place in my heart. Having experienced the shock and scare of a breast cancer diagnosis, I know firsthand how important it is to stay on top of our health. Scheduling an annual mammogram, for example, is key to detecting any signs of breast cancer in the earliest stages. Thankfully, my own breast cancer was detected early, and after a brief treatment, I was declared cancer-free. Navigating the journey through breast cancer is incredibly easier with a team of committed supporters—like Linda Brafford, owner of A Private Affair in Thousand Oaks. Since 1991, the compassionate staff at A Private Affair have been helping women find the perfect bra. With an emphasis on hard-to-fit sizes (A-N), A Private Affair offers women, even those who have undergone breast cancer mastectomies, a custom consultation and bra fitting. “We have something for everyone,” says Linda, and if we don’t have the perfect fit, we’ll get it for you, she told me. In the dressing room, she patiently and efficiently took my measurements and brought me different styles to try. BREAST CANCER IS THE MOST COMMON TYPE OF INVASIVE CANCER AMONG

WOMEN IN THE UNITED STATES AND 1 IN 8 WOMEN WILL BE DIAGNOSED WITH BREAST CANCER DURING THE COURSE OF THEIR LIFETIME, SO TAKING STEPS TO KEEP YOUR BREASTS HEALTHY IS KEY. Routine care is the best way to ensure breast health, and getting a regular mammogram plays a big role in detecting breast cancer at its earliest stages, when it is most treatable. When found early when it is still localized, or restricted to the site of origin, without evidence of spread, breast cancer patients have a relative survival rate at five years of around 98%, according to the National Cancer Institute. When found at a late stage (spread to distant parts of the body), the breast cancer survival rate at five years drops to about 23%. “It is quite devastating for patients when a cancer diagnosis is delayed,” says Dr. Laura Doepke, an Associate Clinical Professor in the Department of Radiological Sciences at the

David Geffen School of Medicine at UCLA, who specializes in breast imaging, particularly imaging of breast cancer. “I don’t want women to wait; we want to screen them, because we want to find it before you can feel it—the shortlived compression of the breast during a mammogram is absolutely worth the long-term benefits,” she adds. Early detection and improved treatment have led to decreased mortality rates for those diagnosed with breast cancer, and screening reduces mortality by more than 40% in women 40 and older, says Dr. Doepke, also the medical director of the new UCLA Westlake Village Women’s Imaging Center that recently opened its doors to provide local women with potentially life-saving imaging services. “In an effort to reach the communities that previously had to drive to our central hubs in Westwood and Santa Monica, the Westlake Village Women’s Imaging Center offers a high level of care by highly specialized physicians—the majority are exclusively breast doctors,” says Dr. Doepke. “We know the need is there. Our primary care doctors in the area would really like for their patients to receive all their care nearby, so they don’t have to drive far. Patients are going to get the same care here in Westlake Village than if they’d driven to UCLA’s main centers in Westwood or Santa Monica,” adds Dr. Doepke, who also practices at UCLA Westwood, UCLA Santa Monica Barbara Kort Women’s Imaging Center, the breast imaging practice at the Motion Picture and Television Health Fund, and the Santa Clarita breast imaging facility, in addition to training residents and teaching UCLA fellows. As a professor of breast imaging at UCLA Health System since 2009, Dr. Doepke notes that technological advances in imaging, such as 3D mammography (Tomosynthesis), offer several advantages for patients. “Breast imaging has changed a lot in the past 10 years, with the advent of 3D mammography, as well as new treatments for breast cancer, which are changing, improving and evolving over time,” she says, noting 3D mammography has been shown to detect more cancers earlier and to decrease the number of call-back rates.

FOR READERS NEARING RETIREMENT, IT IS ADVISABLE TO REVIEW YOUR INVESTMENT ALLOCATION. Yes, the stock market has attained all-time highs as recently as July 15, 2019, and that is notable because people think more about retirement as late stage bull markets reach new heights. Swelling 401(k) balances logically seem congruent with near-retiree optimism and confidence that you may have finally achieved a nest egg sufficient to carry you through your golden years. Yet even for those with fattened quarterly statements, it’s a healthy reminder to make sure your allocation reflects who you are as an investor today. Since you are no longer a 20-something-year-old starting out in your career, your allocation and exposure to risk matters more as you eagerly approach the retirement date you’ve penned in your calendar. The WSJ published an article on August 4, 2019 titled, About to Retire? Watch Your Stock Exposure—Quickly, and it highlights the importance of recognizing the “sequence of returns” that an investor earns and the impact those returns have on the investment portfolio over time. The interesting and maybe surprising reality is that those who retire near a bull market high and kept an allocation heavy in equities often have meaningfully less in their portfolio years later. The reason they have less is because they will suffer from “the double

whammy” effect if stock markets sell off while they maintained both an equity driven portfolio along with taking distributions of 4% annually to complement their retirement income. The impact that distributions can have on a portfolio decreasing in value is significant and can hinder the outcome over a retiree’s lifetime. If you are an investor who owns publicly traded employer stock in your 401(k) account, there is a nuanced decision that you can think about called Net Unrealized Appreciation or NUA. The idea is applicable to employer stock in a 401(k) plan like Lockheed Martin (LMT) or any other publicly traded company. The nuance is this: typically when an employee retires from a company they may choose to do an IRA Rollover. When they do, all of their assets move to the IRA and remain tax-deferred. You only pay ordinary income taxes as you decide to take withdrawals from the IRA account. With Net Unrealized Appreciation, there is a tax benefit that you can utilize if you wish to continue to own the employer’s publicly traded company stock. The tax benefit can occur when instead of rolling the stock in your 401(k) to an IRA Rollover, you transfer the shares to a taxable brokerage account (think your Joint account or Trust account) whereby you avoid ordinary income tax on the stock’s Net Unrealized Appreciation. The only part of the stock that is subject to ordinary income tax is the value or cost basis of the stock in your account as it was acquired by your 401(k) plan. So, if you bought and accumulated employer stock in your 401(k) over the past 30 years and the stock has climbed in value, you would only pay ordinary income tax on the price you paid for the stock. All of the position and the gain in the stock would then be held in your after-tax account and subject to the lower capital gains tax rate when you decide to sell—potentially an enormous tax savings! Of course, there are certain requirements that must be met, which include that the distribution of the employer stock must be in-kind; the distribution must be made in a lump sum, which means that the entire account balance of the employer retirement plan must be distributed in a single tax year whereby other assets not subject to the in-kind NUA distribution may be rolled over to an IRA; the lump-sum distribution must be made in the year of a triggering event, such as separation from employer, death, disability or when the employee reaches age 59.5; and an important caveat is that the NUA gain will not be eligible for a step-up in basis at death. The NUA strategy may be beneficial for those whose company stock has significantly appreciated in value, but it is

Every woman 18 and over should be familiar with their breast tissue and perform breast exams.  Those who are pregnant or lactating can still perform breast exams but

Feel for a lump or thickening, but just as important is looking for skin changes, nipple retraction or nipple discharge. WHAT’S THE BEST WAY TO PERFORM A BREAST SELF-EXAM? Breast self-exams should always be performed lying down. Using the opposite hand, use the vertical strip method by starting in the armpit and making small circles as you progress toward the nipple. This up-and-down approach seems to work best for most women. Be sure to feel all the tissue from the front to the back of your breasts: for the skin and tissue just beneath, use light pressure; use medium pressure for tissue in the middle of your breasts; use firm pressure for the deep tissue in the back. When you've reached the deep tissue, you should be able to feel down to your ribcage. WHAT’S NORMAL AND WHAT’S NOT? There is no “normal” or “standard” breast among women. Breasts differ in size, shape and density, and often one breast will be slightly different from its pair. Pregnancy, as well as monthly cycles, can change the size and tenderness of your breasts. If you feel a hard fixed irregular lump or experience a retracted nipple or clear or bloody discharge, contact your health care provider for further evaluation.
