Treatments Abound for Migraines DEAR DR. DONOHUE: Please say something about migraine headaches. I think that’s what I have. They come on fast, and the pain feels like something is pulsating in my head. I throw up with every headache. Bright lights make things worse. Does all this sound like a migraine to you? Someone told me there’s always a warning before a migraine starts. I have no warning. I’ve used Tylenol, but it doesn’t do much for me. What is the treatment? Can they be prevented? — L.G. ANSWER: Your headaches have many of the characteristics of a migraine headache. The warning mentioned to you is an aura. The aura usually precedes a migraine. It can be flashing zigzag lines, peculiar sensations — often in the hands — trouble finding the right words to speak or weakness of a group of muscles. Only 20 percent of migraine patients have an aura, so it’s not an indispensable migraine sign. Nausea and vomiting are common migraine symptoms. Seventy percent of migraine patients have a one-sided headache that they describe as throbbing or a dull ache. It lasts from four hours to

three days. People with migraines are very sensitive to light and sound, so they seek a dark, quiet room to lie down. Stress, overexertion, sleep deprivation and hunger can provoke a migraine. Some foods and drinks can do the same. Alcohol (especially red wine); caffeine; pickles; bananas; yogurt; avocados; aged cheeses; pickled or marinated chicken, beef or fish; salami; pastrami; bacon; pepperoni; hot dogs; and the taste enhancer monosodium glutamate are on the list of possible migraine inducers. Medicines for treating a migraine abound. Tylenol, aspirin and drugs like ibuprofen take care of mild migraines. For more severe ones, triptans are the standard treatment. They include Imitrex, Axert, Frova, Zomig, Amerge and Maxalt. An older medicine, ergotamine, still has a place in treatment For migraine prevention, propranolol, verapamil and amitriptyline are prescribed if the headaches occur often and disrupt life. looked at me as if I should have done something. Her gasping stopped, and she could breathe. She asked me why I didn’t jump up and hit her back. I heard if you hit somebody choking on the back, it could force the food farther down. Did I do the right thing? — R.M. ANSWER: The Red Cross has changed its instructions on how to handle a choking adult. It now recommends leaning the person forward and giving five blows to the back between the shoulder blades with the heel of the hand. If that doesn’t dislodge the food, then start the Heimlich maneuver. Dr. Heimlich was never in favor of back thumps. While some couples go all out and invite everyone they know to their wedding, others opt for a smaller, more intimate affair. Although they can be less costly, small weddings are not always easy to pull off. Whittling the guest list to a select few can be difficult and cause conflict among family members. If you would like to keep your wedding small, you need to be aware of the pitfalls and approach the initial planning with tact. You must decide first just how small you want your wedding to be-10 people, 20 to 40 people, 100 people? The smaller your guest list, the more lightly you will have to tread. There will be people who expect to be invited to your wedding and they will be hurt and angry when they do not receive an invitation. You can accommodate for this by having a large bridal shower or post-wedding party and inviting everyone not invited to your wedding to that. With number in hand, you can turn your focus to whom to invite. Depending upon how small your target number is, you may have to make some hard decisions like excluding extended family or children from your guest list. Whatever your decision, be prepared for conflict. No doubt there will be some hurt feelings. There might also be pressure to change your mind and have a larger wedding. Don’t give in. It is

your wedding. If you want it to be small, you should have it that way. After you have completed your guest list, make a list of everyone you couldn’t invite and find a way to let them know why they weren’t invited. Send them a note, shoot them an e-mail or give them a call. If you intend to have a large bridal shower, you may be able to wait until then to let everyone know that you will be having a small wedding. When letting others know of your decision, be tactful. Let them know that while you would have liked to invite everyone, you chose not to for personal, financial or other reasons. Reassure them that they are still very much a part of your life and thank them for everything they have done for you. In spite of your best efforts, some people will not understand why they were not invited. They will be hurt, angry and disappointed, so much so that they may refuse to attend your bridal shower or post-wedding party. Try not to let it get to you. Give them time to cool off and focus on making your wedding the best that it can be. Choosing to have a small wedding is easy; implementing the idea, on the other hand, takes work. Be prepared and forge ahead with a smile on your face.

TIDBITS RIDE GUIDE Bowyer Is Long Overdue RICHMOND, Va. — The prevailing view is that the Aaron’s 499, run at T a l l a d e g a Superspeedway and won by Kevin Harvick, was a NASCAR classic. It all depends on the perspective, or so said Harvick’s Richard Childress Racing teammate Clint Bowyer, who finished seventh in that race. “It was boring as far as my situation went,” said Bowyer. “I rode around in the back and snuck in and got a decent finish out of it. That’s the only way I can get a good finish there. “I hate doing it. It’s boring and no good for your sponsor and everything else. But ... Chase. That, of course, is still 16 races away. Only a Cup regular since 2006, Bowyer has already made the Chase twice, finishing third in 2007 and fifth in 2008. All three RCR drivers made the Chase in 2008. None did so in 2009. Bowyer, who will turn 31 this month, has collected three top-10 finishes in the past five races, finishing 12th in the most recent at Richmond International Raceway. He drives the No. 33 Cheerios/Hamburger Helper Chevrolet. Twice a Sprint Cup winner, Bowyer has gone 72 races — exactly two seasons — since his Richmond victory on May 3, 2008. “Not winning ... you never get over it,” he said. “We are racers and grew up racing and winning races. I hate losing. It has been a long time since we won a Cup race, and we’re long overdue. I feel like it’s our turn, and we need to get it done.” Bowyer’s two victories occurred at Richmond and New Hampshire. “I think the RCR shorttrack program has always been good,” he said. “This track (Richmond), I don’t know why, it just kind of fell into my driving style. I love this place. You have got to get up on the wheel and make it happen

at a track like this. “I think this is a perfect size race track. It’s just fast enough that you get that sensation of speed that we all like, and it’s just short enough that we can beat and bang on each other, and root and gouge for a win.
