This patient could not open up so big and had a bit of bleeding (and you all know how much I hate the site of blood!). Fractured #12 was obviously non-restorable. Due to time and working the emergency into my schedule, I did not do a CEREC Guide.

I removed the tooth with luxators. That’s another ditched prototype of some blue forceps. They were too big! I used a Verban stop to help with centering my osteotomy.

Sorry for the theme. I had a spare RePlant lying around so I used that 4.3mmD by 13mmL. Tight space! Again, you can see the 2D limitations of the X-ray as I was really not that close.

Fairly large gap distance on the buccal (greater than 2mm) so we grafted and just packed in a half moon shaped membrane between the socket and the abutment. Stock RePlant abutment and grossly under contoured temp.

I only have the TSV scan posts, so I took an impression and had my lab guy Jack from Level 3 do this case in InLab. Check out how incredibly white the MO 0 block is!! It’s so white it’s not even purple in the unsintered state!

Jack's final crown and abutment! Since I was so under contoured with my temp we have some pull of the tissue on the canine. I will see how that fills in, if not, I may have to take the crown off #11 and re-prep. Which would help the esthetics overall as the tooth looks a little squatty.

Dr August de Oliveira graduated from dental school in 1997 from the University of Washington and completed his General Practice Residency in Los Angeles in 1998. Dr de Oliveira has been lecturing on 3D technology since 2004, when he started as a CEREC Basic trainer. Since 2008 he has been involved with Implant Direct’s R and D department developing Guided Surgery Software and Hardware and testing their CAD Milled Bars and Substructure Department. Dr de Oliveira has written two books on Implantology: Implants Made Easy and Guided Implantology Made Easy. He has been involved with beta testing Sirona’s Sidexis Program, as well as developing the Opti and CEREC Milled Surgical Guides. Dr de Oliveira lectures nationwide for Sirona on the Galileos Cone Beam system and Sirona Guided Implant Surgery. He also teaches for Implant Direct at their Las Vegas Educational Facility and with the Engel Institute in Charlotte NC. Dr de Oliveira lives and practices in Los Angeles CA.
