On 31 March 2020 the overall recorded identification of ‘living’ Palliative Patients against total GP Practice Population was: 0.30% for West ICP Practices and 0.55% for East ICP Practices. Locally we have an agreed standard that stipulates 0.45% (or above) of the practice population will be identified as being palliative. Nationally it is recognised that an estimated 1% of the practice population will die every year. Between the 1 Sept 2019 and 31 March 2020 the overall recorded identification of patients as being palliative prior to their actual death was 26.6% for West ICP Practices and 37.4% for East ICP Practices Locally we have an agreed standard to achieve 40% or more.
