Objectives This new area of the Surgery has been developed as consequence of the results obtained by morbid obese patients that suffered diabetes type 2. These patients few weeks after undergoing to anti-obesity surgery, and even before losing weight, they standardize the levels of blood glucose. During the more than 30 years of experience with obesity surgery has registered a rate of recovery of the diabetes type 2 of more than 90%. Diabetes Surgery would be applied in patients with normal weight but diabetic type 2. It has as objective to exclude of the traffic of food portions of the small bowel, with the aim of cause changes in hormones and receptors located in this area that related to the control glucose levels. It is well know that the GLP-1 is produced by L cells of the ileum and stimulated mainly by carbohydrates, which release it is activated by the secretion of insulin. On the contrary, when food is in contact with the duodeno-jejunal mucosa, stimulate the production of other hormones that tend to elevate the levels of glucose. Therefore surgery should avoid the contact of food with the first portions of the intestine and bring it to parts far from the stomach. Furthermore decrease leptine levels (synthesize by fatty cells) and of glucagon (that acts on carbohydrates). But this new Surgery has just started. Very fast advances are taking place that we intend to review and to update for offering our patients always the best.
