a woman

has an annual mammogram for 10 years, the chance of a false positive leading to further testing is almost 50 percent. And the earlier detection of cancer does not improve life expectancy or reduce breast cancer death. No question the defenders of mammography disagree that the test does more harm than good and strongly believe that deaths from breast cancer have been dramatically reduced. While this contreversy rages, and scientists have widely differeing opinions, it does seem reasonable that woman continue to have mammograms but be more attuned to the potential benefits and harm. Perhaps an annual mammogram might be too frequent. If a mammogram is normal, there is little evidence to suggest delaying the next test for 24 months will identify more cancers than an annual test. In women who have no significant risk factors for breast cancer, a test every three years might be adequate. But this should be limited to women who have no family history of breast cancer (mothers, sisters, aunts, grandmothers), have never had an abnormal mammogram or breast cancer, and have not taken oral contraceptives for 10 or more years. There are even genetic tests that can be performed to identify women who are at an increased risk of developing breast cancer. The value of mammograms in women older than 75 is questionable. Most important medical organizations do not have firm recommendations for mammograms within this age group. For example, the American College of Physicians recommends mammograms every two years for women between the ages of 50 and 75 and is against mammograms in women either older or younger than these ages. In contrast, the American Cancer Society recommends a mammogram every one to two years beginning at age 40. It does seem that in health nothing is absolutely certain. However, until much more information becomes available, please have a mammogram. If you are one of those unlucky few who have developed a cancer, I have no doubt that early detection will dramatically increase remission rates and that the test will have saved your life.

alance serves a purpose just like anything else in life. The difference is its purpose is far greater because once you obtain it you will begin to flow naturally like a river through every other aspect of your life. The reasoning behind this is balance teaches you to be kind to yourself, gives you clarity, as well as helps you feel calm and grounded. So how do you successfully balance work, family, and everything in-between? Webster’s Dictionary describes balance as: A state in which different things occur in equal or proper amounts or have an equal amount of importance. In order to decide what the proper amount of anything in your life is you must discover and then declare what you and your family value most. Does your spouse or significant other value family time over money? Do you value relaxation over being busy? Do your chil-

dren value quality time over regular play time? If your values are different then you should work towards a compromise which is another form of balance. Just remember, the less you align your family’s schedule with everyone’s values, the more unsatisfied and stressed everyone will be. Once you can define what your family’s values are then you can work towards planning your family’s schedule. This requires listening, communication, and joint decision making. Scheduling for me is organization at its finest. I carry around a planner as well as utilize my calendar on my phone. The best part is with technology today you can set up a google email account for you and your spouse which will enable both of you to see and edit the calendar. Think for a minute about successful people you know or have heard of. Do you believe they accomplish all they do without having a schedule? Better yet, do you believe they accomplish everything on their own? They don’t, it’s virtually impossible to be successful at meeting everyone’s needs in a balanced manner without a schedule or with an attitude of selfishness. Now that your family is set with a schedule remind them to be present. I am constantly reminding myself to be present. Often times while I’m driving I catch myself thinking about everything imaginable. When I realize this, I look out into the sky for a moment and repeat the phrase Be Present. The last thing you want is to be thinking about paying bills while spending quality time with your children

or thinking about work while you are on a date with your spouse. Sticking with the idea of keeping a schedule reminds you there is a time and place for everything. What you don’t want to do is be so rigid that you become inflexible. In order to become satisfied balance should be applied to all areas of your life rather than solely work-family balance. What this means is outside of work and family you must find balance with your mind, body, and spirit as well. This type of balance perpetuates health, knowledge and wisdom. This can be achieved as a family or on an individual level. Maybe you enjoy working out alone but have a family bible study every week. If your children are young you may need to assist them in developing a plan to meet their goals but it needs to be their goals, not yours. Remember, taking it easy never produces a champion and no matter what, your goal should be for you and your family to WIN! I wish you the best as you embark on your journey to achieving balance! NEVER IGNORE SUSPICIOUS SKIN LESIONS BY DAVID LIPSCHITZ

M

y teenage years could not have been more idyllic. I spent summers lazing at the swimming pool or on the beach. In those days, sun tanning was routine, and sunblock was hardly ever used. I remember always being beet red from the sun. My nose was often so severely affected that bleeding was not unusual. This behavior, so common in those days, is leading to an explosive epidemic of skin cancers today. At age 50, it happened to me. I noticed a little sore on my nose that bled and would not heal. After a few months, I went to see a dermatologist, who shaved it off and sent it away for analysis. Sure enough, it was a skin cancer. The doctor felt it was so small that no more treatment was needed. All was well for about a year, but then a little white bump developed in the scar. I had a recurrent cancer. I needed a special kind of op-

eration called Mohs surgery. An expert at managing skin cancer, my dermatologist excised the tumor and examined it under the microscope. The purpose was to make sure that the cancer was completely removed. He had to cut out tissue five more times to remove the entire tumor, which had spread deeply into the nose. On the following day, I had reconstructive surgery. This was the bad part, as it took about three hours. The plastic surgeon worked wonders: You have to look really hard to see that I’ve had major nasal surgery. I had a squamous cell cancer, a type that spreads more rapidly and is more likely to recur than the more common basal cell cancer. In retrospect, the physician who initially treated me did not take the cancer seriously enough. More aggressive removal the first time would have prevented the recurrence. Fortunately, it is now many years later, and I am almost certainly cured. I do see a specialist regularly and have had 10 more small cancers removed over the years. Skin cancer is the most common cancer in humans. Of these, the most frequent is basal cell cancer, which grows locally and is readily cured by surgery. Unfortunately, many individuals ignore lesions

on their face and wait until the tumor is very large before seeking help. When this happens, they may need major surgery in which a large portion of the face, nose or ears has to be removed. Squamous cancers are less common but curable, provided treatment is appropriate. The more sun you are exposed to over time, the greater the risk of these types of cancer. The most serious skin cancer is melanoma. This is related more to severe sunburns than to total sun exposure. Fortunately, my numerous burns have not led to melanoma. My sister and mother were not so lucky. Both were diagnosed with small tumors. The incidence of melanoma is markedly increasing, and since this type of tumor has often spread widely by the time of diagnosis, it often leads to fatalities. Melanomas are treated by widely excising the tumor and, if possible, the lymph nodes that drain it. Often a skin graft is required. It is important annual physical examinations include an evaluation of the skin to screen for cancer. Do not ignore a sore that does not heal or a mole that changes color, has developed recently, grows or bleeds. Moles are particularly serious on hands, feet and genitalia. However, melanoma can occur anywhere. The most important fact about skin cancer is that it is definitely preventable. Sun exposure is bad for you and should be avoided. Ultraviolet light is the major cause of all skin cancers, including melanoma. Not only does sun exposure cause cancer, but it also contributes to wrinkles and to an array of skin blemishes that are often unsightly and occasionally premalignant. So if you want to stay beautiful, avoid the sun, and wear a sun hat and longsleeved shirts on very hot summer days, and, of course, use sunblock liberally. If possible, avoid sun tanning. Tanning beds may be just as dangerous. Remember too that a great deal of ultraviolet-light exposure occurs on hot cloudy summer days. Most importantly, do not allow your children or grandchildren out of the house without a hat and a complete covering of sunblock.

I

t seems sibling rivalries may begin before there are even siblings. In a newly published paper, Harvard evolutionary biologist David Haig puts forth an extraordinarily creative hypothesis for why newborns and infants cry so much at night: It means mom is more likely to be continuously exhausted, not ovulating, and thus delaying the arrival of rivals. To be sure, Haig isn’t actually suggesting the caterwauling cuties are consciously conspiring. Sometimes, maybe most times, they’re just hungry. But he told NPR, “It’s clear that babies can get enough milk even if they sleep through the night. The waking becomes a different issue ... I’m just suggesting that offspring have evolved to use waking up mothers and suckling more intensely

to delay the birth of another sibling.” Haig posits that delaying pregnancy and the appearance of other offspring improves a newborn’s survival chances, particularly in tough times when food might be scarce. He concedes that there’s really no way to test the hypotheses. Perhaps it’s just something to ponder at, say, 2 in the morning.

CHOLESTEROL, CALCIUM AND VITAMIN D BY CHARLYN FARGO

H

ere’s another reason to take that calcium and vitamin D supplement (or get a little bit of sunshine) -those bone building nutrients may also help your cholesterol levels. A new analysis of data from the Women’s Health Initiative found that supplements of vitamin D and calcium might modestly improve cholesterol numbers. Previous studies were inconclusive, according to Tufts University Health & Nutrition Letter (June 2014). It’s always best to get most of your calcium through your diet and supplement to fill in the gaps, however, getting enough vitamin D from dietary sources alone can be challenging. A glass of milk provides 100 International Units and most other food sources are less. The recommended RDA for vitamin D for both men and women is 600 IU for ages 19 to 70 and 800 IU for ages 71 plus. In the new study of supplemental vitamin D and calcium’s cholesterol effects, published in the journal “Menopause,” researchers looked at data on 576 participants over an average of six years. Participants were randomly assigned to either 1,000 mg of calcium plus 400 IU of vitamin D supplements or a placebo. The original trial of the Women’s Health Initia-

tive focused on fracture risk and colorectal cancer in postmenopausal women, rather than cholesterol levels. But a subset of the study group had lipid and vitamin D blood levels tested at baseline and years one, three and six. Supplementation was association with an average 4.46 mg/dL decrease in unhealthy LDL cholesterol compared to the placebo group. Triglycerides also decreased in the supplement group, while healthy HDL cholesterol levels increased. Every 38 percent increase in vitamin D levels in response to supplementation was association with a decline of 1.28 mg/dL in LDL cholesterol. You can also boost your vitamin D levels with just a few minutes of sun exposure. Body fat acts like a storage battery for vitamin D. During periods of sunlight, vitamin D is stored in fatty tissue and then released when sunlight is gone. Q: My friend says that spot-reducing exercises can’t really target fat at particular areas of the body. Is that true? A: Exercises that focus on particular body parts can be very effective at strengthening the specific muscles involved and may lead to a more toned appearance, but they do not reduce the amount of fat in that particular area of the body. Depending on the specific muscle an exercise works, it can help you maintain good posture, thus reducing or avoiding lower back pain; promote better balance, thus reducing risk of falls; improve performance in sports, including cycling, tennis and golf; and improve your ability to keep up with life activities like climbing stairs, gardening or carrying suitcases and groceries. What’s more, research is looking at how exercising a muscle may activate signaling in cells that controls hormones involved in blood sugar control and other important health functions. Fire up the grill this weekend. Here’s a recipe for Grilled Chicken Thighs with Peach Lime Salsa from Cooking Light magazine (June 2014). Grilled Chicken Thighs with Peach Lime Salsa • 3 tablespoons olive oil • 1 teaspoon ground cumin • 6 garlic cloves, minced • 6 (5-ounce) skinless bone-in chicken thighs, trimmed • 2 tablespoons finely chopped red onion • 1 tablespoon chopped fresh basil • 1 1/2 teaspoons chopped fresh mint • 1 1/2 teaspoons fresh lime juice • 1/2 teaspoon sugar • 2 ripe peaches (about 1/2 pound), peeled and chopped • 1 red or green jalapeno pepper, minced • 3/4 teaspoon salt, divided • 1/2 teaspoon freshly ground black pepper • Cooking spray • 6 lime wedges (optional) Place first 3 ingredients in a large bowl, stirring with a whisk. Add chicken; toss well to coat. Cover and refrigerate 1 hour. To prepare salsa, combine onion and next 6 ingredients (through jalapeno); stir in 1/4 teaspoon salt. Let stand 1 hour. Preheat grill to medium-high heat. Remove chicken from marinade; discard marinade. Let chicken stand at room temperature 30 minutes. Sprinkle chicken evenly with remaining 1/2 teaspoon salt and black pepper. Place chicken on grill rack coated with cooking spray; grill 6 minutes on each side or until done. Serve with salsa and lime wedges, if desired. Serves six (serving size: 1 thigh and about 1/3 cup salsa). Per serving: 194 calories, 20 g protein, 5 g carbohydrate, 10.4 g fat, 110 mg cholesterol, 1 g fiber, 367 mg sodium.

GOOD TIMES, NO Worries on a Group Tour to Italy BY GLENDA WINDERS

T

he typical response I get when people find out I went to Italy with a tour group is a wrinkled-up nose, a shake of the head and a dismissive “That wouldn’t be for me.” They imagine spending hours in a motor coach, moving to a new place every day and having to put their luggage in the hallway for a 5 a.m. pickup before heading out yet again -- and they’re right. But in concentrating on the less attractive aspects of group travel, they forget that someone else makes hotel reservations, picks good restaurants and buys museum passes. Another positive is that the price of all these experiences is lower because the tour company gets group rates. And while there is a certain amount of mingling with fellow travelers, there’s also plenty of free time to pursue activities that aren’t on the schedule. My adventure began when I saw a newspaper ad for “Classic Italy -- Rome, Florence and Venice” offered by Holiday Vacations. I rang up my travel buddy, Julie Kean, and she said, “Let’s go!” The trip began when travelers from all over the country arrived in New York and met up with our tour guide, Sue McNamara. An overnight flight took us to Milan, where we were met by our Italian-speaking guide, Bruno Monteiro, and our genial coach driver, Ricardo. That’s when the fun really started. As we drove through the countryside Monteiro gave us some background about the sights we were headed to see. He explained that the country was established just 153 years ago, which explains why

people were known before then by the province from which they came -- Romans, Venetians and Florentines. He told us that although the country is small (850 miles long and 80 miles wide), it has been important historically because of its location at the intersection of major trade routes. Before long we stopped at Sirmione in the province of Lombardy (named for a barbarian tribe that invaded in the sixth century), where we explored this lakeside resort town and toured Scaliger Castle. Then fortified by lunch at Ristorante Erica -- bruschetta, lasagna, pork scallopini, Italian bread and house-made hazelnut ice cream drizzled with chocolate -- we set off for Padua, where we would spend the next two nights. This city, we learned during a walking tour with Monteiro, is most famous for being where St. Anthony -- to whom Catholics still pray for lost items -- lived and

worked. That being the case, most of the visitors here are pilgrims, but Padua is also known for its university, the second oldest in Italy, which was established in 1222. (The oldest is the University of Bologna, founded in 1088.) The next morning found us on a ferry headed for the magical city of Venice, which is actually situated on 118 islands connected by bridges and canals. As a group we toured the Doge’s Palace and learned about the area’s political history before crossing the Bridge of Sighs -- so named because that’s where condemned prisoners had their last look at the outside world -- and walking through the prison museum. Since Venice is famous for its glass, we took in a glassblowing demonstration and learned how mineral oxides create jewel-like colors as well as how to tap a piece lightly and listen to its tone to determine if it was handmade.

We had the afternoon free, so Julie and I opted for a lunch of gelato and a long walk through several neighborhoods to the Guggenheim Museum. We rejoined our group just in time for a gondola ride and dinner at Taverna di Doges before getting back on the ferry and watching Venice recede into the mist behind us. We spent the next day en route to Florence with a long stop in the city of Ferrara before crossing the Apennine Mountains. “We are arriving in Florence and the sun is shining,” Monteiro said. “What else do we need? Nothing!” Our hotel was centrally located on the Piazza Santa Maria Novella, so we could walk everywhere we wanted to go. And that’s what we did the next morning when we met our guide, Elvio Ancilotto, whose credentials identified him as a professor of Italian language and culture. As we moved from the magnificent Santa Maria del Fiore, Florence’s cathedral, to the Santa Croce Church where Michelangelo, Galileo and Machiavelli are buried he explained that Florence was the birthplace of the Renaissance, pointing out architectural details along the way that supported his point. At the Accademia Gallery we bypassed the long line of visitors waiting to get in because of arrangements previously made for our group. Inside, Ancilotto gave us what amounted to a short course in art history

as we walked slowly past Michelangelo’s “Prisoners,” and by the time we arrived at the statue of David we knew how tall he was (17 1/2 feet), why he was depicted nude (Renaissance artists saw the human body as a celebration of proportion) and why one of his feet is connected with a tree stump (to give the statue balance). Florence is as famous for leather and gold as Venice is for glass, so our next stop was at a showroom where Julie and others from our group were selected to model the coats and jewelry the company had on offer. Once again we had the afternoon free, so some from our group set off along the Arno River toward the Ponte Vecchio (“old bridge”), the only one in the city not bombed by the Nazis during World War II. The stores along the bridge that once housed butcher shops today sell mostly jewelry and souvenirs. Others toured the Uffizi Gallery, home of Botticelli’s “Birth of Venus” and significant works by Titian, Raphael, Rembrandt and da Vinci. Our trip to Rome the next day took us through Tuscany with a stop at San Gimignano, a well-preserved village from the Middle Ages where the families tried to outdo one another in building towers so that at one point there were 72. Two famous gelato stores are here, so Julie and I kept up our tradition of having some -- but not until we’d first eaten some panini at an outdoor cafe.

After dinner our first night in Rome Monteiro surprised us with an unscheduled walk that led to a lit-up St. Peter’s Square. It was the perfect introduction before our tour the next morning with Filippo Tucci -- another knowledgeable and passionate guide who led us through art galleries, the Sistine Chapel and St. Peter’s Basilica. When the organized tour was over, some from our group climbed up to the dome and cupola of St. Peter’s and later attended a Mass. Julie and I visited the house next to the Spanish Steps where English poet John Keats worked and died. Tucci met us again the following day for a tour of the colosseum, the Roman Forum and Trevi Fountain, where we tossed in a coin in hope of a return visit. That night we ate dinner at LaTana dei Re, where opera singers serenaded us with various arias and ended up with “Arrividerce Roma.” Next morning we had a last gelato at the airport and began the long flight home. “Touring takes the stress out of traveling and leaves the best parts,” MacNamara said. “You’ve seen the highlights of classical Italy while at the same time relaxing and enjoying the group. You knew we’d take care of you.”
