Issues/Risks to Outcomes/Indicators (please detail whether they are vital signs (VS), local area agreement (LAA or part of the national indicator set (NI) of targets) CHOOSING HEALTH VSC26: Rate of hospital admissions per 100,000 population for Alcohol alcohol related harm. [NI 39] LAASSCLP13: Percentage increase in adult dependent drinkers exiting structured specialist treatment through care planned discharges Vital Sign: Total number of people aged 16 and over on GP register, Obesity with a BMI recorded in the last 15 months. VSB09/NI56: Obesity in primary school age children in Year 6. Physical PALS Concern -due to 6/7 week waiting lists for access to scheme as Activity demand exceeds current capacity. Concern - PHCTs re referring patients to scheme. Concern - Patients referred under pain criteria not ringing to book appointments. Concern – PALS being included in pathways of other HITs so further increasing demand. Risk – loss of confidence in PALS from patients and health professionals due to inability to access scheme. Actions – Extending appointment times for existing PALS patients to accommodate more first appointments. Additional appointments if possible, exit interviews undertaken over the phone, re referrals to be reviewed. Prioritization of referral criteria /locality etc Start /MEND Concern – Funding runs out for staff including food specialist in Oct 2010 for Young PALS/Start. Risk – loss of a significant element of Young PALS/Start and reduced number of overweight/obese accessing programme. Action – discussion with obesity programme board, increase number
