Oman's healthcare Industry

From an estimated $3.2 billion in 2017, current healthcare expenditure in Oman is expected to grow at a compounded annual growth rate (CAGR) of 9.1 per cent to $4.9 billion in 2022, according to the GCC Healthcare Industry report published by Alpen Capital, an investment banking advisory firm. The second-fastest growth in the region is attributable to rapidly rising population, the roll-out of mandatory insurance during the year and rising cost of care. Further, treatment of non-communicable diseases and preventive care measures are accounting for a large portion of the healthcare expenditure. Consequently, current healthcare expenditure on outpatient and inpatient services in Oman is projected to grow at an annualised average rate of 10 per cent to $1.5 billion and $2.3 billion, respectively, by 2022. Expenditure on other healthcare services is expected to grow at a compounded annual average rate of 6.6 per cent during the projected years. To accommodate the growing base of patients, the bed requirement in Oman is anticipated to grow at a CAGR of 3.2 per cent through 2022, translating into a demand for more

than 1,100 new beds to reach a capacity of 7,937 beds. “GCC healthcare industry continues to offer a wide gamut of investment opportunities. Though traditionally regional governments played an instrumental role in building the sector, shrinking oil revenues have slowed spending. At the same time, the role of private sector is increasing, encouraged by government incentives, mandatory health insurance and other reforms,” says Sameena Ahmad, Managing Director, Alpen Capital (ME Ltd). “Even though regional governments continue to shoulder a sizeable part of the healthcare expenditure, in the backdrop of budget deficits, the importance of private sector participation is being widely discussed across the Gulf Cooperation Council (GCC) nations,” says Krishna Dhanak, Executive Director, Alpen Capital (ME) Ltd. “With increasing opportunities for the private sector, the healthcare industry is witnessing a surge in mergers and acquisitions. The inorganic route is being adopted by new players to enter the market and by existing providers to expand market share, physician practices and medical capabilities,” he added.

Health Care – Services As of June 16, 2018, Oman’s total population numbered 4,612,824 and is steadily on the rise. The country currently has more than 69 hospitals, with a total of over 6400 beds and with Oman’s population expected to double by 2050, Ministry of Health (MoH) aims to establish up to 10,000 health centers to meet the demands of a growing population. Health Vision 2050 also points to the growing need for geriatric care facilities, with the number of elderly people living in Oman expected to increase five-fold through to 2050. Those over the age of 60 are expected to increase from 6.1% of the population to 13.1% in 2050. In 2015, the MoH expanded the number of primary health care institutions under its management to 235 in order to continue to provide comprehensive primary health care to everyone in Oman. These included health centers, facilities and local hospitals managed by the MoH. It is estimated that more than 95% of the country’s population now lives within five miles of a medical center.

Industry outlook According to Alpen Capital, Current Healthcare Expenditure (CHE) in the GCC is projected to reach $104.6 billion in 2022, registering a CAGR of 6.6 per cent from an estimated $76.1 billion in 2017. Expanding population, high prevalence of non-communicable diseases (NCDs), rising cost of treatment and increasing penetration of health insurance are the factors auguring growth. Given the ageing population and an expected increase in the frequency of visits to clinics for treatment and preventive care, the outpatient market size in the region is predicted to grow at an annualised average rate of 7.4 per cent to $32.0 billion between 2017 and 2022. The inpatient market is anticipated to increase at a CAGR of 6.9 per cent to $45.4 billion. Current health expenditure on ‘Others’ is expected to grow at a compounded annual average rate of 5.2 per cent during the forecast period. Growing size of population, and rising

cost of medicine and ancillary services will be the forces driving the spending on other healthcare services. Between 2017 and 2022, country wise current health expenditure is anticipated to expand at annual average growth rates between 2.6 per cent to 9.6 per cent. The growth range is wide due to country-specific projections of population, cost of healthcare and other factors. In view of the anticipated rise in number of patients, the region is expected to require 12,358 new hospital beds by 2022. This translates into an estimated annual average growth of 2.2 per cent from 2017 to reach a collective bed capacity of 118,295.

Healthcare Overview

Dr Ahmed Mohammed Obaid al Saidi, Minister of Health, ha said that the expansion and easy accessibility of the health services are one of the priorities of the government under the vision ‘Health 2050’, which has led to the establishment of a medical city.” The Medical City will accommodate several existing sub-specialties in the addition to the new services that will be offered at the property. On completion, the Medical City will ensure the availability of quality healthcare services to most people in the Sultanate, the minister said. Spread on over an area of 5 million sqm in Barka, the Medical City will comprise several specialty hospitals consisting of about 1,200 beds. The medical city will facilitate training and education within the country, thereby cutting on the expenditure incurred by sending medicine students spent abroad for the same purpose. The City will help improve the level of training and Continuing Medical Education (CME) for healthcare professionals, and will also promote the scope for clinical research in priority areas. The Minister said the medical city project will be also the first step in the direction towards developing has a hub for medical tourism, which will provide specialized medical care and offer complete infrastructure for training, research and studies and provide specialized medical consultations.

The minister said that there were 1,124 private institutions at the end of 2016, including 19 hospitals, 310 specialized clinics, 251 dental clinics, 48 Chinese medicine clinics and 27 medical laboratories, seven medical diagnosis centers and around 11,634 employees of which 2,134 were physicians – 44 per cent of them are specialist physicians, 868 dentists, 1,798 pharmacists and 3,515 physicians. The ministry grants licenses to the healthcare workers following a process of written and oral tests to ensure their eligibility. He said the expenditure of the Ministry of Health amounted to RO792.96 million for 2016 of which RO757.96 million is for operational purposes and RO 35 million for development expenditure. The percentage of Omani doctors in the Ministry of Health is 33 per cent, compared with that of nine percent in 1990, followed 74 per cent among dentists and 87 per cent among pharmacists. There were nine doctors for a population of 10,000 two decades back in the 1990 compared to 19.5 doctors for every 10,000 in 2016. The number of nurses for a population increased 26 in 1990 to 44.8 in 2016.

Governments across the region have either made health insurance mandatory or are in the process of doing so. The gradual rollout of compulsory health covers across the region will increase the utilisation of medical services at private healthcare facilities.

Rising and ageing population, increase in life expectancy and low infant mortality rate are the key demographics driving the region’s healthcare system. The GCC population size is anticipated to increase by 6.6 million individuals to 61.6 million by 2022, of which nearly 17 per cent will be people aged 50 years and above.

An increase in the incidence of non-communicable diseases has brought about changes in the epidemiological profile of GCC countries. Considering the high cost and length of treating such lifestyle ailments, the healthcare expenditure in the region is expected to rise.

Regional governments have framed long-term strategies aimed at capacity expansion, enhancing the effectiveness of the delivery system, encouraging public-private-partnership (PPP) models, developing medical education and digitization. Such strategies will upgrade the capacity and quality of care.

The GCC has over 700 healthcare projects worth $60.9 billion in various stages of development. This massive project pipeline is likely to augment the scale of healthcare services over the coming years.

Medical tourism is an integral part of the economic diversification plans of the GCC countries and subsequently has been receiving stimulus from the governments. Dubai and Abu Dhabi are at the forefront amongst the GCC nations in attracting medical tourists.
