Do you have more than one super fund?

There are three easy steps to take when consolidating your super. 1

Provide a certified copy of identification (ID) for each rollover form

Certified ID is when an authorised person sights the original document and a copy of it. They will then certify the copy of your ID by either stamping or writing ‘certified true copy’ followed by their signature, printed name and qualification, e.g. Pharmacist, Australia Post employee.


Take LUCRF Super with you when you change jobs! Changing your job does not always mean a change of super fund. Like many Australians, you may have a choice of where you want your super to be paid. If you do change jobs, ask your employer if you have choice of fund. If you do have a choice, provide your new employer with your LUCRF Super member number – this way your super account will continue as normal. No more lost super! If you would like to arrange for a full transfer to LUCRF Super, please complete this form. For a partial transfer, or if you have any questions, please contact us directly. When you transfer your super, your entitlements under that fund (including any insurance) may be cancelled. You need to consider all relevant information and the consequences before you make a decision to transfer your super. • Fees – your FROM fund(s) must give you information about any exit or withdrawal fees. If you are not aware of the fees that may apply, you should contact your fund(s) for further information before completing this form. The fees could include administration fees as well as exit or withdrawal fees. LUCRF Super does not charge entry or deposit fees on transfer. Differences in fees that funds charge can have a significant effect on what you will have to retire on. For example, a 1% increase in fees may significantly reduce your final benefit. • Death and disablement benefits – your FROM fund(s) may insure you against death, disablement, illness or an accident that leaves you unable to return to work. If you choose to leave your current fund, you may lose any insurance entitlements you have. If you have Death & TPD or Death Only insurance with another super fund, you can apply to have your existing level of insurance matched by LUCRF Super – see our Super Member Guide and Super Member Essentials at or contact us on 1300 130 780 for more information.

What should I consider before consolidating? TICK ✔ BOX

When you change your job, address or name over the years, your super funds are usually the last to know. Not keeping up-to-date with your super can mean you have more than one super fund and are paying multiple fees. Over the years, these fees can add up to thousands of dollars, leaving less for you when you need it most.

See below a list of some authorised people who can certify your ID. • A Pharmacist; • A registered Medical Practitioner; • A Police Officer; • A Justice of the Peace; • A permanent employee of Australia Post with two or more years continuous service; • A finance company officer with two or more years of continuous service (with one or more finance companies); • An officer with, or authorised representative of, a holder of an Australian Financial Services Licence (AFSL), having two or more years of continuous service; • A member of the Institute of Chartered Accountants in Australia, CPA Australia or the National Institute of Accountants with two or more years continuous service.
