INTRODUCTION TO THE WORKBOOK Welcome to your Return to Work programme, which aims to help you settle into your job. This Return to Work checklist includes information which you need to know about working for the organisation, and other details you may find useful. This checklist will be completed for all staff returning to the department, and is designed to ensure that all aspects of your Return to Work are covered in a timely and effective manner. As each item is discussed it will be signed off by the person providing the information, and by yourself, once you feel the information has been adequately covered. If any item does not apply to your post please mark N/A. If you feel that any area has been missed, and you require further information, please bring it to the attention of your line manager. Once the Return to Work workbook is complete, you and your line manager should sign the completion form and send a copy of the form to: Amanda Griffiths, OD Admin Support Officer HR/OD Shared Service Ground Floor, Broad Lea House Bradley Business Park Dyson Wood Way Huddersfield HD2 1GZ The copy of the completion form will be scanned into your personal file as proof of completion of induction.
