This grade book report represents the workload of a student I have worked with toward  establishing good work habits and workload management/planning.  The report shows that  throughout the quarter there were several late assignments and several tests retaken due to  low scores.   All work completed and turned in was the primary goal for this student.  This  artifact shows that this goal was achieved.  What may not be clear from this artifact is the  behavioral development that occurred.  The subject specific themes (math) involved solving beginning algebraic equations.  Over a  period of time through much practice and repetition the student showed only slight  improvement chiefly due to consistently battling late work due to poor choices. This artifact shows that the work I do with students involves two areas of focus on my part  ­1) the content/academic, 2) the behavioral attitudes and actions.  Both areas must be  addressed­most of the academic focus is carried out by the classroom teachers.  My chief  function is to help the student develop patterns of good behavioral choices that will lead to  academic success.  Over time I have learned that progress in the behavioral change comes  slowly.  This artifact is gives a glimpse of some of what the progress looks like.
