Introduction

Ethos Few environments touch our humanity as social creatures as profoundly as the healthcare environments we visit, routinely but also at life’s most triumphant and most troubling times. Curl La Tourelle Head’s approach honours their importance, and their complexity. When designing healthcare buildings, we consider: •

The demands of clinical practice, clinical equipment and their ongoing transformation as technology evolves;

Patient experience, including the diverse physical and psychological needs of healthy users and those who are critically ill or mortally in danger, and the needs of those who visit and support patients;

The needs of staff who work in these environments;

The business case for a site, both in terms of capital investment and ongoing operations.

To create high-performing healthcare environments, we view healthcare systems through multiple lenses. Director Wayne Head served as a Governor of Homerton Hospital for four years, a major London NHS Trust. Wayne took an active role in the action plans and monitoring the Trust’s PLACE results. Curl La Tourelle Head is therefore able to combine a management’s perspective with a Knowledge learned from an immersion with clinicians, nonclinical staff, patients, visitors and community supporters. Backed by the track record of partners and directors, Curl La Tourelle Head Architecture provides a 360-degree approach few other studios of our size can offer.

Wayne Head, BA (Hons) Dip Arch (Lond) RIBA (CA), Director Wayne is a highly-experienced healthcare architect, working with a variety of professions both within the NHS and in the wider healthcare development industry. He has worked closely with NHS Trusts on developing clinical strategies to determine capital development needs and a response to the changing care environment. Wayne leads design teams to ensure smooth implementation of capital projects at every stage. Previously a serving public Governor at Homerton University Hospital, a major London NHS Foundation Trust, Wayne took an active role in action plans from CQC reports, selecting indicators for quality audits and monitoring the Trust’s PLACE results. Wayne is therefore able to combine a client-side perspective with an insight from clinicians, non-clinical staff, patients, visitors and community supporters.

Ruth James, BA(Hons) Dip Arch PgDip ARB RIBA, Senior Project Architect Experienced Senior Project Architect with a demonstrated history of working with clients, user group, establishing a brief, design documentation to the production of fully co-ordinated project information including schedules and specification, programming, and leading complex healthcare, science and technology and interior refurbishment project to completion. Ruths Previous project experience includes: A&E Reconfiguration at UCLH: Enlarging the existing majors department and a new RAT suite (Rapid Assessment/Treatment) Anida Development of Postnatal Facility at UCLH: Stage 1 and 2 design for transforming the Fitzrovia Suites into a private post-natal facility including a family area and mothers feeding room. Private Patients Diagnostic & Chemotherapy Treatment Centre for the Royal Marsden Hospital: Complete Design stages through to Tender for the fit out of a Grade II listed building.

Alex Jones, BSc (Hons) MCIAT Alex is a dedicated member of CLTH’s healthcare team. He has played a leading role in architectural development of projects, all RIBA stages, mainly in the healthcare sector. In his previous employment Alex has worked on the following relevant projects: Danbury Medical Centre, Chelmsford: Construction of Class D1 Medical Centre providing 18 Exam Consult Rooms, minor procedure and recovery rooms with a dispensary and support services. Replacement of an older surgery set within a conservation area. Hope wood Park, Sunderland: 122 bed inpatient and outpatient for adults and older people. The development will care for patients who have complex ongoing needs and require rehabilitation services.

Jane Imbush, RGN ENB 249 Cardiothoracic Nursing ENB 998 Teaching and Assessing in Clinical practice. Specialist Advisor, Director, JL Consultancy Ltd Jane is a senior and highly experienced healthcare Director. Her skills learned working both for the NHS and in the private sector give her a deep understanding of Health and the ability to work with and across all professional and clinical disciplines. Jane has an in-depth expertise in people management and the development of successful teams, as well as a demonstrable record of operational achievement.

Dominic Papa, BA(hons) AA Dip Arch (Lond) , Co-founder + Director Dominic has 20 years of experience on large- scale urban and architectural projects. Dominic moves freely between the disciplines of architecture and urban design and brings skills in conceptual design, strategic urban thinking and detailed design. Dominic has been the director in charge of a broad cross-section of projects, including large-scale, residential-led, mixed use developments such as the award winning Schots 1&2 and Tarling Regeneration projects. Currently he is directing the development of a 7hectare sustainable urban quarter and medical centre in Plymouth, a new large mixed-use building in Plymouth.

As part of the Plymouth Hospitals NHS Trust (PHNHST) estate and clinical plans for the future, there is a recognised operational healthcare need to look constructively at a different operating model for some elements of planned elective care. With this in mind, the design team have worked collaboratively with the Trust representative to propose a flexible, generic planned care facility that will provide state of the art facilities for patients and staff in a healing and therapeutic environment.

The scope and specialty of clinical care to be delivered within the PCC is likely to be predominantly orthopaedic, but the design and clinical content of space means that it isn’t limited to a single speciality. By delivering a facility that is flexible, as generic as possible from a clinical perspective will also allow the spaces to be adapted over time as changing health needs are identified. By combining diagnostic, operating theatre, consulting and inpatient spaces into one building means it can function in its own right, whilst remaining part of the main hospital care setting.

3D Physical Model of Care Centre

Urban Form • As part of the NWQ masterplan the PCC is a critical part of a cluster of buildings that delivers important facilities and services for the hospital, firmly establishes the ‘Seaton Arc’ and creates a new heart to NWQ in the from of a generous south-facing plaza

• The building creates a strong continuous building line and street frontage to St Malo Road on the northern side. This is supported by a shallow undercroft to give shelter to patients and visitors being dropped off or waiting for a taxi .

• The ambition is to bring the diverse needs of PHNT into a consistent urban fabric – ‘an urban health campus’.

• To the south one ‘arm’ of the building extends to create frontage along Roscoff Road with a generous retail unit at the street level.

• To this extent it is important that the PCC not only delivers ‘state of the art’ patient and clinical services but also and ‘urban’ building that supports the hierarchy and definition of the streets in the masterplan. This is in contrast to almost every other building in the NHS estate whereby buildings are set back from its site edges by in accessible green buffers • PCC is located on the eastern plot that sits between two key east-west routes; one being Roscoff Road and the other being St Malo Road.

• To the east is a Woodland ‘Respite Garden’ that maintains the character of the green edge to the site visible from the main hospital. This is primarily for the patients of PCC but does allow incidental access through to the building. • To the west is a new central ‘Care Square’ plaza for the wider NWQ development and a Ravine Garden. The Ravine Garden enables the north-south pedestrian movement across the challenging topography and also enhances the quality of the space with a rich bio-diverse environment.

The building therefore proposes a flexible, generic planned care facility that will provide state of the art facilities for patients and staff in a healing and therapeutic environment. • The scope and specialty of clinical care to be delivered within the PCC is likely to be predominantly Trauma and orthopaedic, but the design and clinical content of space means that it isn’t limited to a single speciality. By delivering a facility that is flexible, as generic as possible from a clinical perspective will also allow the spaces to be adapted over time as changing health needs are identified. By combining diagnostic, operating theatre, out patient consulting, and inpatient spaces into one building means it can function in its own right, whilst remaining part of the main hospital care setting.

• The planned care centre is planned as a three-storey building in order to facilitate easy, legible horizontal adjacencies between the different clinical spaces and departments. • Its height is commensurate with its surrounding neighbours as it has large floor to ceiling heights. This is to enable long-term flexibility for the clinical spaces through sufficient space for the mechanical and electrical demands and maximizing the structural spans. • The roof plant room and loading bay spaces are integrated into the form and massing of the building to create a strong silhouette within the masterplan.

Ground Floor • Here the building is accessed directly off of St Malo Road via the ‘Arrival’ area and into the ‘Welcome Lounge’ • The ‘Arrival’ area offers a sheltered drop- off area and forecourt so that patients can be delivered to the front door directly if need be. A bus stop adjacent to the ‘Arrival Area’ will in the future deliver public transport services too. • Entry into the building brings you directly into the Welcome Lounge. This is a place for visitors and patients to wait. A reception and coffee shop is provided. Views to the Respite Garden and to the Courtyard bring legibility and orientation in a calming environment. The courtyard is a point of reference throughout the building bring light deep into the building and the different departments. • Directly accessible off of the Welcome Lounge are the most frequently visited areas; the Diagnostics department and Outpatients department. The location of the Diagnostics at the ground floor also allows removal of CT and MRI equipment from St Malo Road when necessary. The location and organisation of the outpatients department allows for the consultation room sot be split into two groups of ten. As it is elevated off of the Plaza level this brings added privacy with views to the ‘Respite Garden’ and ‘Care Square’ plaza below.

First Floor Here are the more private clinical spaces such as operating theatres, post-operation recovery and Inpatient wards. This also gives the wards and theatres more privacy, and where they can get maximum benefit from daylight / sunlight and long views. At this level there is an upper ‘Welcome Lounge’ with reception for people preparing for pre-op. This is visually linked to the ground floor ‘Welcome Lounge’ via a double height space facing the courtyard to give added legibility to the building.

Dementia Care Research NHS Trust We have been undertaking space studies to develop a strategy for the potential change and improvement for dementia sufferers in existing care centres. Through developing varying degrees of remodelling of rooms and spaces we aim to improve the patients’ experience.

Outline Masterplan Research In addition to the Mary Seacole Centre study, we have developed an outline masterplan for a dementia care village set within the main Trust site. Principles of glass, orientation and light shape the layout to create sheltered lounge and court-yarded spaces.

Copenhagen Collaborators NORD Architects Alzheimer Village - Dax France NORD Architects have designed an Alzheimer Village in Dax, France. it is an innovative renewal of a traditional nursing home for people with Alzheimers disease. The Core of the design revolves around a village centre that caters for everything needed to live a normal everyday life. A Village shop, hairdresser, restaurant, cultural centre and healthcare centre allows access to the most necessary amenities. 4 houses create a neighbourhood with its own identity and courtyard. All together there are 4 neighbourhoods connected by an urban street leading up to the village centre. Paths full of experiences and sensory inputs with different characters lie as loops in the landscape. Images Provided by NORD Architects.

Copenhagen Collaborators NORD Architects First Urban Hospice In Denmark NORD Architects have designed a new hospice for the deaconess community, located in Copenhagen, Denmark NORD have successfully created a warm and protective atmosphere, within the city parameters. The design incorporates both curved and rectilinear elements that allow for an optimal functional layout, built around a private inner courtyard. ‘By considering the needs of the users, client and neighbors, Urban Hospice sets a new standard on how you can build innovative healthcare projects in urban contexts.’ - Morten Rask Gregersen partner in NORD Architects Copenhagen.

The Pulross Centre London Borough of Lambeth Client

Community Health South London NHS Trust

The Pulross Centre, exemplifies the shift away from the hospital to a primary care led National Health Service and combines sophisticated clinical facilities with strong roots in the local community. Serving an area of high deprivation, the centre views health as ‘a state of mental and social wellbeing, not merely the absence of infirmity’. A hub for 23 local GP practices, the Centre provides specialist day clinics for patients in the neighbourhood as well as in-patient beds for people needing 24-hour medical care. Behind a welcoming glazed facade, the main entrance leads into a double height foyer from which most areas are visible and accessible. The ground floor is designed

for open access and houses a variety of public spaces including a reception and café, as well as clinics and a day room. The rest of the building is designed to be more secure. Accommodation for short stay inpatients – wards and single rooms – is located on the first floor, directly linked to a raised private garden, formed from the rubble of the hospital building formerly on the site, which provides a calm yet stimulating sensory environment. Circulation maximises views out to the landscaped forecourt. Walls are protected from trolleys with a specially designed beech and stainless steel handrail and wall guarding system. Children’s Eye Hospital Moorfields Eye Hospital Client

Moorfields Eye Hospital NHS Foundation Trust

This world-class, dedicated treatment centre for children’s eye conditions combines the clinical expertise of Moorfields Eye Hospital with the research base of the adjacent Institute of Ophthalmology promoting fast transference of ‘bench to bedside’ research findings. The new building creates a landmark facility in line with the hospital’s international status and aspirations. The dedicated centre provides a variety of facilities and overnight accommodation for patients and families and addresses highly prescriptive clinical requirements without losing sight of a supportive and child-friendly patient environment. The design dispels preconceptions of hospitals to create an holistic, child-focused and welcoming environment. A potentially traumatic

experience becomes a positive one, supported by a clear layout which gives a sense of security, engaging colour schemes and large relaxation and play areas. Overall, the design offers a textured and engaging experience, and a sense of discovery for patients whose ages range from 0 to 16 and who may find their treatment lasting for months or even years. An integrated art programme includes lighting, interiors and the building’s entrance façade which features an arrangement of freely placed folded aluminium louvres to minimise solar gain and create an iconic street presence. Green Wryth Lane Health Centre Surrey Client Cost

Building Better Health & Wilmott Dixon Construction Ltd. ÂŁ4.3m

This new health centre replaces a modest post-war clinic and offers a range of primary health and social care services. Some social housing will be added to the site in the second phase of the project. The plan and scale of the health centre give it a strong civic presence, with the traditional brick of the street facade and the clay roof tiles respecting the local 1930s vernacular. The building wraps around its corner site, creating a tranquil inner courtyard. The inner facade is clad in timber, contributing to the softer, more intimate feel of this landscaped space.

The courtyard is a constant visual reference point, especially from the three-storey entrance hall, which is the physical and operational heart of the building. Around it, the accommodation is arranged in distinctly zoned service areas. While some of the facilities are shared, it is never necessary to walk through one department to get to another. The large, open-able windows maximise daylighting and, with their innovative acoustic attenuators, allow the building to be naturally ventilated despite the high level of external traffic noise. The Eye Centre, The London Clinic, Harley Street Client

The London Clinic

Alterations and extension to an existing town house in Harley Street London to create a combined ophthalmology consulting house and Eye Centre. With an awkward Edwardian layout and conservation area sensitivity, the key principles in the new design were consistency of public realm spaces, clear visual definition and a consistent layout of the building to silently assist the user. Consultant suites are located on the upper storeys which enjoy town views, and clinical spaces are located on the

lower levels. With laser technologies, vision lanes and optometry facilities in the lower levels of the building, the world class consultants enjoy upper floor rooms with town views and balconies, to create their own individual touches. The design uses waiting areas to carefully keep patients in close proximity of clinical staff. Various lightwell infills and extensions to the building allow a central waiting hub at each floor level, connected by a new revised lift core. All created with the vision of inspired care. The Hybrid Theatre The London Clinic, Harley Street Client

A ‘world class’ ultra clean hybrid operating theatre for the treatment of cardio vascular disease was the result of careful synthesis between emerging clinical techniques and aesthetic objectives. Hybrid operating theatres are likely to become the ‘norm’ as more patients benefit from minimally invasive catheter-based procedures in collaboration with established complex open cardiac, vascular and traditional surgery. The London Clinic intends to use this project for benchmarking future hybrid theatre provision. Shortly after completion HRH The Duke of Edinburgh had surgery in this theatre.
