November is Hospice and Palliative Care Month. Hospice is care providing support to patients facing terminal illness while palliative care focuses on relieving symptoms of serious illness, whether curable or not. Comfort care homes are two-bed, independently-operated hospice homes, and Rochester is credited as having the first such homes in the US. What experience have you had with comfort care homes?

Avoid gifts that are overly personal. Steer clear of giving fragrances or cosmetic products because you do not know what the other person will like. Scented products also may send the wrong message, indicating you think someone needs to cover up with an aroma. Clothing is another gift to avoid, as certain items may be seen as suggestive and you may not be in tune with the recipient’s personal style.
Fruit & Nut Dressing (FamilyFeatures)
Inlargeskillet,sautéonion,celery with 1/2 cup butter and 1 cup chicken broth. Combine vegetables with 3/4 cup apple juice, 2 cups peeled and chopped
Granny Smith apples, 1 cup walnuts and 1 box Seasoned Dressing. Transfer to casserole dish, cover and bake 30 to 40 minutes. If crisp top is desired, bake additional 10 minutes uncovered. You can also try adding dates, dried cranberries andothernutslikepistachiosand sunflowerseeds.









TurkeyMacSoup (Family Features)
Ingredients
1 box Elbows
1 tbls. canola or vegetable oil
1 cup chopped onion or leek
1 cup chopped carrots
1 cup sliced celery
1 cup chopped bell pepper
2 cups broccoli slaw
2 teaspoons poultry seasoning
6 cups reduced-sodium, fat free chicken broth
3 cups chopped or shredded cooked turkey
Salt & Black pepper
Preparation (20 min.)
1. Cook pasta according to package directions. Drain and return to pan, reserving one cup pasta water.
2. Meanwhile, heat oil in Dutch oven over medium-high heat until hot. Add onion, carrots, celery and bell pepper; cook 2 minutes, stirring occasionally. Add broccoli slaw; continue cooking 1 minute or until vegetables are crisp-tender, stirring frequently. Stir in poultry seasoning. Cook and stir to blend into vegetables.
3. Add chicken broth; bring to boil. Reduce heat; simmer 5 minutes. Stir in turkey; heat through. Add pasta water if more liquid is desired.
4. Divide pasta among 8 bowls. Ladle soup over pasta. Season with salt and pepper, as desired.
Makes 8 servings Cook Time: 10 minutes.

What’s the first step in sound proofing your home? Finding which rooms have the least amount of sound resistance. The Government of Canada notes that areas of western Canada are already experiencing frequent and severe droughts, and scientists expect other areas of the country to be affected by drought more often in the years to come. Tipping men and women who work in the food service industry is commonplace in the United States and Canada, where citizens typically tip between 15 and 20 percent when dining out at restaurants. But American and Canadiantravelersabouttodepart for overseas destinations may want to familiarize themselves with the practices regarding gratuities before their planes touch down. Some countries automatically add gratuities onto customers’ bills. For example, the government in Dubai mandates that a 10 percent service charge be added to all hotel, restaurant and bar bills. This service charge is then divided by the staff at the establishment, making the charge the equivalent of a tip. Diners in Morocco, where the gratuity is sometimes included in the bill, might be surprised to learn that a 10 percent tip in that country is considered generous. Tips are included in the bill in Costa Rica, where travelers need not feel obligated to leave any additional gratuities. Service is often included in the bill within the United Kingdom as well, though travelers should inspect the bill before assuming gratuity was included. Visitors to Spain should not feel obligated to leave a gratuity larger than 13 percent, and even a tip in the range of 7 percentisconsideredacceptable.

If you’ve ever felt the pull to talk about death, you just might enjoy attending a Death Cafe. ese casual events bring together community members – o en strangers – to have relaxed conversations about an o entaboo topic. Death Cafes are part of an international movement, but there’s only a handful of these events in western New York. Most of them are run by Jennifer Sanfilippo, who started hosting a Death Cafes series at Pittsford Library in January this year. She has hosted other Death Cafes in the region since 2023, including ones in Parma, Penfield, and Irondequoit.
At a Death Cafes event, a small group comes together to share whatever they’d like about death and dying. “It is simply a dialogue where people come to speak about anythingthatisontheirmindorin theirheartsaboutdeathanddying,”

Sanfilippo says. “ ere is no influence toward anything or attempt to reach any kind of conclusion.” She says the events are more upli ing than you’d expect. “It really is very special and energizing. ey’re not depressing, but rather very thought-provoking conversations.”
Sanfilippo says Death Cafes were created because their founder saw a need for a purposeful space to talk about death within our culture. In past generations, death was more present in the home, with families more closely involved with the care and burial of their deceased relatives. en, Sanfilippo says, “we compartmentalized that part of our lifestyle. Death is natural, but it’s become taboo and spooky.”
Death Cafes help people move beyond the stigma they might feel around death and talk about any thoughts,
feelings, or questions they have. While people can and do share their own experiences with grief and loss, it’s not a bereavement support group. Rather, it’s a way to engage thoughtfully about death with people in your local community. Sanfilippo says, “I’m not sure how many places you can go now and sit and talk to someone you don’t know. It’s remarkable to see what people will share, and how liberating and relieving it is to say those things out loud.”
Conversations at Sanfilippo’s Death Cafes are incredibly varied, and she says, “We talk about everything. Swedish death cleaning to psychic mediums to what happens a er you die, fears and hopes and how people want to live their lives.” People are welcome to talk about their own belief systems and cultural traditions, but there’s never pressure or judgment to adhere to any specific one. Instead, people can find inspiration in these conversations for their own end-of-life plans. Sanfilippo also says the Death Cafes can serve as a place to practice the conversations you might want to have with loved ones. She says, “What I’m trying to do is demystify aspects of death and dying so people can build up resilience for these difficult conversations.”

Sanfilippo began hosting Death Cafes in the years following her husband’s death from a rare form of leukemia. A er he passed away, she began processing her grief by writing a book about their story, Our Last Walk Home: Love, Cancer, and the Agony of Letting Go Working on the book led her to becoming certified as an end-of-life doula and volunteering in hospice settings, and it was during this time Sanfilippo realized this was the work she needs to do. Before her husband’s cancer diagnosis, she had been working on opening her management consultancy a er a long career working as a consultant and strategist for politicians and financial institutions. Sanfilippo says that when she worked with the dying, “Others were afraid to be with them. But I wasn’t. It was the place for me to be.”
Today, Sanfilippo is finishing her master’s degree in counseling from Southern New Hampshire University, which will help her expand the support she can offer in individual and community settings. She’s writing another book, and she still volunteers in hospice homes. She says, “I think it’s really important to stay in

Wow, I hope that person knows how much I appreciated this thoughtfulness and generosity. It was a moment I will not soon forget. What is going on lately with so very many lost cats and dogs? It seems like every day there are notices of lost or missing cats and dogs. I’m sad for the animals and owners who are heartbroken. The other side of that is the sad amount of cats found wandering in back yards, mewing at people’s door steps, and lost dogs wandering on roads at all hours. There is info online to help pet owners understand about securing their homes, windows, and yards and providing an excellent safe pet home. Learning about a cat, dog, or any pet, is essential and understanding that by saying ‘Don’t go out’ or ‘No’ doesn’t do it. Sometimes loud noises, strangers, poorly socialized pets, getting ‘stuck’ someplace, or being just plain fearful can make an animal look for safety and escape, not knowing of course what the ‘big world’ out there is. While humane societies offer so many enticing adoptions, people should not get a pet ‘on a whim’ but rather study pet care. Let’s all keep our pets safe and happy at home.

After stopping at a stop sign when turning left, if you find yourself in the oncoming lane of traffic, you’re doing it wrong. Speed limits. If you can’t at least do the posted speed limit, please pull over and let the forever growing line of traffic go by you. We all make mistakes. If you pull out in front of someone, get going or pull over. Common courtesy needs to make a comeback!

DEAR DR. ROACH: DEAR DR. ROACH: In a recent column, you wrote about reducing cholesterol levels with the help of statins along with a PCSK9i, such as Repatha or Praluent. My question is: Are these PCSK9is at all beneficial if one cannot take statins? -- D.C.S.
ANSWER: Yes, PCSK9 inhibitors are very effective at lowering cholesterol and reducing the risk of heart attacks in high-risk people. Because these drugs are given by injection and are quite expensive, they are not usually the firstline treatments. We have decades of experience using statins, so they remain the first choice for most.
However, if a person cannot take a statin, or one of the alternatives such as bempedoic acid, then PCSK9i drugs are a very reasonable choice. In people with familial high cholesterol and extraordinarily high levels of cholesterol, combination therapy with two or three drugs (a statin, a PCSK9i and sometimes ezetimibe) is frequently used.
DEAR DR. ROACH: My wife had COVID and has lost most of her smell and taste. This has lasted for two years. She is in her late 70s. Strong flavors like barbecue sauce and onions can be detected. Is there any hope that her condition will improve? -- P.M.
ANSWER: Most people recover their loss of smell after COVID. Having had this for two years is a bad sign; however, the fact that she does have some sense of smell is a good sign.
The most effective treatment for the loss of smell after a viral infection is olfactory training. The same scents that were used in clinical trials can be purchased online. Recent evidence has shown that a supplement combination, ultramicronized palmitoylethanolamide and luteolin, along with olfactory training, led to greater improvement. The dose used in studies was 770 mg.
DEAR DR. ROACH: My adult daughter (62) was diagnosed about three years ago with a skin disorder called prurigo nodularis. Her dermatologist has tried three different injections, but they’re not working. She still gets very itchy breakouts. Please give me your thoughts and more information about this. -- K.M.A.
ANSWER: Prurigo nodularis is an uncommon disease that affects about one person per 1,000. I have seen a handful of cases, and fortunately the cases I’ve seen have responded well to potent steroid creams.
Regular use of skin lotions, avoiding scratching especially at night (I recommend cotton gloves), and sometimes antihistamines like Benadryl are also the first-line therapies.
If a patient needs more effective treatment and only has a limited area of itching, then a dermatologist will often inject the area with a high-potency steroid. Since it sounds like this hasn’t been successful for your daughter, there are other options. One is a topical cream called ruxolitinib. About 45% of people responded to this treatment, compared to 20% who responded to a cream without an active drug. Ultraviolet light (narrowband ultraviolet B) showed about 80% effectiveness. For people in whom none of these therapies work, a very expensive injection treatment,dupilumab (Dupixent) or a similar drug called nemolizumab (Nemluvio), can be used, although the response rate is not as good as desired (roughly 40%).
Since many people with prurigo nodularis have other skin diseases, and some may have (or develop) depression or obsessive skin-picking, these conditions may also need to be treated to successfully improve symptoms

DEAR PAW’S CORNER: My 12-year-old cat, “Gaston,” may be losing his vision. He used to jump on high objects easily; now he avoids jumping up or down. He loved chasing his laser light; now he just huddles down and seems to ignore it. At night, he meows loudly until I let him into my bedroom, where he insists on curling up with me. I have an appointment with the veterinarian for next week, but can his vision loss be reversed? -- Jeanne in Evanston, Illinois
DEAR JEANNE: I’m very glad that you’re taking Gaston to see the vet. With suspected vision loss, it’s very important to get your cat tested to determine how severe it is and the underlying cause.
Blindness in cats can be gradual or sudden, and there are many conditions that can cause it. Age, infection and common eye conditions like glaucoma, cataracts or retinal detachment are all in play here.
Can Gaston’s vision loss be reversed? That really depends on what the vet finds. I can say that the sooner a problem is diagnosed, the better the chances of treatment to reverse or at least halt vision loss.
Cats whose blindness is progressive or irreversible can adapt. Those who develop it gradually tend to adapt better, while those whose vision loss is sudden may experience more anxiety and be less prone to adapt.
You can support your vision-impaired cat by keeping them indoors (if they must go out, create a fenced-in area of your yard for them). Avoid moving furniture too often. And don’t coddle your cat by carrying them around. Let them explore and adapt -- their resilience will surprise you.

The news is in,a week late this year due to the current government shutdown: The expected increase in Social Security benefits will be 2.8% for 2026.
For the average monthly benefit of $2,015, that equates to an increase of $56, up to $2,071 for 2026 for a single person. For a couple, the $88 increase will net a $3,028 monthly benefit for 2026.




Nutritional benefits of dairy: Dairy products are nutrient-dense and great sources of protein,calcium and vitamins.Protein builds and repairs muscle tissue; carbohydrates in milk provide energy; and calcium and phosphorous can help strengthen bones and teeth. Vitamin D helps promote the absorption of calcium. Dairy also contains riboflavin, vitamin A, pantothenic acid, potassium and niacin. One eight-ounce serving of milk contains the recommended daily values of these nutrients based on guidelines from the U.S. Food & Drug Administration.
The actual Social Security increase will be less, however, because, as is typical, the Part B cost has gone up. A $21.50 increase will bring the monthly cost up to $206.50, and that comes out of the Social Security benefit.
The COLA (cost of living adjustment) is calculated every year by the price index from third quarter 2024 to third quarter 2025. Once again they’ve used the Consumer Price Index for Urban Wage Earners and Clerical Workers (CPI-W), which many think is the wrong index. (How many of us are clerical workers?) To be more accurate, the Social Security increase should be calculated by the Consumer Price Index for Elderly People (CPI-E), as that has the costs and expenses more in line with where seniors spend money.
Meanwhile, Medicare open enrollment will be in effect until Dec. 7.You still have time to look over your options and see if you need to make some changes. For example, are you taking different drugs? Visiting your doctor more now? Then there is your current plan -- has the cost changed or any of the benefits you get?
