CMHW/Practice Nurse/GP provide all women with a copy of “Understanding your options for Maternity Care” booklet from CMH

SEM to contact Family Health Practice to inform they are the LMC

Provide woman with a list of Self Employed Midwives – and a copy of the “Looking for a Self Employed Midwife in the CMH area” booklet

Family Health Practice makes ‘back up’ referral to CMH with woman’s consent detailing woman’s choice of maternity care provider

Ereferral/fax all woman’s details including two updated contact telephone numbers and info on clinical assessment to Maternity Administration on 09 276 0055

Maternity Admin process: - woman’s medical and obstetric history reviewed - determine if woman’s choice of maternity care provider appropriate - referral to self employed midwife process commenced
