Update Excellent Care; Every Patient Every Time

Einstein 360 Update

We continue to see improvements in revenue cycle as our money collected with RVU continues to increase. Advanced Access has been rolled out in Neurology and for the first time in a long time, appointments can be made within two weeks of a request. All ECHA practices, Cardiology and Orthopedics are on Advanced Access schedules. With our partner ZocDoc, appointments now can be scheduled online for many of our physicians and Advanced Practice Providers from the Einstein.edu website. The direct link is providers.einstein.edu. Please encourage your patients to use the online scheduling! After initial improvement, our no-show rate has plateaued at an unacceptably high level. This has put us behind the number of visits that we budgeted in December and January. We are focused on improving our reminder system, implementing our no-show policy and will be considering strategic overbooking in areas that are behind our budgeted visits that have high no-show rates. Our Leadership Academy continues to educate Einstein Physician Leaders, Emerging Physician Leaders and Administrators. As part of our EMR Optimization strategy we will be implementing Dynamic Documentation pilots in our North Wales and Mayfair primary care practices and in EPPI Endocrinology and Urology on March 6. We hope to gain experience to better prepare us for practice wide go-live now planned for September.

p|2 ED Initiatives Improving Patient Flow

Throughout the last year, Dr. Elizabeth Datner, Chair of Emergency Medicine, and ED physicians and staff have been working on initiatives to improve patient flow, addressing patient wait times and ED crowding.

Maternity Care Virtual Tour

Time-Off Notification Planning a conference or meeting? Policy requires Einstein Physicians to provide 90-day notification for time off so as not to disrupt scheduled patients. Exceptions will only be for unexpected, urgent events. Remember, we are patient centric.

Strategies included establishing new roles of rapid assessment providers and patient flow nurses, allowing nurses to initiate order sets, incorporating more national evidence-based medicine into treatment, creating an observation unit and new internal waiting room and streamlining discharge processes. “Our goal with all of these changes is to achieve the highest quality care in the most efficient manner and be able to treat as many patients as possible,” said Datner.

Some Early Measurement of Success: • •

The ED has decreased its door-to-physician time from 47 minutes in July 2017, to 31 minutes in December 2017. The patient walkout rate has improved from 4.90% to 3.52%, resulting in an additional 1,318 patients being cared for in their time of need. In December 2017, the patient walkout rate was at an alltime low of 1.67%. The time from arrival to admit decision has improved from 220 minutes in July 2017, to 175 minutes in December 2017.

According to Datner, the EDs also have established a “pull ‘til full” strategy to speed up patient flow. “If there is a bed available, we do not keep a patient in the waiting area.” “We want to connect patients and physicians as quickly as possible so that we can make a timely diagnosis, provide appropriate treatment and achieve great outcomes,” said Datner. In addition, ED leaders and staff have been diligently rounding on patients and families, either through hourly waiting room updates or leader rounding, to improve communication with patients and families and enhance the patient experience. “These departmental improvements have been made through cohesive leadership, and the hard work of all ED staff,” said Steve Chapman, Senior Director, Emergency Services. p|3 Radiology Clinical Decision Support Pilot Group Needs Volunteers As many of you may already know, radiology clinical decision support (CDS) was mandated for ordering advanced imaging studies in a bill known as PAMA (Protecting Access to Medicare Act). The bill requires the use of clinical decision support prior to ordering of advanced imaging (defined as CT, MRI, and nuclear medicine studies) in certain practice settings. The implementation date mandated by CMS is January 1, 2020, at which point CMS will reimburse only advanced studies that have demonstrated use of clinical decision support.

Please consider participating in this extended pilot program. The more input we receive; the better we can make the software. We would like to have residents participate too.

In addition, the recent CMS final rule (published in November) confirms the use of radiology CDS as a measure in the Merit-Based Incentive Program (MIPS) with possibility of bonus points and thus physician reimbursement beginning in 2018.

Endocrinology Associates Relocation and Provider Schedule

Endocrinology Associates relocated their office in Plymouth Meeting to various offices in the community. (The Plymouth Meeting office closed December 29, 2017.) New offices are located in Collegeville Multi-specialty, King of Prussia Family Medicine, Einstein Physicians Blue Bell, King of Prussia Multi-specialty and Colmar offices, as well as an office at EMCM. p|4 Leadership Transitions Dr. Vincent Figueredo Appointed Chair of the Division of Cardiology Dr. Vincent (Vince) Figueredo has been appointed the Chair of the Division of Cardiology. He served as interim chair of the department since July 2017. During more than a decade at Einstein, Vince has distinguished himself as an outstanding cardiologist and physician leader in his roles as Program Director for the Fellowship Program in Cardiology, Associate Chair in the Department of Medicine and leader for EPPI’s physician engagement initiatives. He will continue to serve as Program Director for the fellowship program and Associate Chair in the Department of Medicine. Please welcome Vince as our Chair of Cardiology, and offer him our enthusiastic support.

Leadership Transition in the Division of Pulmonary, Allergy and Critical Care Medicine Dr. Sunil Sharma stepped down from the Chair position in the Division of Pulmonary, Allergy and Critical Care Medicine to dedicate more time to the training program and to building research in the division. He will remain Program Director for the Fellowship Program in Pulmonary and Critical Care Medicine, and Director of Research in the Division. During his tenure as Chair, Dr. Sharma initiated multiple innovative programs and research projects, and recruited excellent faculty members. Dr. Michael Walkenstein will serve as Interim Chair of the Division. Dr. Walkenstein has been at Einstein for more than 35 years, and has made important contributions to the division and to the Einstein community. Please join me in supporting Dr. Walkenstein in his new role. The search for a new Chair of the Division is underway.

Physician Recognition Orthopedic Residents Lead the Nation! Every year, orthopedic residents in all 157 residency programs in the country take the Orthopedic InTraining Examination (OITE). A test of resident knowledge, it’s also used to measure the quality of teaching within individual programs. So which program has had the highest overall score in 2017 – for the last FOUR years? Einstein, of course! For the fourth year in a row, Einstein’s orthopedic residents achieved the 100th percentile mark, the best overall score of all 157 orthopedic residency programs nationwide. p|5 Dr. Jamie Raphael, Chairman of Orthopedic Surgery, said, “The department has gathered together a fantastic group of intelligent, professional, hardworking residents. Our chief residents, Dr. Mark Ayzenberg and Dr. Dillon Arango, have done an extraordinary job leading this team well and I believe Dr. Shaffer is the best orthopedic residency program director in the nation. I’m so proud to be a part of this team.” Dr. Raphael credits the structure of the residency program and its prioritization of daily educational time as the driving forces behind these high marks. “Each day, all residents have protected educational time (an hour and a half four days a week, and two hours one day). This protected educational time (used for lectures, case reviews, studying and grand rounds) is known and respected by all of our attending physicians; we know we cannot interrupt it.” Well done!

Emergency Medicine Resident Dr. Doug Stewart to Present Case at 2018 National Emergency Medicine Conference Congratulations to Dr. Doug Stewart for having his case on psoas abscess infectious disease accepted for presentation at the 2018 National Emergency Medicine Clinical Pathologic Case (CPC) Conference at the Council of Emergency Medicine Residency Directors (CORD). Dr. Megan Stobart, Associate Program Director of Residency, will participate as the faculty discussant. The case was recognized among the top 72 submitted this year. Dr. Stewart will present it in San Antonio, Texas, on April 22, at the CPC semi-final competition, for the chance to go onto the final competition at the 2018 ACEP Scientific Assembly in San Diego later this year. We wish Dr. Stewart good luck at the competition!

Department of Radiology at RSNA Annual Meeting

It was an unparalleled year of success for members of Einstein’s Department of Radiology at the Radiologic Society of North America (RSNA) annual meeting. EHN radiologists won eight awards, including two Magna Cum Laude, five Certificates of Merit and one Quality Storyboard award. And a total of 17 projects (educational posters, scientific posters and quality storyboards) were accepted. EHN’s Musculoskeletal Radiologists, Drs. Tetyana Gorbachova and Micah Cohen, set the bar high with two Magna Cum Laude awards (out of only three Magna Cum Laude designations). Co-authors on these award-winning presentations include Dr. Hongju Son, Chief of Thoracic Imaging and three residents, Drs. Bridgette McCabe, Matt Callaghan and Brett Cerniglia.

New Physicians Dr. Scott Beaudoin Joins Department of Emergency Medicine Faculty Please join me in welcoming Dr. Scott Beaudoin to the Emergency Medicine faculty. Dr. Beaudoin comes to Einstein from Lower Bucks Hospital Emergency Department. He received his medical degree from Drexel University College of Medicine and completed his Emergency Medicine Residency at Temple University. Dr. Beaudoin has been an EMT (in Pennsylvania and New Jersey). He has academic interests in cardiac arrest resuscitation and Emergency Medical Services.

Georgetown University School of Medicine, and completed her residency in Emergency Medicine at Drexel University College of Medicine/Hahnemann University Hospital, where she received the Mercy Award for clinical excellence in Emergency Medicine.

Dr. Jeanne Sandella Joins Trappe Family Practice Dr. Jeanne Sandella has joined Trappe Family Practice as of January 1, 2018. Dr. Sandella practiced at Philadelphia College for Osteopathic Medicine since January 2010 and was the Vice President for Clinical Skills Testing – National Board of Osteopathic Medical Examiners. Jeanne graduated from PCOM and completed a Family Medicine residency at Mercy Suburban Hospital in Norristown.
