IPRO improves the quality and Partnering with healthcare providers to improve health outcomes IPRO supports healthcare quality improvement by working with healthcare providers across all settings, including hospitals, nursing homes, hospice programs, dialysis centers, physician practices and more. Coordinating Care for Chronic Care Patients IPRO is supporting a community-based health information technology (HIT) project that will streamline and improve the quality of care for more than 30,000 adults with diabetes through the patientcentered medical home model. The medical home model emphasizes centralized care delivery for individuals with chronic disease at risk of receiving fragmented and uncoordinated services.

Improving Healthcare Ensuring that healthcare is medically necessary and appropriate IPRO evaluates patient care from perspectives of medical necessity, quality of care, appropriateness of decisionmaking and length of hospital stay, as a safeguard against unnecessary and inappropriate medical care.
IPRO audits Medicaid providers in eight states, Puerto Rico and the U.S. Virgin Islands to ensure that Medicaid payments are for covered services that were actually provided, properly billed and documented.
IPRO annually conducts more than 110,000 reviews to assess the medical necessity and appropriateness of acute hospital care for the New York State Medicaid program.

Supporting Patients with End-Stage Renal Disease
As the QIO for New York State, IPRO works with providers on clinical topics, as directed by the Centers for Medicare & Medicaid Services, to implement best practices and improve patient outcomes. Current projects include:
IPRO works with New York State consumers, End-Stage Renal Disease (ESRD) network facilities and other providers to improve the quality of care and quality of life for patients with end-stage renal disease.
Improving care continuity for Medicare patients after they are discharged from the hospital, with a goal of reducing avoidable re-hospitalizations. Partnering with physician practices across New York State to help implement processes that improve care management and increase the appropriate use of preventive services such as mammograms, colorectal screening, and immunizations. Reviewing quality of care complaints lodged by Medicare patients and their families, and where appropriate, working with healthcare providers to conduct system-wide quality improvement activities.
Building Collaborative Learning Networks IPRO is working with hospitals and quality improvement organizations across the country to support dissemination of Agency for Healthcare Research and Quality tools and resources, which have been shown to improve the quality and safety of healthcare delivery.
Optimizing Physician Use of Electronic Health Records IPRO serves as an implementation agent for the NY e-Health Collaborative, a federally recognized Health Information Technology Regional Extension Center, charged with supporting and accelerating healthcare providers’ efforts to become meaningful users of electronic health records.
for the Common Good Early Intervention Monitoring On behalf of the New York State Department of Health, IPRO monitors quality and appropriateness of early intervention services provided to developmentally disabled children under age three. This program helps to ensure that eligible infants and toddlers and their families have access to timely, consistent and appropriate services.
Collecting, analyzing and validating data to measure healthcare performance Testing Patient-Level Hospice Performance Measures IPRO is partnering with eight New York-based, Medicare-certified hospice programs to promote timely referrals to hospice care, support the integration of hospice into patients’ overall care plan and test patient-level performance measures. IPRO is conducting statistical analyses of these measures and facilitating focus groups of family members, hospice staff members, hospice referral sources and Medicare beneficiaries to evaluate the usefulness of the measures to each group.

and value of healthcare services by: Reviewing Data to Ensure Quality Care for Patients with HIV/AIDS IPRO manages the activities of the New York State AIDS Intervention Management System, with the goal of improving care delivered to patients with HIV/AIDS across the state. IPRO conducts approximately 75,000 ambulatory and acute-care medical record reviews annually, delivered at more than 150 facilities.
Analyzing Medical Errors As the review agent for the New York State Patient Safety Tracking System, IPRO validates more than 30,000 reported preventable adverse medical events each year, with the goal of identifying underlying problems and improving patient safety in hospitals.
