Getting Started Eligibility
Enrolling In and Changing Benefits
You’re eligible to enroll in the benefits described in this guide if you’re a regular, full-time employee working at least 30 hours per week. Eligible employees working in the U.S. can also enroll their eligible dependents in the Manitowoc benefits described in this guide. Eligible dependents include your:
As an eligible employee, your first opportunity to enroll in benefits is when you join the Manitowoc team. After that, you can enroll or change your benefits during the annual enrollment period or if you experience a qualified family status event.
• Legal spouse (see page 15 for information about how enrolling a same-sex spouse could affect your income taxes)
day of the month after completing one month of continuous employment. You must complete your enrollment before your benefit start date. We encourage you to complete enrollment forms at least two weeks before the benefit start date so you’ll receive insurance cards in a timely manner. Elections will remain in effect until December 31 unless you have a qualified family status event and make a change.

If you’re covering a dependent who no longer meets the eligibility requirements listed above, remove him or her from your benefits coverage by completing a benefits enrollment form. This form is available from your local Human Resources representative and at My.Manitowoc.com.

If you’re adding a dependent to your coverage, you’ll need to provide proof of the dependent’s eligibility to your local Human Resources representative. Some of the types of information that are considered acceptable proof include:
− Change in spouse’s employment − Loss of coverage for yourself or your spouse − Dependent child turns age 26 and becomes ineligible for coverage
• Marriage certificate
To change your benefits due to a qualified family status event, you need to complete a benefits enrollment form and submit it to your local Human Resources representative within 31 days of the event. Any changes you make must be consistent with the qualified event; for example, if you get married, you can add your new spouse.
• Birth certificate • Adoption certificate or proof of placement for adoption Certain other documents may also be acceptable. Please contact your local Human Resources representative for information about the documents needed for your particular situation.

We’ve got benefits that will help you if you get sick, plus programs that help to keep you healthy and reduce the amount of care you may need. Your medical benefits are provided through Anthem Blue Cross and Blue Shield (Anthem BCBS). You get the best value when you use in-network providers. Anthem BCBS has one of the largest networks of doctors, hospitals, and health care facilities in the country. To locate a network provider, go to www.anthem.com or call (800) 599-8069. Preventive Care Preventive care screenings and services assess your overall health, identify potential health problems, and help diagnose untreated conditions. Best of all, the Manitowoc medical plan includes certain preventive care benefits at no cost (no copay, coinsurance, or deductible) when you visit an in-network provider. These benefits include services recommended by the U.S. Preventive Services Task Force (USPSTF). If you obtain these same preventive care services from an out-of-network provider, you’ll be charged a copay, coinsurance, and deductible. Prescription Drug Benefits The Manitowoc medical plan includes prescription drug coverage through CVS Caremark. CVS Caremark offers two easy ways to get your prescriptions filled. You can get up to a 34-day supply filled at a participating retail pharmacy. You can get up to a 90-day supply filled through the mail order (home delivery) program.
Retail Pharmacy vs. Mail Order Example Andrea and Kevin both take one daily dose of the same generic maintenance medication. However, instead of going to the pharmacy every 30 days, Andrea saves time and money by having her prescription drugs delivered to her home. Read their stories below to learn how you can save time and money on your prescription drugs.
Meet Andrea
Meet Kevin
To refill her prescription, Andrea goes online to www.caremark.com or calls (877) 779-7342. A few days later, Andrea receives a 90-day supply of her medication delivered to her doorstep.
Every 30 days, Kevin drives to his local pharmacy to get his prescription refilled. Once at the pharmacy, Kevin waits up to 45 minutes to have his prescription filled. He repeats this month after month.
With free shipping, Andrea pays $18 for a 90-day supply, or $72 per year.
Kevin pays $9 for a 30-day supply, or $108 per year. And this doesn’t account for the gas used and time spent driving to and from the pharmacy.
Over the year, Andrea spends $36 less than Kevin for the same prescription.

Mail Order: Fast Start
Your Cost for Prescription Drugs
If you have a refill or a new prescription that you want to convert to a mail order prescription, just call the CVS Caremark Customer Care number on your ID card and ask for Fast Start. You’ll be transferred to a department that will take your information and then call your doctor and get everything set up for you to receive your prescriptions by mail. Nothing else for you to do!
The amount you pay for prescription drugs depends on the drug type and where you have the prescription filled. In general, you get the best value when you use the CVS Caremark mail order program. In addition to saving money on your prescription, you save yourself a trip to the pharmacy.

Generic Step Therapy Program
• If you have not tried a generic drug for your condition, CVS Caremark will not approve coverage of the brand-name drug until you have tried the generic form.
Manitowoc’s prescription drug plan has a generic step therapy program that requires participants to try less expensive, generic drugs before the plan will pay for certain brand-name drugs. This program applies to the 15 classes of drugs listed in the table below. Here are important details about the generic step therapy program:
• You have the right to have a brand-name drug even if CVS Caremark doesn’t approve payment, but you’ll have to pay the full cost yourself. • Exceptions may be made for members who can’t tolerate generic drugs. (Note: Most generic drugs today were popular brand-name drugs at one time.)
• The generic step therapy program applies to both new prescriptions and refills. • If your doctor prescribes a brand-name drug listed below, CVS Caremark will fax your doctor a form to complete and fax back to CVS Caremark.
• If you have tried a generic drug for your condition recently, then CVS Caremark will authorize coverage of the brand-name drug, as needed.

Dental Care Benefits Good oral hygiene is important to your overall health and well-being. That’s why Manitowoc offers two dental plans from HumanaDental. With over 130,000 participating dentists, HumanaDental has one of the largest dental networks in the country. Finding a HumanaDental dentist is as easy as going to www.humanadental.com and selecting your state on a map, or by calling (800) 233-4013. Comparing Your Dental Plans Different families have different dental needs. Assuming that one plan is better based solely on price may be a mistake. Compare both plans and consider your needs before enrolling. 1. Dental Plan 1. This plan provides a $1,500 combined annual maximum benefit for your entire family. Preventive and
diagnostic services are covered over and above the $1,500 annual maximum if you use a preferred Humana dentist. So, for example, if you have a family of five and you see an in-network dentist, each member can have routine cleanings without the fees counting toward the $1,500 annual family maximum. 2. Dental Plan 2. This plan provides a separate $1,500 annual maximum for
each person or family member. Preventive and diagnostic care charges are included in the $1,500 annual maximum.

Situation: The Johnson family has three members. In 2013, each member visited the dentist for preventive care only. They don’t anticipate any major dental expenses in 2014, except for maybe a filling or two. Since the Johnson family expects to use mainly preventive care in 2014, it would be more cost-effective for them to enroll in Dental Plan 1, which has a lower cost per paycheck deduction.
Situation: The Martin family has six members. In 2013, their preventive/diagnostic visits resulted in two fillings, bridgework, and orthodontia. Total costs exceeded $1,500. Because they spend more on dental care, Dental Plan 2 would probably be the more cost-effective option for the Martin family.

Did You Know? More than 90% of diseases can show up in your mouth first. Your dentist may be the first medical person to find health problems or disease. Here are examples of how oral (mouth) health and overall health are related:

• Heart disease risk. Bacteria from your mouth may contribute to the blockage of arteries. Research has found a link between gum disease and heart disease, stroke, and other illnesses. • Diabetes risk. Diabetics are more likely to get mouth infections and gum disease. Oral infections can make it harder to control diabetes. People without diabetes are less likely to have these problems. • Lung problems. The lower respiratory tract (your bronchi and lungs) can draw in bacteria from the mouth and throat. Bacteria can cause infections and make lung problems worse.
So, maintaining good oral health helps your overall health!

Vision Care Benefits The Manitowoc medical plan with Anthem automatically covers routine eye exams for the cost of a $25 copay. The Anthem medical plan and Humana dental plan both offer vision discount plans that can save you money on the cost of glasses or contact lenses. No special enrollment is needed to participate in these discount programs. For more comprehensive vision coverage, Manitowoc also offers an enhanced vision care program through UnitedHealthcare. With this program, you can see any provider you choose, but you maximize your benefit when you visit an in-network provider. For a weekly premium, you receive the benefits shown on the next page, including a free in-network eye exam every 12 months. Keep your eyes open to good health. Even if you’re not having vision problems, you shouldn’t put off getting regular vision checkups. There are several good reasons for routine vision care: • Eye diseases can be detected early. Eye diseases, like glaucoma, don’t always show symptoms before they begin to cause damage. With a routine eye exam, diseases can be caught early enough so treatment can begin before vision is severely compromised or blindness occurs. • Other diseases or illnesses may be detected. Conditions such as diabetes and high blood pressure can be discovered during a routine eye exam. If an ophthalmologist detects a physical disease or illness that’s displaying symptoms through the eyes, a medical doctor can provide treatment options to improve your overall health.

Save for Today and Tomorrow Flexible Spending Accounts
Important Information
One of the best ways to reduce your out-of-pocket medical and dependent care expenses is to take advantage of Manitowoc’s flexible spending account (FSA) program. Through FSAs, you can be reimbursed for your deductibles, coinsurance, and certain health care and dependent care (day care) expenses with tax-free dollars. There are two types of FSAs:
• For a health care FSA, you can be reimbursed for health care expenses incurred up to two and a half months after the end of the plan year (through the middle of March of the following year). • For a dependent care FSA, expenses must be incurred prior to the end of the year.
• Health care FSA. This account allows you to be reimbursed
for your and your dependents’ out-of-pocket health care expenses such as deductibles, copays, and services not covered by your benefit plan. This applies to medical, prescription drug, dental, and vision expenses. A complete list of eligible expenses appears in Publication 502 on the IRS website at www.irs.gov. For 2014, the maximum annual contribution you can make to your health care flexible spending account is $2,500. The minimum annual contribution is $130.
• You cannot use the money in your health care flexible spending account to pay for dependent care (day care) expenses and vice versa. • Your out-of-pocket medical, prescription drug, dental, and vision costs from your Manitowoc plans will automatically be reimbursed from your health care flexible spending account. No need to file a claim form! • Over-the-counter (OTC) medicines and drugs can be reimbursed from a health care flexible spending account when they are prescribed by a physician. For more information, see the Flexible Spending Accounts Guide on My.Manitowoc.com.
• Dependent care FSA. This account can be used to pay
for dependent care expenses, such as adult or child day care, summer day camps, or in-home care for an adult or child. A complete list of eligible expenses appears in Publication 503 on the IRS website at www.irs.gov. The maximum annual contribution to your dependent care flexible spending account is $5,000. The minimum annual contribution is $130.
401(k) Plan Manitowoc helps you save for retirement by giving you the opportunity to contribute a percentage of your pay into the 401(k) Plan. To encourage you to save even more, Manitowoc matches a portion of your contributions. Plus, when we reach certain financial goals, Manitowoc may make an additional annual contribution to your account. Manitowoc’s 401(k) Plan is administered by BMO Retirement Services (formerly M&I). Some employees may receive a better tax benefit by taking the child care expense tax credit on their income tax returns. Please compare the two programs to determine which is best for you. Journeys can be accessed over the Web or via iPhone or Android smartphone. Many people find them to be a refreshing, re-energizing alternative to more traditional online wellness programs. Select a RedBrick Journey focused on a health topic of your choosing and you’ll be presented with bite-sized, fun steps tailored to your interests. Select the steps you’d like to commit to as part of your Journey. Steps may incorporate activities as well as brief videos. Continue to personalize your experience by giving feedback on the steps you like and the ones you don’t. If you’re looking for additional encouragement, you can request live support by phone. Work on multiple Journeys at once or complete one at a time. By moving at your own pace, you can make progress in as little as a minute a day, completing a Journey in a few days to a few weeks. It’s all up to you. Sample Journeys include Get Strong at Home, Move It and Lose It, Smart Snacking, Dine Out: Take Charge, and Stress to Energy Sampler. Phone Coaching RedBrick health coaches are certified experts who will work with you by phone to help answer your health questions, provide support in overcoming obstacles, and help set small goals to work on between sessions. Choose from topics including managing blood pressure, cholesterol, diabetes, stress, and more. RedBrick Tobacco-Cessation Program To help you make the lifesaving choice to give up tobacco and stay tobacco-free, RedBrick Health offers a proven tobaccocessation program to Manitowoc employees and your covered dependents over age 18. This program gives you the choice of working with a health coach by phone and online. Dependent Life Insurance You can also purchase coverage for your eligible spouse and dependent children, as long as you buy supplemental coverage for yourself.
Salary Continuation Benefits and Disability Insurance If you become disabled and are unable to work, you can receive benefits equal to 100% of your salary for up to 26 weeks through the salary continuation plan. For added protection if you continue to be disabled, you also receive long-term disability benefits (equal to 60% of your base salary) after your salary continuation plan benefits end. Disability benefits are administered through Matrix Absence Management. For details about your disability benefit amount and eligibility, contact your local Human Resources representative.
For your spouse, you can choose coverage amounts in increments of $1,000 up to $200,000, but not more than 100% of the supplemental coverage you elect for yourself. If, as a new employee, you elect spouse coverage that is more than $50,000, your spouse will need to provide proof of good health for the amount over $50,000. The Reliance Standard Life Insurance Company must approve spouse coverage for the portion over $50,000. Important Note: If you elect an amount for your spouse that
exceeds $50,000, your spouse will need to complete a Personal Health Statement. You can obtain a Personal Health Statement from your local Human Resources representative.
Employee Life and Accidental Death and Dismemberment Insurance
You can purchase coverage for your dependent children between the ages of 2 weeks and 26 years. You can choose an amount between $2,000 and $20,000 in increments of $1,000. You do not need to provide proof of good health for your children.
Manitowoc provides you with basic life insurance in the amount of one times your base annual salary through Reliance Standard. You also receive accidental death and dismemberment (AD&D) coverage equal to one times your base annual salary. This means that if you die as a result of a sudden accident, your beneficiary will receive a total benefit of two times your base salary from your basic life and AD&D coverage. AD&D benefits may also be payable to you if you suffer certain severe injuries due to an accident.

life coverage is subject to automatic reduction if the insured person is age 65 or over. Reductions are based on the schedule below:
Supplemental Employee Life Insurance You can purchase additional employee life insurance coverage in $1,000 increments, up to a maximum coverage amount of the lesser of $1 million or five times your basic annual earnings. If, as a new employee, you elect an amount that is more than the guaranteed issue amount of $200,000, you’ll need to provide proof of good health (also known as evidence of insurability). The coverage amount over $200,000 will not take effect until approved by the Reliance Standard Life Insurance Company.

Paying for Coverage Unless otherwise stated, you and Manitowoc share the cost of each benefit described in this guide. If you enroll, your cost is deducted from your pay each pay period. Deductions for your medical, dental, and vision coverage and your flexible spending accounts are made on a before-tax basis. This saves you money by reducing the amount of taxes you pay. Your contributions for supplemental life insurance are made on an after-tax basis. The rates are shown below. The Manitowoc medical, dental, and vision plans offer five levels of coverage, so you can choose the coverage level that best fits your family. If you and your spouse are both employed by Manitowoc, you can each enroll in Employee Only coverage, or one of you can enroll in one of the Employee plus dependent coverage levels and list your spouse and/or children as dependents. However, you and your spouse cannot both enroll in one of the Employee plus dependent coverage levels. Note: Manitowoc recognizes a same-sex marriage if it’s legally performed in a state that allows it. The value of coverage for a same-sex spouse is not subject to federal income tax nor state income tax if you and your same-sex spouse live in a state where same-sex marriage is legal. However, the value of same-sex spouse coverage is subject to state income tax if you live in a state that doesn’t recognize same-sex marriage.
