1st Annual Christian Car, Truck, and Bike Show & Fall Festival Thank you for choosing to participate in our event! Enclosed in the following packet are the rules and regulations for the event and a vendor registration form. Your signature on the regulations form, completion of the registration form, and receipt of your payment are required for your participation in this event. Please note: The event will be held on Saturday, September 11, 2010 from 8:00am to 6:00pm. This event WILL NOT BE CANCELLED DUE TO INCLEMENT WEATHER. If you choose not to attend or participate due to the weather, no fees will be returned to you. All events proceed rain or shine. There will be no rescheduling. There will be power sources made available to you at your request. It is the vendor’s responsibility to provide extension cords and/or power surges needed to connect to power sources. VENDORS MUST SUPPLY OWN DUCT TAPE TO SECURE ALL CORDS, AND IT IS VENDORS’ RESPONSIBILITY TO SECURE CORDS. The vendor is responsible for bringing all tables and all other equipment and items needed to set up the booth. Each vendor space is 10ft x 10ft. You may purchase multiple spaces if you feel additional space is needed. 1st Annual Christian Car, Truck, and Bike Show & Fall Festival Vendor Regulations Form VENDORS ARE REQUIRED TO SUBMIT A FULL LIST OF ALL PRODUCTS SOLD PRIOR TO THE EVENT. NO VENDOR IS ALLOWED AT THE EVENT UNLESS THEY HAVE BEEN CONTACTED BY THE EVENT MANAGER. Each Vendor must initial each line to acknowledge the rules of the show: All vendor booths must be reserved in advance. No booth will be held without the required deposit. Assignment of vendor space will be at the event host’s discretion and the vendor must exhibit within the space provided. Full payment is required to reserve space. No space will be reserved until payment is received. A vendor may cancel this agreement upon the receipt of written notification to the event host. Vendor will be notified within 10 days of receipt of the registration form of acceptance to the event. If vendor is not accepted, all fees will be refunded. If vendor is not accepted due to vendor requirements, a new registration may be submitted. The host will provide power to vendor for an additional charge. If you require power, please indicate on registration form and pay applicable fees. All power cords used by vendor must be securely taped to the ground to avoid accidents. VENDOR MUST SUPPLY OWN CORDS AND DUCT TAPE TO SECURE CORDS. The vendor agrees to hold blameless the event host New Citadel of Hope Christian Church and its employees, officials and principals and expressly releases it from all liability from loss or damage caused to persons or property for any cause whatsoever. Vendor shall indemnify New Citadel of Hope Christian Church against all liability or expenses arising out of any claim or injury, damage to any person or property, together with all costs in connection with the defense thereto, including attorney’s fees. VENDOR IS REQUIRED TO SIGN ATTACHED RELEASE AND WAIVER OF LIABILITY AGREEMENT. Load-in begins at 6:00am. NO VENDOR WILL BE ALLOWED TO SET UP BEFORE THIS TIME. No items may be left unattended. No admittance to event area after 6:00 p.m. All vendors must remain open during the event hours of 8:00am to 6:00pm. Packing will not be permitted until the close of the show at 6:00pm. Packing should be completed by 8:30pm. Vendors are required to collect Georgia State Sales Tax of 7.0% on taxable sales. To obtain more information on a Temporary Tax Certificate, contact the Georgia Department of Revenue at 1-877-4236711. Food Vendors are required to be licensed with the Department of Business and Professional Regulation and inspected the day of the event. To obtain more information on a Temporary License, contact the Department of Business and Professional Regulation at 1-800-436-7442. VENDORS ARE REQUIRED TO PROVIDE COPY OF LICENSE. Vendors are required to clean-up their space after the event. Any vendor who leaves garbage in their area will be assessed a $50.00 fine. Food vendors must provide a trash receptacle near their booth and are responsible for keeping the trash at a minimum.

If I change/add a new category or merchandise, I must submit a request to the event host. I understand that any vendor that does not comply with these rules and regulations will be asked to not return.

Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement IN CONSIDERATION of being permitted to participate in any way in the 1st Annual Car, Truck, and Bike Show & Fall Festival hosted by the New Citadel of Hope Christian Church, EACH OF THE UNDERSIGNED, for himself, his personal representatives, heirs, and next of kin: 1. HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE the promoters, participants, sanctioning or administrative organizations or any affiliated entities thereof, sponsors, advertisers, business owners, lessors, and lessees of premises used to conduct the event, premises and event inspectors, surveyors, underwriters, brokers, consultants and others who give recommendations, directions, or instructions or engage in risk evaluation or loss control activities regarding the premises or event and for each of them, their directors, officers, agents and employees, all for the purposes herein referred to as “Releasees”, FROM ALL LIABILITY TO THE UNDERSIGNED, his personal representatives, assigns, heirs, and next of kin FOR ANY AND ALL LOSS OR DAMAGE, AND ANY CLAIM OR DEMANDS THEREFORE ON ACCOUNT OF INJURY TO THE PERSON OR PROPERTY OR RESULTING IN DEATH OF THE UNDERSIGNED ARISING OUT OF OR RELATED TO THE EVENT, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. 2. HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the Releasees and each of them FROM ANY LOSS, LIABILITY, DAMAGE, OR COST INCLUDING BODILY INJURY OR PROPERTY DAMAGE they may incur arising out of my presence or participation in the event, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. 3. HEREBY ASSUMES FULL RESPONSIBILITY FOR ANY RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE arising out of or related to the event whether caused by the NEGLIGENCE OF RELEASEES or otherwise. 4. HEREBY agrees that this Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement extends to all acts by the Releasees and is intended to be as broad and inclusive as is permitted by the laws of the state of Georgia in which the event is conducted and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. As a condition of my participation in this event, I hereby grant the New Citadel of Hope Christian Church and its event, the 1st Annual Christian Car, Truck, and Bike Show & Fall Festival, a limited license to distribute, broadcast, and exhibit my name, logo, and trademark for the purposes of advertising or promoting the event. This agreement does not, however, constitute consent to use my name, logo, and trademark in an endorsement of any product or service without my specific written consent. I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
