It is a subject no one seems to be paying much attention to, and yet it can cause serious health issues for dental workers. Our goal in providing information about ergonomic tools and usage techniques is to help our customers avoid common long-term health issues, such as carpal tunnel syndrome and other musculoskeletal disorders, which are both painful and preventable. Besides offering information on ergonomics in this issue, we’re also excited to announce the launch of our new website, at www.nedental.com. The expanded site is up-and-running after many months of hard work. We hope that it makes your job easier. Building on our goal to be a source of education for our customers, we are pleased to announce that Dr. Ross Nash will be conducting an in-house seminar for our local customers this summer. These conditions are serious, painful and can reduce productivity especially in occupations where wearing disposable examination gloves is an essential requirement. Selecting the right hand protection for the task and choosing gloves with an ergonomic design can help increase worker comfort and reduce repetitive motion injuries. To help prevent CTS and other diseases, the American International Health Alliance (AIHA) recommends that companies select gloves that improve worker grip. Gloves should be thin enough that they do not interfere with a worker’s ability to close his or her hands around an object. Furthermore, gloves should be comprised of materials with a degree of surface roughness that promotes a strong grip force, especially in wet or oily environments. In addition, gloves should fit properly and not pinch hands. Nitrile gloves are an excellent alternative for applications that require extra grip. They increase the friction between the hand and the object, which allows the worker to get a better grasp. Gloves are available with texture on the entire gripping surface or simply limited to the fingertips. Texture helps reduce grasping

aintaining a healthy, comfortable and productive work environment for the dental team takes an awareness of the ergonomic risk factors. Often, the best intervention is the simplest. For example, something as simple as choosing alternate instrument grips; body, arm or finger positions; instruments or instrumentation techniques can improve the work environment.

Studies show that dentists have a high frequency of finger-related and other upper-limb symptoms.1 There exists a high prevalence of osteoarthrosis involving the distal interphalangeal joints.2 An association exists between the severity of finger joint osteoarthritis and finger joint pain among female dentists.3 Furthermore, finger joint osteoarthritis and pain increased the risk of low pinch-grip strength.4 It is also known that carpal tunnel syndrome is common among dental hygienists.5 In addition, there may be a greater risk of developing upper extremity disorders among dental hygienists than among dentists due to the long hours of dental scaling and root planing.6

Instrument Design
No industry standard for an “ergonomic” instrument currently exists. Most instruments approximate the size and shape of a No. 2 pencil. When gripping a small diameter handle (1/4 inch or less) the operator often grips the instrument with the very tips of the fingers. During movements requiring force, this manner of grasping concentrates most of the strain within a very small set of hand muscles. Larger diameter instrument handles allow the operator to grip the instruments with the pads of the fingers and consequently distribute the strain through a larger group of muscles.
THICK VS. THIN
In a recent study of the ergonomics of dental instruments7, participants’ perceived the best instruments were those with the thickest silicone handles. The worst instrument was one with the thin metallic handle (⅛“). Participants reported that the instruments with the thickest and second thickest silicone handles caused the lowest strain, both for the fingers/palm and for the thumb. Strain in these body parts was statistically significantly different between the instruments with the thickest handles compared to the instrument with the thickest metallic handle. The conclusion of this study was the use of instruments with thick (diameter 12–14 mm) silicone handles were found to be more usable and cause lower perceived musculoskeletal strain. In addition, the use of these instruments was more productive than the use of instruments with thinner handles in a scaling task. any hand and wrist problems in dentistry come from postural stress – assuming an extreme posture at or near the normal range of motion. There is a neutral zone of movement for every articulating joint in the body. The correct range of motion is defined by movements that do not require high muscular force or cause undue discomfort.
When selecting a handpiece ergonomic preference such as weight, length and pliability (see table)* is just as important as clinical performance. Maneuverability is also very important when dealing with limited-access areas of the mouth, and optimal fit and balance make
SWIVEL COMPONENTS Swivels that connect to the handpiece allow for improved positioning without having to fight torque forces in the tubing and create seemless manueverability. They can transorm a non-swivel instrument into a superior instrument. CORDLESS HANDPIECES A comfortable hygienist is an efficient hygienist. But the combination of cord drag with the daily use of the hands during the prophylaxis procedure can lead to hand and wrist strain over time. Weight Hand-pieces should be as light as possible and well balanced. Length Hose length should be as short as possible; extra hose length adds weight. Avoid retractable or coiled hoses. Cordless handpieces eliminate this. Having a good stool is essential in maintaining good posture, because what you sit on provides the base of support from which you work all day long. You probably work 6 or more hours per day, 4 days per week, 4 weeks per month, and about 11 months out of the year. That makes a high-quality stool with adjustable features well worth the cost of saving your back, neck, arms, and hands. When choosing a stool look for important features like adjustable height, width, tilt, backrest, seat pan, and armrests. In most dental offices many

Ideas about operator positioning, range of motion and equipment have evolved…
T
he quest for “ideal” operator positioning comes from the studies of industrial hygiene (factories not dentistry!) at the middle of the past century. Basic principles—these are the ones that you probably learned in school:

• Back upright • Minimal head tilt • Thighs parallel to the floor (no longer true) • Feet flat on the floor (not always possible with traditional foot pedals) • Patient low to the legs Most dental equipment, operatory layouts, and procedure techniques were developed with an average male dentist in mind (the average man being about 5’10”). As a result, female dentists have been forced to adapt their (usually) smaller and differently shaped frames to a workplace environment designed for a man (average height for a woman is 5’5 1/2”). In addition, if we assume that approximately 98 percent of hygienists are female and 21 percent of dentists are female, that means that over 70 percent of the time the clinician rolling up to the dental chair is a woman, not a man. So the “average” practitioner is, in fact, female. The dental industry has been slow to catch on. “We learn to compromise and put ourselves in positions that feel comfortable,” explains Elizabeth B. Lewis, DDS, of the Indy Dental Group, Indianapolis, Ind.At 5’6”, Dr. Lewis is of average size for a woman, which is shorter than the typical male. Like many other women dentists,

she finds that sometimes she must adjust her body to fit a work situation. This is the opposite of ergonomics, which has a goal of adapting the worker’s environment to fit the worker.

Flexible Equipment: Allows the operator to sit, stand or sit-stand in function and permits work from 7° to 1° for right handed operators (11° to 5° for left handers).
A flexible equipment layout should not place dynamic instruments to the side or rear of the operator or assistant except in very special cases. Equipment flexibility can be accomplished with over-the-patient or over-the-head equipment. But properly designed and placed equipment isn’t enough. Dentists (particularly those outside “average” size ranges) must learn how to adjust the equipment and how to use proper ergonomic techniques to avoid injury. If it is not adjusted to the individual who is using it, the ergonomic features may be useless. TECHNOLOGY AND MANUFACTURING Dr. Lewis speaks for many women dentists when she says: “There is a need for manufacturers to provide a broader range of settings and positions for dental equipment.” Manufacturing has been slow to catch on to new ergonomic ideas, but there are some breakthroughs. Enter new tap-on ultrasonic scalers and ultrasonic polishing systems. These activate scaling with a single tap of the foot pedal, allowing your foot to rest as you scale. They are designed to improve efficiency and reduce hygienist leg strain. Their water control allows for precise and convenient adjustment of lavage on the handpiece and single-push rinse and purge modes.
