14th September 2018 Important changes to NHS dental care at East Ayton Dental Please read this letter carefully

Dear Patient I am writing to inform you that, in order to maintain the service we currently offer, the practice has made the difficult decision to no longer offer treatment on the NHS after 1st March 2019. Please be reassured any appointments currently booked before 1st March will be on the NHS as usual, and I look forward to discussing your future options with you at your next visit. To ensure that I can continue to provide regular continuing care after 1st March I will be offering dental care under a Denplan dental payment plan provided through Simplyhealth Professionals, or on a private fee per item basis. There are several Denplan options available which we will be happy to discuss with you at your next routine appointment allowing you to decide which option suits your needs. Please find enclosed some information leaflets about the Denplan services and a sample of the basic private charges. Denplan Care helps you budget for your routine dental care including fillings, root fillings and extractions along with examinations, x-rays and scaling. 98% of patients will pay between £15.50-£29.00 on this plan. Your monthly fee is based on your previous dental treatment, current oral health and anticipated future needs. A free assessment can be carried out at your next NHS appointment. Denplan Essentials allows you to budget for routine examinations, x-rays and scaling and offers a discount of 10% on standard private fees. £18.40 per month will provide two examinations and two scale and polishes per year. Denplan for Children (age 6-17) is £5 per month, and covers routine examinations, x-rays, fluoride treatment and preventative care. Children of registered patients will be seen for free up to the age of six. There are also discounts of up to 15% on monthly fees when two or more adults pay on the same Direct Debit (T&Cs apply). The prices above all include the optional Denplan Supplementary Insurance from Simplyhealth Access. This insurance provides dental emergency and dental injury cover supported by a 24-Hour Worldwide Dental Emergency Helpline. Please see the enclosed leaflet for terms and conditions. If you choose not to take out the insurance the monthly fee is reduced by 60p. I will aim to complete any courses of NHS treatment I’ve already started. If you would like to enquire further about NHS dental services please contact the NHS Helpline on 111. Please give us a call on 01723 864295 if you have any further questions. Thank you for your loyalty in supporting our practice, we all look forward to seeing you again soon. Yours sincerely
