CARING FOR OUR NEIGHBORS In 1992, Dr. Nadkarni and Dr. DeMarco had a vision— a vision of a volunteer-driven health clinic that provided free, high-quality primary medical care for the working uninsured. Paul DeMarco was chief resident and Mohan Nadkarni was a third-year resident in internal medicine at the University of Virginia. A fellow medical student, who was Mohan Nadkarni (L) and Paul DeMarco, (R) MDs, uninsured, had fever and aches that would not go away. Free Clinic Founders Eventually, the student needed major heart surgery and valve replacement. The student’s condition could have been treated with a dose of antibiotics if it had been caught in time. Dr. Nadkarni still sees patients each month during evenings at the Free Clinic. The love a provider has for his patients has fueled his passion to care for those who do not have access to healthcare. This love extends his days as a medical provider into the evenings. Not long ago Dr. DeMarco wrote a message to supporters of the Free Clinic: “The idea of the clinic is so basic and powerful that it speaks for itself … Very few words are necessary—only the understanding between you and the donor of the eternal truth that we are all better off if we care for our neighbors.” Each one of you reading this Annual Report has taken care of our neighbors. BUILDING A HEALTHY COMMUNITY TOGETHER A COMMUNITY RICH IN HEALTHCARE SHARES RESOURCES TO CARE FOR 20,000 NEIGHBORS OVER THE LAST 25 YEARS. A SIGNIFICANT NUMBER STILL NEED ACCESS Our area has approximately 12,660 uninsured individuals who earn incomes high enough to qualify for Free Clinic services. Last year, we cared for a total of 2,913 patients. This means that there are almost 10,000 community members who are working, uninsured, and still need access to health care. When the Affordable Care Act passed in 2010, we anticipated that the need for our services would diminish. We were wrong. When the health insurance marketplace opened in 2014, we provided insurance counseling to support our patients in enrolling through the health insurance exchange. Many purchased affordable plans, but with such high deductibles that they could no longer afford their medications.

Over the years, our volunteer medical professionals saw a growing number of patients with long-term, chronic conditions. We have spent the past 25 years increasing our primary medical care, expanding our pharmacy formulary, adding access to specialty medical care, engaging community wellness providers, and introducing acute dental care services.

But in the most fundamental way, we have not changed at all. We are still a collective effort of world class health care providers who volunteer, and a generous community willing to create access to care for our neighbors. Through community partnerships, we extend our patients’ care. Both the University of Virginia Health System and Sentara Martha Jefferson Hospital (SMJH) donate free lab services every month. Sentara also supplies free radiology services throughout the year. A medication access worker from United Way, with support from the Virginia Health Care Foundation, obtains more than $3 million dollars in donated pharmaceuticals through patient assistance programs. Local community practices offer patients comprehensive diagnostic testing and extended care at very nominal fees. The Thomas Jefferson Health District–Virginia Department of Health provides vaccines.

HEALTH, WORK AND SCHOOL The Free Clinic supports our local workforce by providing access to primary care. This year, our partnership with the Charlottesville Works Initiative works to connect individuals in workforce training with healthcare. A regular place for primary care helps our patients maintain employment and lead healthy lives.

INTEGRATING PRIMARY CARE BUILDING PRIMARY CARE WHICH INCLUDES MENTAL HEALTH SUPPORT, ACCESS TO SPECIALISTS, AND WELLNESS PROGRAMS. In our own community, 17% of adults do not have a primary care doctor*. According to Healthy People 2020, individuals with a usual source of care enjoy healthier lives and have lower health expenses. That’s why the Charlottesville Free Clinic was founded, and why we exist today—to provide a “regular place” for primary health care for our neighbors who do not have access to mainstream healthcare. We are their lifeline to health. Without insurance, you are less likely to have a primary care doctor and more likely to have adverse health outcomes. 92% of the Free Clinic’s clients remain uninsured.

74% of our patients have chronic illnesses. CFC’s primary care clinic helps our neighbors manage chronic diseases so they can live healthier lives. Our integrated medical clinic provides access to primary care—whether it’s medical care, mental health support, medication, or specialty care.

One of the largest gaps in healthcare is the separation of medical and mental health care. By bringing these services together under one roof, we treat the whole person. Twenty percent of our medical patients benefitted from mental health support in the past year.

Beyond primary care, we provide access to a broad range of medical specialties on site during evening clinics. Volunteers provided 612 specialty care visits, 12% of total evening visits. 45% of our patients last year were referred to partnering community practices, who delivered a total of 858 extended care visits. In addition, we embrace community wellness programs on site, including twice-a-year Quit Smoking Charlottesville in partnership with SMJH, Thomas Jefferson Health District, and UVA Health Systems, weekly acupuncture sessions through Central Community Acupuncture, and fresh vegetable distribution through our Local Food Hub.

insurance counselor, but there was not a plan I could afford that would cover the costs of my insulin.” With CFC’s help, Rabia has been able to control her diabetes, making it possible for her to work part time—including walking dogs. With her diabetes controlled, Rabia is able to earn enough money to become a donor—contributing a donation at the pharmacy every time she picks up medications.

DENTAL CARE IS PRIMARY CARE FIFTEEN YEARS AGO, THE U.S. SURGEON GENERAL IDENTIFIED ORAL DISEASE AS A PRIMARY HEALTH CONCERN, CALLING IT “A SILENT EPIDEMIC.” Good oral health is essential to healthy lives and a healthy community. Individuals with periodontal disease are about twice as likely to suffer a fatal heart attack and three times more likely to suffer from a stroke. Oral diseases can negatively affect those with diabetes and can cause adverse pregnancy outcomes. The Charlottesville Free Clinic first took on the challenge of creating a Community Dental Program in 2007 to meet high demands for dental care in our community. Every year, the Free Clinic continues to increase the size and scope of our dental clinic to meet high demands for dental care in our community. Our response to the continued, dramatic community need for access to dental care includes Walk In Clinics, new this year, offered three mornings a week from 8 a.m. to 1 p.m. Walk In Clinics allow us to see more than twice the number of patients in a morning, and to treat those with the most acute needs—extractions and infections—first.

Hygiene Mission Days, modeled after local Mission of Mercy Clinics, provide expanded access to hygiene and education for our community. Dental hygiene, cleaning and education comprised 40% of visits this year. We’ve also expanded our Community Dental Program, referring patients to 34 local dental practices who donate their time and materials for fillings and oral surgery. A partnership with the Neighborhood Family Health Center, a federally supported community health clinic, provided dental services to community members who would otherwise have no access to dental care.

This year, the Charlottesville Free Clinic treated 1,816 dental patients through 4,547 visits—a 27% increase over last year’s dental patient visits. Restorative Mission Day, an all-day event in a community dental office, created access to restorative care such as fillings, which we can provide on only a limited basis. Our vision for the future is integrated dental and medical care. “Oral health is a primary care homerun,” says Judith Haber, NYU College of Nursing. We’re already innovators by housing dental and medical services under one roof. By linking these services more strongly, we create greater access to primary care, which includes dental care.

Hygiene Mission Days at the Free Clinic help patients catch problems early, and educate our neighbors about the crucial role of hygiene in oral and overall health. We’re able to do this because the Virginia Health Care Foundation supported the expansion of our hygiene care.

Medicare does not cover dental care. “Dental is the front line, the main deal,” he says. Hari had his teeth cleaned at the Free Clinic Hygiene Mission Day last year. He also had infected gums, and needed a tooth pulled. “I think we should treat our teeth like pearls or diamonds—they are that valuable,” Hari now says.

PROVIDING DAILY MEDICATIONS A MAYO CLINIC STUDY SHOWS THAT SEVEN OUT OF TEN AMERICANS TAKE AT LEAST ONE PRESCRIPTION DRUG—AND THAT MORE THAN HALF OF AMERICANS TAKE TWO OR MORE. Last year we dispensed 30,374 free prescriptions with an estimated retail value of $3.1 million.

Donations made it possible—individual patient assistance programs through United Way and the Virginia Health Care Foundation, bulk donations from pharmaceutical companies, plus generic medications we purchased at a significant discount, supported by donors and patient contributions.

Declining donations create challenges for our pharmacy. The number of donated medicines has drastically declined. In 2012, we received donations of seven medications from one company. This year, the same pharmaceutical company donated only two medications. Four of the top ten prescription drugs in the U.S. have also increased in price more than 100% since 2011. In 2016, Humulin, an insulin drug we purchase, cost CFC $13 a vial. This spring, our cost increased to $142 a vial. While we continue to meet our patients’ needs for prescription drugs, this “new normal” makes our work more challenging.

“One year ago, Dr. Paul DeMarco met a part-time graduate student who had a fever and aches that wouldn’t go away. Because the young man had no health insurance and was living on a tight budget, he had just kept hoping—for two months—that his annoying symptoms would disappear. But they got worse. Eventually, the student was persuaded to see a physician. The diagnosis was endocarditis (inflammation of the heart’s membrane). The young man had to have major heart surgery in the form of a valve replacement. A typical CFC medical patient is an adult who lives in Charlottesville or Albemarle, is between 36 and 64 years old, and is employed full or part time. He or she earns between $12,060-$24,210 a year. In terms of ethnicity, half our patients are Caucasian, 20% are Black, 20% are Latino, and 7% are Asian.
