November is Hospice and Palliative Care Month. Hospice is care providing support to patients facing terminal illness while palliative care focuses on relieving symptoms of serious illness, whether curable or not. Comfort care homes are two-bed, independently-operated hospice homes, and Rochester is credited as having the first such homes in the US. What experience have you had with comfort care homes?

- I’ve volunteered at a comfort care home

- A family member or friend received care at one

- I’ve donated to or participated in a fundraiser for a comfort care home

- I’ve visited a comfort care home, but haven’t been directly involved

- I’ve heard of comfort care homes, but don’t know much about them

- I’m not familiar with comfort care homes at all

HOUSEBREAKING PUPPIES

Macks

Macks is an 8-year-old beagle/lab mix. Macks loves to go on outdoor adventures, especially kayaking on Keuka Lake.

Arguably one of the most challenging - and often frustrating - components about puppyhood is training the dog to use the outdoors for bathroom needs. It’s easy to underestimate the time involved in housebreaking a puppy. The Humane Society of America says puppies typically can control their bladders for one hour for every month of age. That means if the dog is three months old, he can hold it for about three hours.

Help Reduce Animal Overpopulation

One of the easiest ways to prevent pet overpopulation is to spay and neuter animals. Cats can reproduce at very fast rates. According to the Cat Rescue, Adoption & Foster Team of Central Oregon, two uncontrolled breeding cats can create the following situation if they have two litters a year at a survival rate of 2.8 kittens per litter: 12 cats in the first year, 66 cats in the second year, and 2,201 cats in the third year. Cats reach puberty between 4 and 12 months of age. Female cats reproduce between January and September, and might come back into “heat” every 14 to 21 days until they have bred or daylight decreases considerably. Cats can give birth 60 days after they have bred.

PET OBESITY IS A GROWING PROBLEM

Vets advise that dog owners should be able to easily feel and count dogs’ ribs when they run their fingers across the side of the animal. Pets viewed from above should display an hourglass figure that includes an indentation near the midsection. The absence of this “waist” may indicate a problem. When observing a pet from the side while he or she is standing, a slight upward slope of the stomach should be visible. A low-hanging abdomen means abdominal fat may be present.

Is my cat going blind?

DEAR PAW’S CORNER: My 12-year-old cat, “Gaston,” may be losing his vision. He used to jump on high objects easily; now he avoids jumping up or down. He loved chasing his laser light; now he just huddles down and seems to ignore it. At night, he meows loudly until I let him into my bedroom, where he insists on curling up with me. I have an appointment with the veterinarian for next week, but can his vision loss be reversed? -- Jeanne in Evanston, Illinois

DEAR JEANNE: I’m very glad that you’re taking Gaston to see the vet. With suspected vision loss, it’s very important to get your cat tested to determine how severe it is and the underlying cause.

Blindness in cats can be gradual or sudden, and there are many conditions that can cause it. Age, infection and common eye conditions like glaucoma, cataracts or retinal detachment are all in play here.

Can Gaston’s vision loss be reversed? That really depends on what the vet finds. I can say that the sooner a problem is diagnosed, the better the chances of treatment to reverse or at least halt vision loss.

Cats whose blindness is progressive or irreversible can adapt. Those who develop it gradually tend to adapt better, while those whose vision loss is sudden may experience more anxiety and be less prone to adapt.

You can support your vision-impaired cat by keeping them indoors (if they must go out, create a fenced-in area of your yard for them). Avoid moving furniture too often. And don’t coddle your cat by carrying them around. Let them explore and adapt -- their resilience will surprise you.

MAKE THE HOLIDAY SEASON MORE MERRY Sharefavoritememoriesatfamilyholidaydinners.

Be especially careful with power tools. Power tools can do a lot and they’re fun to use. But it’s important that DIYers do not let fun distract them from safety. The Power Tool Institute urges power tool users to wear personal protective equipment, including eye and hearing protection and a dust mask, whenever using power tools. The PTI also advises users to make sure tools are powered down before plugging them in and to inspect the tool before using it. Inspections can confirm that all guards are in place and that tools are up to the task at hand. Much like it’s unsafe to overreach on a ladder, overreaching when using power tools increase the risk for mistakes, accidents and injuries.

PROTECT YOUR HOME FROM WINTER’S FREEZE

Plumbing

Plumbing and pipes may be vulnerable to cold weather. Frozen pipes may burst and cause substantial damage to a home, potentially causing flooding and structural damage.

Homeowners should disconnect and drain garden hoses before winter arrives. Water to outdoor hose bibs should be turned off, though the valves on these outdoor faucets should be left open to drain. Also, outdoor faucets can be covered with insulating foam covers.

The Red Cross says pipes that freeze most frequently include pipes in unheated areas, such as basements, attics, garages, and crawl spaces. Close vents to the outside in areas like attics and basements to limit the amount of cold air that gets indoors. Think about insulating unheated areas, as well as using pipe sleeves, heat tape or wraps on exposed pipes. By opening kitchen and bathroom cabinet doors, homeowners can allow warm air from a home to reach pipes under the sink. During extreme freezes, keep cold water dripping from a sink to prevent pipes from freezing.

A Sweet Way to Support Literacy in Wayne County

At2pmonSaturday,November15th,theMacedonPublic Library hosts its second annual Pie Palooza Literacy Fundraiser. is lively, family-friendly event blends elements of musical chairs with a classic cake walk, giving attendees the chance to win delicious pies in time for anksgiving. e event promises an hour of fun and community spirit, all in support of literacy programs across Wayne County.

mentioned attending a cake walk elsewhere, the concept of a pie-themed version was born – something festive, community-oriented, and helpful for busy anksgiving hosts.

Library Director Stacey Wicksall says the idea for Pie Palooza came during a winter cra ing session. A er hosting pumpkin walks in the past, the library sought a ew seasonal event. She says, “ anksgiving o en flies under people’s radar, especially with Halloween and Christmas so close,” so when a patron

Proceeds from Pie Palooza benefit Literacy Volunteers of Wayne County, supporting regional literacy efforts, as well as the Friends of the Macedon Public Library, whose fundraising helps power a wide range of programs, fromsummerreadingandconcerts to art workshops, historical talks, and respite gatherings like Seniors Helping Seniors. Wicksall emphasizes, “We wouldn’t be able to do all that we do without their generous sponsorship.”

With nearly two decades in library service, Wicksall deeply values Macedon’s spirit. “Macedon is a small town with a big heart, tightly knit connections, and a cozy library dedicated to people and ideas,” she says. “As a librarian, the most important part of my job is to listen and connect people and ideas at just the right time.”

Literacy Volunteers of Wayne County: Building a CommunityofLifelongLearners

For Heidi Barnes, director of Literacy Volunteers of Wayne County, literacy means far more than reading books. It’s the key to navigating everyday life. Growing up in Wayne County, Barnes has lived here most of her life. Her family instilled in her “a huge love of reading and reinforced the value of education. is isn’t something everyonehastheprivilegeofreceivingintheirchildhood, so I try very hard to pay it forward into my community in both my volunteer work and my career choices.”

At Literacy Volunteers, that mission translates into practical help for people of all ages. “Literacy isn’t just about reading a book,” Barnes explains. “It’s about being able to balance your bank account and manage your money, it’s about being able to use a computer appropriately, it’s about navigating the health care system knowledgeably.”

Share your original stories about local heroes, community events, school achievements, or hidden gems–email your story idea to hometown@gvpennysaver.com!

e need is great, and she says, “In Wayne County alone, one person in eight reports trouble reading on the job and in their day-to-day life. One in ten won’t finish high school this year,” she said. “Every single one of those people will struggle with their ability to get and stay employed, manage their money properly, and help their families with their educational needs. at impacts every one of us.”

Barnes wants more people to know that helping doesn’t require special credentials, but instead anyone with a high school diploma who can fluently speak and read the English language is “more than qualified to really change someone’s life” with Literacy Volunteers.

Community partnerships play a big role in the group’s success. Events like Macedon Library’s Pie-Palooza highlight Literacy Volunteers’ collaboration with local libraries. “I think maintaining a strong relationship with our local libraries is critical to creating lifelong learners and good community members!” Barnes said. “Where else in our communities can you get books for free, ask experts for advice, use a computer to visit your doctor, research your family tree, and attend countless free community education classes?”

Anyone interested in donating their time or to a specific program can visit lvwayne.org. Potential volunteers can attend an online information session on ursday, December 4th at 6:00 p.m. Registration is required by emailing heidi.barnes@lvwayne.org.

For Barnes, every effort, large or small, helps strengthen the community. rough Literacy Volunteers of Wayne County, she’s helping ensure that the gi of learning is shared with all.

HOW TO DETERMINE IF IT’S TIME TO DOWNSIZE

Space: Many people love the extra space that single-family homes provide. But seniors can take a walk through their homes and see how many rooms they still use on a consistent basis. If much of the home is unused, seniors can probably downsize without adversely affecting their daily lives.

Bergen Fire Department

Shout out to the Bergen Fire Department for their nice Halloween setup! A truly great stop on a pretty windy, cold night. Complete with cider, mini donuts, candy to FILL my son’s basket, and we could even check out the fire trucks! Bonus of the free smoke detector for residents; never have too many. We appreciated it all, thank you!

Veteran appreciates generosity

I was wearing a ball cap which identified me as a veteran, and dining with my wife on 10/29 at Applebee’s in Geneseo. You can imagine my surprise when our waitress said, to thank you for your service, your dinner has been paid for by an anonymous person. This kind gesture meant so much to me. Wow, I hope that person knows how much I appreciated this thoughtfulness and generosity. It was a moment I will not soon forget. I am so thankful for this reminder that goodness in the world still exists and will find ways to spread my thanks as I pass it on.

You made my day!

I’m so grateful to the man with the braids and car full of grandsons. Thank you for making my day!

What is going on lately with so very many lost cats and dogs? It seems like every day there are notices of lost or missing cats and dogs. I’m sad for the animals and owners who are heartbroken. The other side of that is the sad amount of cats found wandering in back yards, mewing at people’s door steps, and lost dogs wandering on roads at all hours. There is info online to help pet owners understand about securing their homes, windows, and yards and providing an excellent safe pet home. Learning about a cat, dog, or any pet, is essential and understanding that by saying ‘Don’t go out’ or ‘No’ doesn’t do it. Sometimes loud noises, strangers, poorly socialized pets, getting ‘stuck’ someplace, or being just plain fearful can make an animal look for safety and escape, not knowing of course what the ‘big world’ out there is. While humane societies offer so many enticing adoptions, people should not get a pet ‘on a whim’ but rather study pet care. Let’s all keep our pets safe and happy at home. www.featuredmedia.com

PCSK9i Drugs Are Reasonable Alternatives to Statins

DEAR DR. ROACH: DEAR DR. ROACH: In a recent column, you wrote about reducing cholesterol levels with the help of statins along with a PCSK9i, such as Repatha or Praluent. My question is: Are these PCSK9is at all beneficial if one cannot take statins? -- D.C.S.

ANSWER: Yes, PCSK9 inhibitors are very effective at lowering cholesterol and reducing the risk of heart attacks in high-risk people. Because these drugs are given by injection and are quite expensive, they are not usually the firstline treatments. We have decades of experience using statins, so they remain the first choice for most.

However, if a person cannot take a statin, or one of the alternatives such as bempedoic acid, then PCSK9i drugs are a very reasonable choice. In people with familial high cholesterol and extraordinarily high levels of cholesterol, combination therapy with two or three drugs (a statin, a PCSK9i and sometimes ezetimibe) is frequently used.

DEAR DR. ROACH: My wife had COVID and has lost most of her smell and taste. This has lasted for two years. She is in her late 70s. Strong flavors like barbecue sauce and onions can be detected. Is there any hope that her condition will improve? -- P.M.

Q. A.

ANSWER: Most people recover their loss of smell after COVID. Having had this for two years is a bad sign; however, the fact that she does have some sense of smell is a good sign.

The most effective treatment for the loss of smell after a viral infection is olfactory training. The same scents that were used in clinical trials can be purchased online. Recent evidence has shown that a supplement combination, ultramicronized palmitoylethanolamide and luteolin, along with olfactory training, led to greater improvement. The dose used in studies was 770 mg.

DEAR DR. ROACH: My adult daughter (62) was diagnosed about three years ago with a skin disorder called prurigo nodularis. Her dermatologist has tried three different injections, but they’re not working. She still gets very itchy breakouts. Please give me your thoughts and more information about this. -- K.M.A.

ANSWER: Prurigo nodularis is an uncommon disease that affects about one person per 1,000. I have seen a handful of cases, and fortunately the cases I’ve seen have responded well to potent steroid creams.

Regular use of skin lotions, avoiding scratching especially at night (I recommend cotton gloves), and sometimes antihistamines like Benadryl are also the first-line therapies.

If a patient needs more effective treatment and only has a limited area of itching, then a dermatologist will often inject the area with a high-potency steroid. Since it sounds like this hasn’t been successful for your daughter, there are other options. One is a topical cream called ruxolitinib. About 45% of people responded to this treatment, compared to 20% who responded to a cream without an active drug. Ultraviolet light (narrowband ultraviolet B) showed about 80% effectiveness. For people in whom none of these therapies work, a very expensive injection treatment, dupilumab (Dupixent) or a similar drug called nemolizumab (Nemluvio), can be used, although the response rate is not as good as desired (roughly 40%).

Since many people with prurigo nodularis have other skin diseases, and some may have (or develop) depression or obsessive skin-picking, these conditions may also need to be treated to successfully improve symptoms

Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. Email questions to ToYourGoodHealth@med.cornell.edu. © 2025 King Features Synd., Inc. All Rights Reserved

SENIOR NEWS LINE

Social Security Increase for 2026

The news is in,a week late this year due to the current government shutdown: The expected increase in Social Security benefits will be 2.8% for 2026.

For the average monthly benefit of $2,015, that equates to an increase of $56, up to $2,071 for 2026 for a single person. For a couple, the $88 increase will net a $3,028 monthly benefit for 2026.

The actual Social Security increase will be less, however, because, as is typical, the Part B cost has gone up. A $21.50 increase will bring the monthly cost up to $206.50, and that comes out of the Social Security benefit.

The COLA (cost of living adjustment) is calculated every year by the price index from third quarter 2024 to third quarter 2025. Once again they’ve used the Consumer Price Index for Urban Wage Earners and Clerical Workers (CPI-W), which many think is the wrong index. (How many of us are clerical workers?) To be more accurate, the Social Security increase should be calculated by the Consumer Price Index for Elderly People (CPI-E), as that has the costs and expenses more in line with where seniors spend money.

Meanwhile, Medicare open enrollment will be in effect until Dec. 7.You still have time to look over your options and see if you need to make some changes. For example, are you taking different drugs? Visiting your doctor more now? Then there is your current plan -- has the cost changed or any of the benefits you get?

You can review plans in your area at Medicare.gov/plan-compare. Remember, scammers are everywhere and would love to have your Social Security information. They’ll approach in person, via mail, on the internet or over the phone. Don’t give out your personal information!

If you suspect fraud, you can make a report at oig.ssa.gov/report or call the Inspector General’s Fraud Hotline at 800-269-0271.
