bout one month after the 2009-10 school year began, Christa Newman, an elementary schoolteacher, collapsed at work and was rushed to Memorial Medical Center in Springfield, Ill. Newman, now 34, was resuscitated on the way to the center, rushed into the intensive care unit and had her body temperature cooled with a form of hypothermia to slow down her brain's demand for

oxygen. Two months later, after open-heart surgery to repair a coronary artery, Newman was back at work. Today, she's enrolled in a master's degree program at the University of Illinois at Springfield, and she says she's had no problems with her memory or any memory loss. The procedure used at Memorial is called therapeutic hypothermia, a procedure that helps stop brain damage in patients who suffer cardiac arrest. The treatment lowers a patient's body temperature to slow the brain's demand for oxygen and prevent damage to cells.
How it works Ordinarily, a person whose body temperature drops significantly risks organ failure and death. But in the controlled conditions of a hospital, dropping a cardiac patient's body temperature is meant to stave off brain damage. Dr. Rajagopal Sreedhar, medical director of the adult intensive

care unit at St. John's Hospital in Springfield and assistant professor of pulmonary and critical care medicine at Southern Illinois University School of Medicine at Springfield, says therapeutic hypothermia can be used after a cardiac patient is resuscitated. "A person has a cardiac arrest, and their heart stops beating, and there is no circulation to the brain. The goal is to allow the brain to tolerate blood reperfusion following oxygen deprivation," Sreedhar said.

Study tests low-sugar cereals A new study published in the January 2011 issue of Pediatrics measured what 91 children at a summer day camp ate when served either high-sugar or low-sugar cereals. All of the children were given their choice of either three high-sugar or three low-sugar cereals, as well as milk, orange juice, bananas, strawberries and sugar packets. Children served themselves and answered a questionnaire after breakfast. Children in both groups said they "liked" or "loved" the cereal they ate. But children in the high-sugar group ate larger portions of cereal, consuming almost twice as much refined sugar (24.4 grams) as children in the low-sugar group (12.5 grams). Children who ate low-sugar cereal consumed similar amounts of milk and total calories and were more likely to put fresh fruit on their cereal. — American Academy of Pediatrics

Use for strokes? At St. John's, this involves using the Artic Sun cooling machine, which circulates cold water to pads that are placed on the patient at the torso, thighs and abdomen. The patient's core body temperature drops down to between 91 and 92 degrees, which induces a therapeutic hypothermia. Patients are cooled for about 24 hours, and then
the body is slowly re-warmed for another 24 hours. Sreedhar also said St. John's is researching whether the treatment could be used to help patients with brain injuries and strokes. Studies have shown that cardiac patients who have had the procedure are less likely to suffer brain damage than people who receive standard care, but the disruption of blood flow to the brain can have some lingering effects.

Worry about children with heart palpitations?
Q
:My 8-year-old son said it felt
like his heart was pounding. I measured his heart rate (I am an EMT), and it was 110 and regular. What can this be from? A: Palpitations (the awareness of a fast, irregular or harder than normal heart beat) are common in childhood. They are usually a reaction to stimuli like fever, exercise, anxiety or, less commonly, low blood count or a thyroid problem, rather than problems with the heart itself. However, there are also cardiac, or heart, causes. Although these can occur in any child, they are more common in children with a history of congenital heart problems, especially those who have required heart surgery. Any seriously ill child should be urgently evaluated, but healthy-looking children with palpitations should be evaluated by a health-care provider, too. The first step in this evaluation is a thorough history and physical exam. Identifying non-cardiac stimuli that caused the heartbeat to be rapid reduces concern for a heart problem. An abrupt start and stop to the palpitations is suggestive of an issue with the heart's rhythm coordinating electrical system. This creates the potential for the electrical signal to go up (or down) the shortcut pathway then down (or up) the normal pathway, racing around and around and stimulating an abnormally fast heartbeat, typically 220-280 beats per minute in infants and 180-240
is a very common and typically benign occurrence. An electrocardiogram, or ECG, looks at the heart's electrical activity and can identify many possible anomalies. Because this is an easy, non-invasive test, it is often done as an additional part of the evaluation of palpitations. Thankfully, most cases of palpitations in children are benign and are caused either by non-heart related issues or to occasional extra-systole beats.

LAW PLACES LITTLE VALUE ON CHILDREN, SENIORS In case you hadn’t heard, there was another fatal car wreck in Livingston County recently. Sadly, a two year old boy from Wyoming County was killed in Leicester in a weather related crash. When someone dies in such an accident and the other driver is at fault, the family of the driver who was killed can commence a lawsuit against the responsible person for the “wrongful death” of their loved one. However, under New York law, all that the family can sue for on a wrongful death claim is the present value of the financial or “pecuniary” loss sustained by the family of the deceased. This means that the family can only get the value of the deceased’s lost future wages and value of the work that the deceased would have performed around the house. When the deceased is a child or a retired senior, this amount is very small, often not more than $25,000 or so. In order to get any more than this financial or pecuniary loss, the family members would have to prove that their loved one was conscious after the accident for a prolonged period, known as “conscious pain and suffering.” The longer the person is awake and suffering, the more the family would be entitled to. However, when death comes almost immediately, the family is not entitled to money for their loved one’s pain and suffering, under the theory that, simply, that person didn’t suffer. I think it is rotten that, as a state, we place so small a value on the lives of children and seniors. Why should it matter if they died quickly or lingered? While no amount of money can ease the pain of losing a child or parent or grandparent, the law at least ought to give fair compensation to the surviving family members. What do you think?
