Eight Easy Steps

How To Use


StoryCare

®

is an easy to use, web-based system for healthcare organizations and teams to address important issues of patient safety, quality, patient satisfaction, and staff engagement. StoryCare’s 30-day rapid improvement cycle is a focused team simulation that occurs on the job targeting a specific performance outcome. Teams are able to learn, practice, and ‘hard-wire’ new behaviors that are sustainable over time. With StoryCare®, team members collectively decide which behaviors and best practices to adopt based on what works in their setting.

Here’s how it works. Step 1 :

Identify Your Team’s Issue Whatever challenge(s) your team is facing, StoryCare’s robust solutions libraries are ready to tackle it. Search from three libraries with hundreds of stories designed to align with almost any improvement goal, each with a complete set of support materials and coaching guides: StoryCare Essentials : contains our most comprehensive library with stories that focus on the following outcomes: Patient Safety, Patient Satisfaction, Quality of Care, Staff Satisfaction and Professional Development, and Teamwork and Communication. TeamSTEPPS® Tools : contains stories that focus on the implementation of evidence-based tools aimed at improving team performance and patient safety. Partnership for Patients : contains stories that align with the improvement goals to reduce preventable harm.

Select the story that works best for your challenge, your type of unit or work environment, and your team. Or let one of our StoryCare® Coaches help you develop a solution set to specifically address your situation.


Step 2 :

Access Your Story Materials Every StoryCare® story comes with a streaming MP3 audio file to playback a narrated Audioscape of the story, Story Handouts for the team to follow along with and make notes while listening to the story, and a Discussion Guide for the team leader to use in leading the team during the 30-day rapid improvement cycle on this issue. Posters and desktop wallpapers are also provided to keep the story top of mind for the team as they work together and practice these new behaviors.

Step 3 :

Kickoff Huddle | Day One Assemble your team for a 15-minute huddle or include during a regular team meeting. Distribute the story handouts and play the story’s audio file. Afterward, lead a short group discussion about the story using 3 questions designed to facilitate reflection and contemplation by the team and generate new behaviors to address the issue. Send the team off to practice these new behaviors over the next 2 weeks.

Step 4 :

Rounding | First Two Weeks Your StoryCare® Discussion Guide includes rounding tips for what to observe and tools to encourage and support your team as they reflect on the story and practice these new behaviors.

Step 5 :

Halftime Huddle | Day Fifteen Assemble your team for another 15-minute huddle or include during a regular team meeting. Play the story’s audio file again to activate the team’s identification with the story. Afterward, continue the group discussion by asking team members to share their experiences over the past 2 weeks with the new behaviors they have been implementing. Challenge your team to practice additional behaviors their other team members have been using that they would like to embrace. Send the team off to practice these additional new behaviors over the next 2 weeks.

Step 6 :

Rounding | Second Two Weeks Your StoryCare® Discussion Guide includes rounding techniques for how to observe changes in your team’s performance and determine if they are ready to move on to another topic at the end of this 30-day rapid improvement cycle or remain on this topic with a new story from the StoryCare® library addressing this same issue. Save your rounding notes for entry into the End-of-Cycle Progress Report.

Step 7 :

Goal Line Huddle | Day Thirty Assemble your team for a final 15-minute huddle or include during a regular team meeting. Review the new behaviors observed during rounding and have the team determine the behaviors that they believe best solve the issue being addressed. Confirm these new behaviors and commit to making them part of the unit’s practice. Share with the team the decision to move on to a new topic or remaining with this issue and addressing it with a new story from another perspective, in which case you would return to Step 3 and select a new story on this issue for a new four-week cycle.

Step 8 :

Report Your Progress Select the End-of-Cycle Progress Report feature on the StoryCare® website. Enter email recipients that you want your Progress Report to be sent. Fill in the report fields using your facilitator notes you have collected during the past StoryCare® cycle. The report fields encourage entering free text from your success stories. Your team’s StoryCare® experience is now stored in your organizational reporting database for tracking system use and effectiveness. Remember, this reporting feature is a simple way to keep your leadership updated.
