This analogy illustrates that hazards are prevented from causing human losses by several layers of defense. The cheese slices represent safety regulations, tools, initiatives and human factors. In daily life there are flaws in each layer and if these wholes align, accident happens. The more holes there are, the bigger danger we are in. Accidents can be caused by active errors or error traps. Our programs aim to eliminate holes that can be linked to the human factor and to insert new cheese slices into the system.
