Concussion information: In 2011 the State of Wisconsin passed Act 172. Which requires any child under 19, who participates in an organized athletic game or competition, to receive information about concussions and their symptoms. Parents and the athletes must return a signed copy of the “Parent and Athlete Agreement” prior to their participation in each sport. Weather Cancelations: In the case that school may be canceled due to bad weather we will be canceling all recreation department activities. Weekend cancelations will be done via email, Facebook posts or contact from your child’s coach. Winter/Spring Season info: Registration Starts December 1 Season runs December 7 through May 9 Typo, Human Error, We Goofed…!?! Occasionally there may be an error in days, times, registration requirements or fees in the brochure. When such errors occur, our staff will do everything possible to correct the situation promptly. We thank you for your patience and understanding when these situations arise.

General Policy Information Residency: For program enrolment residents are those who reside within the Lake Mills Area School District boundaries. Residents pay the fee directly before the “R”. The second fee listed is for non-residents “NR”. Non-Residents are those who do not reside within the Lake Mills Area School District. Residents support the school district through tax-dollars, while non-residents pay to participant through higher registration fees. Non-Residents will be responsible for paying a minimum of an additional $10 per activity and participant they register.

Insurance: The Lake Mills Area School District and Lake Mills Recreation Department do not provide medical or accident insurance for any program participants both residents or nonresidents. Participants are strongly encouraged to obtain their own insurance coverage prior to participating in programs. Photo Policy: Participants or their parents (if participant is under 18) permit the taking of photos, audio and videotapes during Recreation Department activities for publication and use as the Recreation Department deems necessary. If you do not want a picture or video taken of yourself, please inform the director or instructor prior to the start of the program. Fees/Late Registration: Fees are based off of the minimum amount of participants needed to successfully run a program we will do our very best to anticipate needs for now and the needs for the future of our programs so that we may set our prices accordingly. To ensure proper planning we may set deadlines for our programs. Those who register after a set deadline will have to pay an additional $10 late fee and may also run the risk of not being able to participate if the program is full. If you don’t hear from us, that’s good! After signing up for a program you may not receive a confirmation. Consider yourself registered for that program. Please be sure to mark your calendar with the dates for each program. If a program requires specific information we may send out an informational sheet via email. Notice of Non-Discrimination: No person shall, on the basis of sex, race, national origin, ancestry, creed, marital or parental status, age or disability, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity, including employment. Age/Grade Requirements: All youth program age restrictions will be based on the first day of the program unless otherwise specified. All grade restrictions will be based on the current 2013-14 school year. Summer will officially mark the new school year. Age and grade requirements are set to benefit all children and make instruction easier for the program leader.

Trips NEW!!! Chicago’s Magnificent Mile Shopping Trip Registration Deadline: December 6 Enjoy a fun filled day of shopping surrounded by colorful holiday decoration that line the streets of Chicago. The eight block stretch of downtown North Michigan Avenue is Chicago’s premier shopping destination. Participants will travel by charter bus down to historic Michigan Avenue where you be sure to find that perfect gift for your loved ones. Children under 16 must be accompanied by an adult. We will be meeting at the Lake Mills Middle School (Campus Field) Parking lot off of Church St. in Lake Mills. The bus will leave promptly at 7:00 am so please arrive 15 minutes early. Adult Men’s and Women’s Volleyball 4’s Registration Deadline: March 21 Our Adult 4’s leagues are perfect for individuals looking to meet new people and have fun. Each player will put their name into the draft while team captains draft a new team each year. Anyone can sign up to play and anyone can sign up to be a captain. Adult Basketball Open Gyms This is your opportunity to stay and play some basketball, meet some new friends and get some exercise in! This is a free program open to all adults interested in playing pick-up style basketball. New teams will be formed weekly. As this is an ongoing program be sure to frequently check the facilities calendar for possible time/location changes or cancelations. During this 7 Week participants will utilize equipment like the vault, low/ high beams, uneven bars, parallel bars, and others. Participants will work on balance, strength and coordination while having tons of fun! Recreation/Optimist 3v3 Basketball League Registration Deadline: December 13 New this year! This is a in-house 3v3 league designed to follow the Optimist/Rec Skills and Drills program. Each week participants will be divided into new teams and spend 30 minutes practicing followed by three 15 minute games. The last week of the season (February 15) we will participate in a tournament ending at Noon, followed by the Annual Optimist Tri Star competition and pizza party. This program includes a t-shirt. Optimist Tri Star Competition It’s back! The annual Optimist Tri Star basketball skills competition is run by the Lake Mills Optimist Club. Participants will show off their skills while passing, shooting and dribbling their way through the competition. The top participant in each age bracket will qualify for the state competition. No preregistration is required. This event is FREE!!! NEW!!! Spring Rec Soccer 5K-5 Grade Registration Deadline: March 7 New this year Spring soccer! LMRD offers a pure recreational soccer program for children in 5K- 5 grade. Players will have fun while learning the fundamentals, rules and skills of the game. Practices will start 2-3 weeks before the season . The games will be held at LMHS Practice fields off of E. Lake Park Place on Saturday mornings. Participants will be contacted by their coach with league schedules & team information. Grade/Division 5K/1 (U6)

Description The Pee Wee league will play on a condensed field. They will field a 3v3 team with no goal keeper using a #3 soccer ball. This league is focused on fun and learning the game of soccer.

NEW!!! Baseball & Softball Open Gyms Dust off your glove and warm up that throwing arm. This is a great chance to get yourself warmed up for softball before the weather starts warm up! Pre-registration not required. But all participants must fill out a waiver prior to participation. Waivers will be available to print on line or at each open gym. Volunteer coaches are needed to provide a safe and productive atmosphere. Only indoor balls can be used in the gym.

Girls Softball U8-U16 Registration Deadline: March 21 Windmill Girls Fast Pitch Softball League is an ASA sanctioned league. Girls are divided into divisions according to their age as of January 1, 2014. The Windmill league is comprised rec. teams from our surrounding communities, parent transportation is required. Coaches will contact their team with practice times and team information. Team equipment and uniform tops are provided. For more information about Windmill softball visit

Youth Baseball L-Cats Baseball Camp and Coaches Clinic Get hands on instruction from the LMHS Varsity coaching staff and current LMHS baseball players. Participants will go through a number of drills while receiving valuable tips to help improve their game. There will be a coaches clinic from 2:00 pm-3:00 pm open for all coaches to attend. Little League Spring Training Day Included Little league Registration LMRD will be hosting a mandatory “Training Day” for all kids interested in playing in American or National League baseball. American League (9-10) 11:00 am – 12:30 am, National League (11-12) 12:30 pm - 2:00 pm. Coaches will be selecting teams directly following the event. Little League Baseball Registration Deadline: March 14 LMRD Little League Baseball program is open to both boys and girls who are currently age 9-12. Players must be 9 prior to May 1. Current 2013-14 school year 4 graders who are 8 year-old will be allowed to play in the AL league. Teams will participate in the Eastern Suburban Youth Baseball League which consists of the following surrounding communities: Cambridge, Columbus, Jefferson, Deerfield, Marshall, and Waterloo. Transportation is required. More information about the ESYBL can be found at http://www.esybl.org/index.php There will be a coaches and advisory committee meeting on Wednesday, February 12 at 5:30 pm in the LMMS commons. Parents interested in coaching, assistant coaching, fundraising, concessions coordination should attend. There will be a mandatory parent meeting/uniform handout on Tuesday, April 22 from 6:15 pm - 6:45 pm for American League (9-10) and 6:45 pm - 7:15 pm for National League (11-12) at the LMMS Commons.

ESYBL American League (AL) Registration Deadline: March 14 American League is a recreational traveling league (parent transportation required). This is a development league with rules in place designed to encourage skill development in preparation for National League. Coaches will contact players with practice times and locations when teams have been formed. ESYBL National League (NL) Registration Deadline: March 14 National League is a recreational traveling league (parent transportation required). NL is played more like the traditional game of baseball. Participants in this league will be using more advanced baseball skills and they will learn different strategies used in the game. Coaches will contact players with practice times and locations when teams have been formed. Games are played on Monday and Wednesday nights. Little League Baseball Tournament Come support the Lake Mills Youth Baseball players at our 14th annual fundraiser tournament! Each year we welcome outside teams for a fun filled weekend of baseball that is supported by our parent volunteers! Parents are needed to donate their time to help make this tournament a huge success. Money raised from this tournament will be used to support the baseball program. Pony-Teener League Baseball Registration Deadline: March 14 LMRD Pony & Teener Baseball program both participate in Travel Leagues (parent transportation required). Player will be divided into teams depending on numbers. Participants cannot turn 16 before August 1. There will be a coaches and advisory committee meeting on Wednesday February, 12 at 6:30 pm in the LMMS commons. There will be a tryout day for any 13 or 14 year old interested in playing on the Teener team on Thursday, April 17 at 6 pm at Campus Field or Wallace Park diamond. Only those interested need to attend the tryout. All Current 15 year olds will automatically be on the Teener team due to league rules. A mandatory parent meeting/uniform handout will be held on Tuesday, April 22 at 7:15 pm in the LMMS Commons. Pony League Baseball Code: YBS.PT Dates: Late May thru late July Ages: 13-14 (Grades 7 and 8) Fee: $80/R ; $90/NR (Tax included) LMRD Pony Baseball program participates in the Sandy Koufax Baseball League. This is a transitional league, games are played on a modified field with 80 ft bases and 54 ft pitching rubber. Players will receive practice and league schedules from their coach. Player uniforms will be provided. Home games will be played at Faville Park and or Wallace Park Teener League Baseball Code: YBS.PT Ages: 14-15 Dates: Early June thru late July Fee: $80/R ; $90/NR (Tax included) LMRD Teener Baseball participates in a competitive traveling baseball league. Team practices and league schedule will be set by the coaches. Player uniforms will be provided. Commitment, attitude, skill level and sportsmanship will be considered when determining playing time. Senior Citizens Senior Walking During the winter months we open a welcoming door to the seniors who are looking for a warm and safe place to walk. Days: Monday - Friday Dates: Ongoing – Starts September 3 - Ends June 3 DATES OFF: December 23- January 1 (Winter Break) January 27 February 20, 21 March 24-28 (Spring Break) April 18, 21 May 5, 26 Time: 6:00 am - 7:45 am Location: LMHS Upper Gym and Halls Fee: FREE!

NEW!!! Senior Pickleball- Open Net The game of pickle-ball was created back in 1965, and has been a favorite in PE classes and retirement communities ever since. The tennis like game is a great way for seniors to stay active during the winter months. Participants play with a ball much like a whiffle ball and wooden paddles. The court is a the size of a badminton court with the net just 3 feet off the ground. Games can be played as doubles or singles. Dates: December 3 – March 20 (Except 3/18) Days: Tuesdays and Thursday Time: 6:15 am - 7:45 am Location: LMHS Upper Gym Fee: Free
