Sickle cell disease is a group of disorders that affects hemoglobin, the molecule in red blood cells that delivers oxygen to cells throughout the body. The signs and symptoms of sickle cell disease are caused by the sickling of red blood cells. When red blood cells sickle, they break down prematurely, which can lead to anemia. Anemia can cause shortness of breath, fatigue, and delayed growth and development in children.
The rapid breakdown of red blood cells may also cause yellowing of the eyes and skin, which are signs of jaundice. Painful episodes can occur when sickled red blood cells, which are stiff and inflexible, get stuck in small blood vessels. These episodes deprive tissues and organs of oxygen-rich blood and can lead to organ damage, especially in the lungs, kidneys, spleen, and brain. A particularly serious complication of sickle cell disease is high blood pressure in the blood vessels that supply the lungs (pulmonary hypertension).
Pulmonary hypertension occurs in about onethird of adults with sickle cell disease and can lead to heart failure.
Finally, Researchers Have Found A Way To Relieve The Pain Associated With Sickle Cell Disease Researchers at Duke University and Howard Hughes Medical Institute discovered one of the keys to the cause of pain in Sickle Cell Anemia, also known as Mediterranean Blood Disease. Their findings were reported in the Proceedings of the National Academy of Sciences, Jan. 31, 2005. Here’s what they found. Picture red blood cells floating in blood in the arteries of someone who does not have sickle cell anemia. Their blood pulses with each beat of the heart. As the pulse of blood moves through the arteries, they expand to allow the blood to pass. This happens because the red blood cells produce and release a signaling molecule to tell
arterial walls to expand. The signaling molecule they make is Nitric Oxide. Now picture the red blood cells of someone with sickle cell anemia. Many of them are distorted into a sickle shape, so big that some get stuck going down small capillaries. But even worse, when the blood pulses, researchers at Duke discovered that the walls of the arteries don’t expand. Since they don’t expand circulation is restricted and oxygen starvation occurs in tissues downstream, causing the pain and other symptoms of sickle cell anemia.
In normal blood, when red cells enter a narrow passage, they sense pressure on their membranes, and respond by releasing nitric oxide. This nitric oxide causes the smooth muscle in the blood vessel wall to relax. This opens up the vessel to allow the cells to pass through.

So What Causes Sickle Cell Pain? In sickle cell anemia patients, the red blood cells can’t open up blood vessels as they should. The reason is because there isn’t enough nitric oxide present in the cells to do the job. Inadequate amounts of nitric oxide is directly correlated with the severity of pain. The Duke/HHMI researchers were able to confirm this with their studies.
The less nitric oxide your blood cells can produce, the greater the pain. And to make matters worse, with severe sickle cell, when oxygen levels start to drop, blood vessels naturally constrict or get smaller. This just complicates the problem even more. So what does all this mean?
The Duke/HHMI researchers concluded that this abnormal nitric oxide processing might be the real cause of restrictions in the circulatory system of sickle cell patients. And when it comes to the severity of sickle cell symptoms, the sickle shape of the red blood cells may be less important than oxygen depletion.
