GUARDING THE FLAME For 24 years, the vision of the Charlottesville Free Clinic (CFC) has been to serve as an open door to health care for our neighbors in need. This would not be possible without the amazing commitment of our medical providers who serve our patients with compassion and the “lay” volunteers who make sure that patients are welcomed; medical folders are filed; and patients receive friendly reminder calls about upcoming appointments. This combination of expertise, skill, experience and commitment by our volunteers powers the mission of CFC to be the unique community health care provider that it is today.

FROM THE EXECUTIVE DIRECTOR AND BOARD CHAIR OF THE CHARLOTTESVILLE FREE CLINIC

For our annual report this year, we looked back to our founding ideals. From the opening of our first clinic in a garage on West Main Street in 1992, we have been inspired by volunteers. Twenty-four years later, we continue providing inspired volunteer health care. The continued engagement of volunteer providers powers the mission of the Charlottesville Free Clinic. We are a community rich in health care resources. The willingness of local providers to offer care to Free Clinic patients during our evening clinics on Rose Hill Drive ensures that these valuable heath care resources are shared with our neighbors. The Charlottesville Free Clinic is a compassionate, high-quality, cost-effective health care safety net. In this annual report, you will find photographs and reflections from a few of the 669 volunteers who cared for our patients last year. Together, they donated 14,703 hours of patient care. We are powered by their good will, their deep compassion and their excellent patient care. Every evening clinic, they provide tangible evidence of our generous and caring community. Our clinics offer access to primary care. At the Free Clinic, primary care includes medical care, dental care, mental health services, pharmacy and insurance counseling. For community members with low incomes who remain uninsured — or have insurance with extraordinarily high deductibles — these services are free. Our volunteers improve the health of community members who do not have access to primary care. And the work and resources necessary to provide care for 24 years is not limited to our volunteers. Our staff is an extraordinary group of individuals committed to supporting the mission our medical volunteers execute during every evening clinic. Our donors provide resources that can’t be generated by the giving of someone’s time. Together we have fueled the delivery of free care for our underserved neighbors with both passion and commitment. Through our commitment to care, we improve the health of our neighbors. Inspired by volunteers, the Free Clinic was founded to share the resources of health care providers’ time, training and compassion. This extraordinary commitment to giving generates an equally extraordinary sense of gratitude for our volunteers. Their motives and actions continue to inspire us year after year. WHO WE HELP The Working Uninsured and Underinsured All CFC clinic services are free to our patients. A patient of CFC has an individual income between $11,800 and $47,500, and the majority of individuals are uninsured. After the passage of the Affordable Care Act, about 14% of our patients were able to enroll in insurance plans. While this is progress, many neighbors remain uninsured, and for many who purchased new plans, their deductibles are extraordinarily high. Today, CFC serves both the uninsured and those who are considered underinsured — those for whom high premiums and deductibles prevent access to primary care and necessary medications.

HOW WE HELP Whole Person Primary Care CFC provides primary health care services through identifying and addressing the medical, dental, mental health and pharmaceutical needs of our patients. An integrated care approach enables us to be intentional in developing collaborative treatment plans between primary medical care and mental health providers, planning referrals to medical specialists in the community and providing access to the CFC pharmacy. This holistic approach allows us to provide the best possible care for our patients, who otherwise would not have access to primary health care.

PRIMARY MEDICAL CARE CFC patients come to us to receive quality health care for everything from the flu, high blood pressure and diabetes, to depression and anxiety. These are medical concerns that any person would seek care by a primary care physician. For our patients, the CFC is their primary care provider. Our medical services are made possible by volunteer doctors, nurses and other medical professionals who are dedicated to providing high-quality care to our patients during evening clinics, three times a week. Evening hours are crucial because many of our patients work during the day and are unable to take time off for medical care. Some of our providers have been part of the CFC volunteer family of care for close to 20 years. Primary care access is also offered in the community through CFC’s Physician Partners Network (PPN). This collaborative agreement with local medical practices allows CFC patients to see area physicians who agree to have CFC patients come to their respective offices on an ongoing basis for primary care. As CFC patients, they are eligible for free medications from the CFC Pharmacy. PPN also provides a unique opportunity for physicians to “volunteer” for CFC from within their practices. This collaboration is a win-win for all involved. CFC also provides Wellness & Education programs:

• Smoking cessation classes through Quit Smoking Charlottesville

• FreshFarmacy for fresh produce distribution • Acupuncture clinics • Diabetes education As part of our mission, we continue to advocate for greater access to health care for the underserved. The CFC insurance counselor works one-on-one with every patient to navigate the insurance exchanges for an affordable insurance plan. We assist our patients who transition to Medicare to find the optimum coverage for the medications that they need to maintain good health. In the cases where patients are not eligible, we refer them to another door in the community.

SPECIALTY HEALTH CARE Beyond primary care, we offer patients access to a broad range of medical specialties during evening clinics which are scheduled regularly throughout the year. We also refer patients to specialists in the community as needed. Other specialists, such as gastroenterologists and breast surgeons, are available through our expanded care agreements which provide vital care at no cost or greatly reduced cost to CFC patients. Specialty care provided on-site during evening clinics is always free. We are especially grateful and proud of the depth and breadth of access to these specialists that vastly improves the quality of life for our patients, many of whom may not see a specialist otherwise.

MENTAL HEALTH CARE Again this year, depression and anxiety are in the top five medical diagnoses for community members who arrive at CFC’s Integrated Care Medical Clinic. A large percentage of our patients have chronic illnesses and many of them also end up struggling with depression and anxiety. Integrated care insures that the patient’s mental health concerns are addressed in tandem with their medical issues. All CFC patients are screened for depression and anxiety, and offered mental health support if appropriate. Between our staff Psychiatric Nurse Practitioner and 19 volunteer mental health professionals, our patients are able to participate in care plans that define wellness as a whole — body and mind. Our integrated primary care approach allows patients access to mental health services just down the hall from the medical clinic. It is this “warm hand-off’ which helps connect individuals with access to mental health care. We know that depression and anxiety frequently occur with the challenges of low income employment and also with chronic disease. Our integrated primary care model provides crucial access to psychiatry, counseling and medication management to low income community members. We also partner with the following community institutions to extend our reach for client advocacy, education and referrals as part of a cohesive mental health safety net for the community: • Community Mental Health and Wellness Coalition • Partners for Mental Health • ReadyKids • Region 10 refers patients to CFC • Shelter for Help in Emergency (SHE) • The Women’s Initiative refers patients to CFC

PHARMACY HEALTH CARE In addition to receiving more than $4,000,000 worth of donated medications, CFC also invested $66,000 to purchase additional medications at substantially reduced costs through partnerships with national and local pharmaceutical entities. These partnerships enable CFC to purchase at low-cost medications that we would be unable to stock on a regular basis. The cost to our patients for all medications: zero. Our patients can have as many as five or more different diagnosed conditions, many of which are chronic. Our pharmacy works closely with medical providers to monitor potential drug interactions, and makes sure that medications are taken as directed by all patients. Our Clinical Pharmacy, made possible through a partnership with Kroger Pharmacy, provides medication management and helps patients understand the vital role of lifestyles and managing medications in maintaining good health and controlling chronic diseases.

DENTAL HEALTH CARE Dental health care is an essential component of primary care. The oral health of a person can significantly impact overall health and quality of life. The focus of the CFC Dental Clinic is to provide basic oral health care: relieving pain and arresting infection. Alongside our staff and the volunteers who work on-site, 25 community dental practices accept CFC patients by caring for them in their offices at no charge to the patient through our Community Dental Program.

DENTAL CLINIC EXPANSION CFC’s Dental Clinic opened in 2007 as a community dental program. Volunteers opened clinics in the evenings and we offered a weekend Mission of Mercy clinic. In 2008, CFC partnered with the Charlottesville/Albemarle Health Department to share a dentist. In 2009, CFC hired a full-time dentist and two dental assistants. Today, we have a dentist, a hygienist and dental assistants to see patients by appointment and via walk-in clinics. Completed with the Help of Volunteers This summer, CFC renovated the Dental Clinic, converting an old patient chart room into a reception area and a storage room into a fourth operatory. This renovation was completed with the contracting help of Martin Horn, Inc., painting services by former CFC patients Rudy and Veronica Salvatierra, and architectural design guidance from Brian Williams, Assoc. AIA. We are truly grateful for their generosity of time, expertise and service to help expand the Dental Clinic space for a modest investment. A Fourth Operatory: Expanded Dental Care with Hygiene A fourth dental operatory significantly increases the number of patients we can see, the engagement of volunteer dentists and our ability to offer hygiene services. We are better positioned to develop partnerships such as the one we established with the new federal clinic in Charlottesville, the Neighborhood Family Health Center, to provide dental care for their patients. The expanded space also enabled us to open our doors for walk-in dental clinics three mornings each week.

CFC STAFF VOLUNTEERISM Mobile Dental Clinics in Wise County, Virginia This July, CFC staff traveled to Wise, Virginia, to support a Mission of Mercy (MOM) mobile dental clinic. “A lot of folks spend the night in the parking lot, and then they’re waiting in line in the wee hours of the morning,” said Dr. Andrew Vorona, CFC Staff Dentist. One memorable patient recalled by Jamie Davila, CFC Staff Hygienist, concerned a 17-year-old boy who drove four-and-a-half hours to Wise so he could see a dentist about his advanced gum disease. He had not received dental or medical care for the past seven years since his father abandoned the family. Jamie spent an hour-and-a-half cleaning the patient’s teeth on one side and encouraged him to return the next day to complete treatment for the other side. “The very next day, he showed up with a huge smile on his face, tears in his eyes, and he said, ‘I can’t believe it. This whole side of my face feels so good. My teeth are really white, and they’re straight, and they’re nice.’ I’m glad he came to my chair.”

PHILANTHROPY At the Charlottesville Free Clinic, we believe that quality health care is a basic need that should be met for every member of our community. Last year, 1,159 donors gave wholeheartedly to help continue our mission to provide access to health care for the underserved. Inspired volunteers provide quality care for our patients. Sustaining contributions from donors in this community support a volunteer community health care network. Their inspired giving goes hand-in-hand with inspired volunteering at the Charlottesville Free Clinic.

CFC BENEFIT CONCERT An Evening with Emmylou Harris and Lyle Lovett & His Large Band Thursday, August 11, 2016 | Sprint Pavilion Emmylou Harris and Lyle Lovett & His Large Band headlined our 13th Annual Benefit Concert at the Sprint Pavilion. We hosted our pre-concert Sponsor’s Party at the Paramount Theater with food provided by some of the hottest restaurants on the Downtown Mall.
