From my Chicago apartment in 1994, then to a commercial basement in a nearby suburb, Gold Leaf Design Group was born. Out of PASSION to create, a NEED to survive and the DESIRE to bring smiles to happy customers, my DEDICATION continued to thrive and grow. Many ask how our company got its long name. Everyone likes GOLD and all the color represents. Since I'm a nature lover, LEAF (as in leaves on a tree), needed to be recognized. DESIGN is my quest in any medium. I envisioned a GROUP of like-minded people working together to create. These 4 elements continue to shape our vision since those early days. ordering information We are a wholesale company, selling "to the trade" only. All customers must provide their current resale certificate and tax ID to process their first order. BACKORDERS Although we strive to keep our products in stock at all times, this is not always possible. All backorders will be shipped automatically as product becomes available. Backorders will remain open for six months after original shipment date. Customer cancellations must be received in writing and acknowledged by Gold Leaf. Without such acknowledgment, customer assumes all responsibility.
CUSTOM, SPECIAL ORDERS, AND CHICAGO MADE DESIGNS A 60% non-refundable, non-transferable down payment is required to procure materials and begin production.
SHIPPING Most inventory items are shipped within four weeks. We ship via UPS or common carrier, consolidating freight whenever possible. We assume that we are shipping to a commercial location with a loading dock. If your location cannot accept freight, it is your responsibility to inform us at the time the order is placed.
Production time varies per project, and cannot be confirmed until down payment is received. Custom and special ordered items are nonrefundable and non-returnable. We will not reserve or schedule dates for installation services without receipt of the agreed upon, non-refundable down payment. After receipt, a minimum of four weeks will be required to schedule the installation, based upon availability.
Help us to serve you better. Please advise us in advance of any special delivery needs that may be required, including: lift gate, inside delivery, residential delivery or advance notification. These services incur additional charges. You are responsible for
all related shipping fees, either at time of invoicing or after invoicing. See TERMS for a list of charges. Orders requesting a change to the desired ship date are subject to storage fees starting at $25.00 per week. Orders requesting a drop shipment are subject to additional processing fees. Ask your Sales Representative for details. Third Party and Freight Collect Shipments: • Requesting third party or freight collect shipping will incur a $20.00 service fee • Crating and pallet charges will be applied (see TERMS above) • GLDG assumes no responsibility for freight disputes, claims, shortages and damages. All claims are the responsibility of the purchaser and the carrier. All claims are to be resolved between the purchaser and the carrier. CLAIMS Note, we do not accept claims for 3rd party & freight collect shipments (see above). Any other claims for damages or shortages, whether visible or concealed, must be made GLDG.NET

within five (5) business days of merchandise delivery, at the consignee’s address listed on the sales order. This includes storage facilities and freight forwarders. Visible damage must be noted on the delivery receipt to be considered. Please complete our “Claims & Damages” form with all of the requested information within 5 business days. The form can be found on the Contact Us page on our website. In order to process claims all boxes and packaging materials must be available for inspection. Digital photos of both the product and packaging must be provided. Submit all required information to claims@goldleafdesigngroup.com RETURNS All returns must be authorized by our claims department. No returns accepted without a Return Authorization. All items must be in their original packaging. Return Authorizations are valid for ten days. Returns that are not defective will be charged a 30% restocking fee and the purchaser assumes all freight

charges. Any remaining balance will be held as a credit on customer’s account. Authorized returns that arrive to our facility damaged due to improper packaging by the customer will not receive credit.
