ELIGIBILITY & ENROLLMENT

Who is Eligible to Receive Benefits?

•

Medical/Prescription Drug, Dental, and Vision Plans: Full-time employees who are regularly scheduled to work at least 30 hours per week.

•

Basic and Supplemental Life and AD&D Coverage: Full-time employees who work at least 40 hours per week.

You may enroll your eligible dependents when you enroll yourself. Eligible dependents include: •

Spouse or civil union partner

•

Your dependent children, including: — Your natural born child(ren), legally adopted child(ren), step-child(ren), or court-ordered dependent child(ren) for whom you are the courtappointed legal guardian — Your dependent child(ren) up to age 26. *

Your continuously disabled child(ren) [if disabled prior to age 26] who is incapable of self-sustaining employment and dependent upon you for support, regardless of age. Please note, documentation is required if you are covering a disabled dependent over the age of 26.

How Do I Enroll?

You have 7 days from your date of hire to complete your benefits enrollment. If you enroll within this timeframe, your coverage will begin on the first of the month following 30 days of employment. To secure your benefit elections for the plan year, you MUST complete your enrollment through ADP Workforce Now. The benefit elections you make during this enrollment period will remain in effect until May 31, 2027, unless you experience a qualifying life event.

Qualifying Life Event IRS Section 125 prohibits you from changing your enrollment during the plan year, unless you or one of your eligible dependents experiences a qualifying life event. A qualifying life event includes: •

Marriage

Divorce

•

Death of a spouse or a dependent

•

Birth or adoption of a child

•

Termination or commencement of employment for your spouse

•

A change in employment status (full-time to part-time or part-time to full-time) for you or your spouse that affects benefit eligibility

•

Taking an unpaid medical leave of absence by either you or your spouse

There are no out-of-network benefits for the Low and Middle plans. Services provided by doctors, hospitals, and other healthcare professionals that do not participate in the Low and Middle plans’ network will not be covered, except in the event of a true emergency.

Individuals enrolled in the Low and Middle plans can access doctors, hospitals, and healthcare professionals within the Aetna Whole Health (AWH) Network. These plans do not offer out-of-network benefits; services from providers outside the AWH Network are only covered in genuine emergency situations. No Primary Care Physician (PCP) selection is required. You can visit participating specialists directly without a referral.

Why Colon Cancer Screening Matters Colorectal cancer is the third most common cancer in the U.S., but early detection saves lives. Screenings starts age 45, with options including a colonoscopy every 10 years or a Fecal Immunochemical Test (FIT) every year thereafter. A FIT is a simple, at-home test that checks for hidden blood in your stool — a possible early sign of cancer.

Ages 45+: Schedule a preventive colonoscopy* or order a FIT — both covered 100% under the health plan.

Ages 30-44: You may order a ColoFIT kit through LabCorp, which will be covered 100% by Air Group from June 1 to December 31, 2026. Future availability will be reviewed after this period.

* Colonoscopy includes pre-procedure specialist consultation, removal of polyps during a screening procedure, and a pathology exam on any removed polyp.

Express Scripts Pharmacy

Specialty Medications

Express Scripts has a network of nearly 60,000 independent and chain pharmacies nationwide. When you visit a participating pharmacy, you will receive up to a 30-day supply after paying the applicable cost share (or a 90-day supply of maintenance medications through the Smart90 Program). Remember to take your Express Scripts ID card with you.

Specialty drugs are medications that require special handling, administration, or monitoring — these are often given by injection. Your provider will let you know if a medication you are prescribed is considered a specialty drug. Specialty medications must be filled through Accredo.

Please note, if you take a specialty medication, you are not automatically enrolled with Accredo Specialty Pharmacy.

SaveOnSP To help lower the high cost of specialty medications, Air Group’s plan includes a copay assistance benefit through SaveOnSP. Nearly 400 specialty drugs for conditions such as hepatitis C, multiple sclerosis, psoriasis, inflammatory bowel disease, rheumatoid arthritis, and cancer are eligible.

If you fill an eligible prescription, SaveOnSP will contact you to complete enrollment. By participating, you will pay a reduced cost for your medication; if you choose not to enroll, you will pay a higher coinsurance.

MAIL ORDER PROGRAM EXPRESS SCRIPTS Smart90 Program

Filling Maintenance Medications by Mail Order

Maintenance medications are provided through the Smart90 Program. Smart90 is a feature of your prescription drug benefit, managed by Express Scripts

•

If you are enrolled in the Middle Plan or the High Plan, you can receive up to a 90-day (3-month) supply of medication for the cost of two retail copays.

•

If you are enrolled in the Low Plan, you can receive up to a 90-day supply. You are responsible for the cost of the medication prior to meeting your Tier 1 deductible.

Once your Tier 1 deductible is met, you will pay 30% coinsurance until you reach your out-of-pocket maximum for the plan year. Members may receive greater discounts through mail order compared to purchasing at a retail pharmacy.

Members have two ways to get up to a 90-day supply of long term maintenance medications (drugs you take regularly for ongoing conditions). You can conveniently fill those prescriptions either through Express Scripts’ home delivery or through a participating 90-day retail pharmacy such as CVS or Walgreens.

Contact your doctor to write a prescription for up to a 90-day supply. This is the fastest way to get your medication filled!

Express Scripts will make outreach to your provider.

Once you log in, click “Request an Rx” on the right side of the home page, follow the instructions, and an Express Scripts representative will reach out to your provider.

HEALTH SAVINGS ACCOUNT INSPIRA FINANCIAL The Health Savings Account (HSA) is available only to eligible employees who are enrolled in the Low Plan and meet all other HSA eligibility requirements.

HSAs are known for their triple-tax advantage.

Contributions are made pre-tax, growth is tax-free, and withdrawals used for qualified health-care expenses are also untaxed. When you elect the HSA, you will be issued a debit card from Inspira Financial. If you are currently enrolled in the Healthcare FSA and wish to participate in the HSA, you must be sure to have used all FSA funds prior to beginning your HSA contribution.

HSA Eligibility To be eligible for an HSA, you: •

Must have coverage under an HSA-qualified HDHP (Air Group’s Low Plan)

•

Cannot have other first-dollar medical coverage (i.e., policy with no deductible)

•

Cannot be enrolled in Medicare

•

Cannot be claimed as a dependent on someone else’s tax return

HSA Advantages •

The HSA is portable, meaning you can take your funds with you if you leave the organization

•

There is no “use-or-lose-it” provision - unused money stays in your account and earns interest on a taxdeferred basis

•

The HSA includes a banking partner that offers several investment options. Third-party substantiation for transactions is not required, but you should keep records in case of an IRS audit.

You can use HSA funds tax-free for qualified medical expenses, including future or retiree health-related costs

Contributions to your HSA are limited by the IRS. Once you are enrolled in Medicare, contributions to the account must stop. However, you can keep the money in your account and use it for eligible expenses tax-free.

Air Group’s HSA Contributions If you are enrolled in the Low Plan, Air Group will contribute $500 for employee-only coverage or $1,000 for all other tiers to your HSA for the 2026-2027 plan year.

Air Group’s contributions ($500 / $1,000) are split into two installments taking place the first pay periods in June and December. The employer HSA contribution will be pro-rated for those hired after the start of the June 1st plan year.

FLEXIBLE SPENDING ACCOUNTS WEX Air Group provides eligible employees with the opportunity to pay for out-of-pocket medical, dental, vision, and dependent care expenses with pre-tax dollars through a Flexible Spending Account (FSA). Those who elect to participate in the Healthcare FSA will receive a debit card from WEX.

How Much Should I Contribute?

Dependent Care FSA

You should contribute the amount of money you expect to pay out-of-pocket for eligible expenses for the plan period.

If you do not use the money you contributed, it will not be refunded to you or carried forward to a future plan year.

This is known as the IRS “use-it-or-lose-it” rule.

The Dependent Care FSA is used to reimburse expenses related to the care of eligible dependents. A qualifying “dependent” may be a child under age 13, a disabled spouse, or an older parent in eldercare.

To contribute to the FSA, you must make your election in ADP Workforce Now annually.

Healthcare FSA The Healthcare FSA is used to reimburse out-of-pocket medical expenses incurred by you and your eligible dependents. The maximum amount you can contribute to the Healthcare FSA is $3,400.

Eligible expenses include: •

Doctor office copays

Non-cosmetic dental procedures (i.e., crowns, dentures, orthodontics)

•

Prescription contact lenses, glasses, and sunglasses

•

LASIK eye surgery

•

Over-the-counter medications and feminine care products

Important: You cannot elect the Healthcare FSA if you elect to participate in the HSA.

Eligible employees may contribute up to $7,500 per year ($3,750 if married and filing separately).

FSA Grace and Runout Periods These rules apply to both the Healthcare and Dependent Care FSA. Unused funds or unsubmitted claims after 90 days are forfeited.
