Step 7 :
is an easy to use, web-based system for healthcare organizations and teams to address important issues of patient safety, quality, patient satisfaction, and staff engagement. StoryCare’s 30-day rapid improvement cycle is a focused team simulation that occurs on the job targeting a specific performance outcome. Teams are able to learn, practice, and ‘hard-wire’ new behaviors that are sustainable over time. With StoryCare®, team members collectively decide which behaviors and best practices to adopt based on what works in their setting.
Goal Line Huddle | Day Thirty Assemble your team for a final 15-minute huddle or include during a regular team meeting. Review the new behaviors observed during rounding and have the team determine the behaviors that they believe best solve the issue being addressed. Confirm these new behaviors and commit to making them part of the unit’s practice. Share with the team the decision to move on to a new topic or remaining with this issue and addressing it with a new story from another perspective, in which case you would return to Step 3 and select a new story on this issue for a new four-week cycle.

Identify Your Team’s Issue Whatever challenge(s) your team is facing, StoryCare’s robust solutions libraries are ready to tackle it. Search from three libraries with hundreds of stories designed to align with almost any improvement goal, each with a complete set of support materials and coaching guides: StoryCare Essentials : contains our most comprehensive library with stories that focus on the following outcomes: Patient Safety, Patient Satisfaction, Quality of Care, Staff Satisfaction and Professional Development, and Teamwork and Communication. TeamSTEPPS® Tools : contains stories that focus on the implementation of evidence-based tools aimed at improving team performance and patient safety. Step 8 :
Report Your Progress Select the End-of-Cycle Progress Report feature on the StoryCare® website. Enter email recipients that you want your Progress Report to be sent. Fill in the report fields using your facilitator notes you have collected during the past StoryCare® cycle. The report fields encourage entering free text from your success stories. Your team’s StoryCare® experience is now stored in your organizational reporting database for tracking system use and effectiveness. Remember, this reporting feature is a simple way to keep your leadership updated.
