Intensive care can be defined as a special medical treatment where a patient is dangerously ill has to be kept under constant observation, typically in a dedicated department of a hospital. The nurses are the ones who take care of everything that comes under intensive care. As unfortunate as it may sound, there are many incidents that hit us out of the blue. Accidents that happen and almost all of them need to be treated with medicines and most importantly intensive care. It can also be said that intensive care is a process of continuous monitoring and treatment of a seriously ill patient using special medical equipment and services.

Patients may be admitted for intensive or invasive monitoring, such as the crucial hours after a major surgery can be too unstable to transfer to a less intensively monitored unit. They require intensive care may require support for instability, respiratory compromise such as a ventilator support, acute renal failure, potentially lethal cardiac arrhythmias or the collective effects of multiple organ failure; more commonly referred to now as multiple organ dysfunction syndrome. Intensive care is usually only offered to those who have a potential to reverse the condition and who have a good chance of surviving with intensive care support.

Critical care medicine is a relatively new and increasingly important medical speciality. The specialty requires additional fellowship training for physicians having completed their primary residency training in internal medicine, paediatrics, anaesthesiology, surgery or emergency medicine. Intensive care medicine or critical care medicine is a branch of medicine that is concerned with the diagnosis and management of life-threatening conditions that may require sophisticated organ support and monitoring.

The certification in critical care medicine is available through all five specialty boards. Intensivists are the ones who have specialized in the course of intensive care. With a primary training in internal medicine, people sometimes pursue combined fellowship training in another subspecialty such as pulmonary medicine, cardiology, infectious disease or nephrology. Practitioners work in the ICU including nurses, respiratory therapists and physicians. Most medical research has demonstrated that ICU care provided by intensivists produce better outcomes and more cost-effective care.

For being a part of the intensive care team, it is necessary to have at least some knowledge about the work. The basic ICU course is undertaken by the ones who are willing to be intensivists. Intensivists include doctors, nurses and everyone who works in this department. Patient management in intensive care differs significantly between countries. In countries such as Australia, New Zealand and Spain, where intensive care medicine is a well-established speciality, many larger ICUs are described as ‘closed’.
