I hope you will find it a useful publication telling you about the support choices available across the county, either for yourself, a member of your family or friend, or someone you care for.

We know that making decisions about help, support and care can be daunting and stressful, which is why we are here to help you each step of the way. This Directory runs alongside www.mychoicemycare.org.uk and our telephone helpline (01522 782155) in providing you with clear, easy-to-understand information and assistance.

Working closely with families and carers to shape this support, we know you want to be confident that you’re making informed decisions so you can make the right choices for yourself or your loved one. We know you want effective, high-quality care in Lincolnshire and, as a resident, you deserve nothing less than a positive and safe experience. You deserve to be treated with dignity and respect – and for every aspect of your care to consider you as an individual, whose own circumstances and needs are taken into account.

So these are our main priorities for you. Because you also deserve care which isn’t off the shelf, piecemeal or uncoordinated between care providers but instead tailored to you and your needs, helping you to stay independent in your own home for longer and helping you bounce back more quickly from any setbacks. You may have heard terms such as ‘personalisation’, news about plans to provide greater choice of care services and proposals for joining up Adult Care and Public Health.

You’ll hear more in the near future too as each will play its role in changing how care services are provided for the better.

Introduction How to use this Care Services Directory The articles in this Care Services Directory have been compiled with contributions from Adult Care of Lincolnshire. When the words ‘we’ and ‘us’ are used in the Directory it means Adult Care.

Many people will use this Care Services Directory looking for advice and information about adult social care. The Directory aims to give an overview of all aspects of adult care to everyone who is looking for support.

Thinking about the care or support either you or someone else in your life will need can be daunting and making any change in your life will require making decisions. We try and provide the basic information and guidance you will need about adult care in this Directory, and give you the details of other organisations that can help. There is a lot of information in this Directory, and, depending on your circumstances, some parts will be of interest to you and others won’t. We have tried to think about who will use this Directory and have set out below the parts that we think will be of most interest to you following the same major themes on our website www.mychoicemycare.org.uk.

There is a wide range of information on My Choice My Care which will help you or the person you care for find the support and services you require to help you lead a better quality of life. If you need help and don’t know where to start; you can complete a short assessment and someone will contact you. If you are a carer looking for support, or you have received a personal budget and want some help with the next steps; there are providers that can help. You can consider a range of options that may be able to help and you can find out about providers of different services and contact them directly.

There is a section where you can find out about financial costs involved in paying for your care needs, and how much we will fund towards this. Using the new financial calculator you will easily and quickly be able to find the information you need to help you make your own decisions about where to go next.

Quality Assurance of Care and Support Care and support for adults in Lincolnshire is delivered by a variety of public, voluntary and private organisations.

Because of this diversity, there are also different organisations Lincolnshire County Council - Adult Care Adult Care delivers and commissions care and support, and works in partnership with others to deliver services. Adult Care offers an assessment of your needs and if you meet eligibility criteria, can offer a range of care and support services to help you stay safe and well. Adult Care is committed to assuring the quality of its services. Our ‘Quality Circle’, describes what contributes to a good quality services. A good quality service needs to include elements of all three sections shown in the quality circle ,for example a service is not considered good quality if it is safe but your experience is poor.

The Care Quality Commission The Care Quality Commission make sure that the care provided by hospitals, dentists, ambulances, care homes and home-care agencies meets government standards of quality and safety. They register health and adult social care services across England and inspect them to check whether standards are being met. You’re in charge There are a number of services available to support you if you are experiencing poor health. They can prevent you from going to hospital, or offer extra care and support after you

have been discharged. Whatever your level of involvement with us we want you to be assured that your needs and wishes will be paramount throughout the process.

Personalised care and support Lincolnshire County Council and other local authorities have made changes to the provision of care and support to older people and people with learning and/or physical disabilities. This is a national requirement that sets out the way people and those who care for them should be supported.

In the traditional system of social care, professionals made many of the decisions about the support people could have

and who would provide it. As a result, people often had limited choice about the services they might receive.

A system called ‘Self-Directed Support’ seeks to replace this old approach of prescribed care, by putting people in control of the support they need to live the life they choose. It recognises that some people can manage their support on their own whilst others need help – from family or friends or people who are paid to help.

Self-Directed Support How does it work?

If you or someone you know or care for are eligible for social care, Self-Directed Support allows you to have more choice and control over your life as you are able to make decisions about the support you need. It asks how you want to live your life and what support will best enable you to do this.

Support can be given if you are a carer to help you continue your caring role and maintain a life outside of caring.

If you or the person you care for are eligible for social care support, Self-Directed Support gives you a Personal Budget based on an assessment of your eligible social care needs. You may need support or help to manage

everyday tasks such as washing and dressing, help with eating and drinking or getting out into the community.

You can be as creative as you want, as long as the money is spent to meet your needs.

Eligibility for care services We use guidance from the government to decide which people are eligible to receive help funded by us. Everyone who comes to us to request help will have an assessment to see if they meet the eligibility criteria – previously known as Fair Access to Care Services. Sometimes these assessments will be over the phone where people have less significant difficulties and sometimes they are carried out face to face.

We provide services to people with ‘substantial’ and ‘critical’ levels of need. People who do not meet the eligibility

criteria will receive information and advice about how their needs can be met through services available to all in the community such as those available through voluntary agencies or private organisations.

You’re in charge continued Personal budgets We have now introduced personal budgets across the whole of Lincolnshire for all eligible users of social care services following assessment. They bring flexibility and transparency of funding to the provision of social care services.

A personal budget is an amount of money that is allocated according to your needs, to be spent on services of your choice. At the time of going to press (November 2013) in Lincolnshire, you can choose to receive parts of your personal budget, or your entire personal budget, in various ways: • a Direct Payment (DP) – Lincolnshire County Council pays you an amount of money. The money can then be

spent on solutions and services of your choice to meet your eligible social care needs, and you are in control of this money; • If someone is not able to consent to a Direct Payment, but where a Direct Payment is deemed to be in their best interest it can be paid to a person who is suitable through a third party agreement.

• A Managed Budget - you know how much money is available to you and you still have choice and control to choose but Lincolnshire County Council will arrange and pay for your support on your behalf.

Direct Payments Direct Payments are financial payments made to people who have been assessed as eligible to receive support from Lincolnshire County Council. We can make a Direct Payment to most people who need our support, including: • people who have been assessed as needing community care services (including those with Mental Health needs) aged 16 and over, incorporating short as well as longer term needs; • carers over 16 for carer’s services; and • parents of disabled children, for children’s services.

Why would I want Direct Payments?

Direct Payments allow more control over the decisions that affect your life. They provide more flexibility and choice, as you can buy appropriate support tailored to your individual needs. Another option is for us to source a portion of the care/support needs through services commissioned directly by Lincolnshire County Council and also provide a Direct Payment to arrange those remaining services that you wish to have more control over.

What can Direct Payments be used for?

If you receive a Direct Payment, the money is used to arrange support to meet eligible needs. You may choose to employ someone directly, to buy services from an

independent or voluntary sector provider or purchase equipment to help live more independently. With Direct Payments, people or agencies employed are accountable to you and not to Lincolnshire County Council. Therefore, Direct Payments enable more control, choice and flexibility over how you plan your support to fit with your life.

It is important that your plans meet your assessed needs.

Therefore, provided the money is used lawfully, and eligible needs are met, the flexibility that these payments allow means it is impracticable to outline what the money can be spent on. However, there are certain items on which these payments cannot be spent, for instance: • on services that should be provided by Health; and • on routine living expenditures: utility and household bills for instance.

Most Direct Payments are made to meet regular ongoing support needs, however they can also be made as a single payment, for example, to purchase equipment or a short respite break.

What do I do next?

What care is available?

This section is about the guidance, support and care that we can provide. It describes all parts of the social care process, from someone who might be leaving hospital, worried about how they are going to cope at home, to advice about specialist care for someone who might have

very complex needs but wants to stay at home. Some of these sections may not be relevant to you, others may be of more interest. Feel free to go straight to the section that interests you, or browse through the whole section to give yourself a full flavour of the support available.

Help after leaving hospital Adult Care and the NHS in Lincolnshire work closely together to ensure that if you are a hospital patient, you are discharged safely and are able to return to home life as quickly as possible.

Our staff are available to help with information and advice when you are ready to leave hospital.

No matter what hospital you are in, you can request an assessment of your needs to help you stay independent and confident when you return home. A social care worker will come and speak to you to arrange an assessment which can either be done whilst you are still in hospital, or once you return home.

The assessment involves asking you about your needs and what is required to help you live safely and independently at home. There will also be an explanation of any charges that may be made.

A relative, carer or a member of the hospital staff may also approach the hospital social care team and ask them to carry out an assessment, but this will not go ahead unless you agree.

If you already have a social care service in place we will make an assessment of any other needs you may have after your stay in hospital.

If you are a carer, you may also need help and support and should also be offered an assessment. We can also introduce you to local voluntary organisations who can help, with a variety of services such as laundry and domestic cleaning.

Reablement What is reablement and how can it help you?

Reablement is about giving you the opportunity and confidence to relearn and regain some of the skills you may have lost because of poor health, disability, following a spell in hospital or problems at home. After events like this you can lose your confidence or worry about your ability to cope at home again. Reablement is a very personalised approach – the kind of support given is tailored to your individual needs. It focuses as much on your emotional and social needs as on your medical needs.

Reablement provides support for a short time to help you become as independent as possible. You may need support with aspects of day to day living such as: • Personal care • Preparing meals • Shopping • Teaching and practicing exercises • Ensuring a safe home environment • Finding practical solutions to support your independence.

What care is available? continued Reablement continued • Promoting community activity and social integration • Supporting you in your own home • Keeping and improving your independence and safety in areas of daily life • Improve your quality of life • Reduce the need for ongoing care and support How does the reablement service work?

Following a referral to the service, a member of the reablement team will visit you to discuss your needs. A member of the team will be your ‘Key Worker’ who will be your contact throughout your reablement journey.

Together we will agree what goals we are working to achieve. Your goals are written down in a reablement care

plan. Reablement support will take place in your own home.

Your key worker will arrange for your care to be delivered by the reablement support workers. They will review your progress towards your goals, oversee your care and agree the right time for discharge from the service.

What happens when your reablement ends?

At the end of reablement your key worker will assess your ongoing needs. If you still need continued support we will discuss this with you and if appropriate, refer you for an Adult Care Assessment.

Assessment If you have been through a period of reablement, and require ongoing support, we will work with you to assess your needs. Even if you haven’t been through the reablement process we can provide you with an assessment.

An assessment is simply a discussion with you that will help us understand what help and support you might need coping with life on a day to day basis and living independently in your own home.

Where does it take place?

It usually involves a chat over the phone, or in your own home and we’re very happy if you have a friend, relative or carer with you. If you are in hospital, we’ll come and talk to you before you are discharged to see about any help you might need in the home whilst you are recovering. The assessment is a two way conversation. We’ll ask you to tell us something about yourself and you can tell us how you think we can help and support you. We might also ask the opinion of other professionals who know and work with you, like your doctor, but we will only do this with your permission. We will also take account of the needs of your family or carer and assess them separately if they ask us to.

Remember we are only coming to see you to understand how you are coping day to day and how we might help to improve your quality of life.

How quickly will this happen?

If your situation is very urgent and you are in a crisis or at serious risk of harm, we will aim to see you within

24 hours. If your carer is finding it very difficult to cope, we will try to see you within seven days. In any other circumstances it will be within 28 days of you asking for the assessment.

What about confidentiality?

We respect your privacy at all times. Any information you give us will be treated as confidential and will only be shared with anybody else involved in your care with your permission - other than in exceptional circumstances where the law says we must show it to someone else.

Will I be able to get help?

After the assessment we will identify and agree your needs with you and discuss the help and support available to meet them, according to the seriousness of your situation. There may be a charge for services; the amount will depend on your capital and income. We will always give you full information before a service is provided.

If you are eligible for help from Adult Care we will aim to arrange this within seven days of the assessment being carried out. If you are not eligible for help, we will still give you information and advice on other services that are available to help you to continue to live independently and to maintain your quality of life.

If you are not happy with the outcome of the assessment you can ask us to look at your situation again with you and if you are still not happy you can get in touch with us to make a comment, suggestion or complaint.

Living at home There are many different forms of support that can enable you to remain in your home for as long as possible ranging from equipment to people who can come in to support you in your own home. Starting with First Contact below, there are many ways in which you can

help yourself if you are living at home. Be it accessing the support you need directly yourself on www.mychoicemycare.org.uk or through the First Contact checklist and contacts below.

First Contact If you are aged 60 and over you can now get in touch with a range of essential services through one point of contact to help you to stay safe and well in your own home.

The First Contact service, which is run in partnership by a range of organisations across Lincolnshire, works by encouraging people to complete a checklist in a First Contact leaflet.

The checklist addresses issues such as fire and community safety, personal care, housing, heating, falls prevention, money and benefit entitlements, transport, social isolation, home care and caring for others.

Organisations that provide help through First Contact include:

• Adult Care • Lincolnshire Fire and Rescue • District Council housing teams • Age UK • Lincolnshire Police • The Pension Service • Volunteer centres.

First Contact leaflets are widely available in the community from places such as libraries, local Community Access Points and GP surgeries. Living at home continued A Guide to TeleCare in Lincolnshire What is TeleCare?

TeleCare is a service that helps people to remain independent and feel safer in their own home.

TeleCare helps reassure an individual that if they do need help, a member of their family, a carer or the emergency services will be contacted. TeleCare can be used to support older people, those with long term conditions, disabilities or mental health worries.

TeleCare can tell when there is an emergency in someone’s home and when they need assistance.

TeleCare uses a range of sensors either worn by the individual or placed in their home that can alert the local monitoring centre 24 hours a day.

Who can TeleCare help?

TeleCare can help people who: • are at risk of falls; • are vulnerable due to reduced mobility or a disability; • have a carer who may need support; • have memory problems including dementia; • have long term health conditions such as epilepsy; • have a learning disability and want to be more independent; • are starting to leave their home at inappropriate times; or • have recently been discharged from hospital.

Case study - Derek’s story Derek has a sensory impairment - he was born profoundly deaf and struggles communicating with others to make his needs known. He uses an interpreter for all communication. Derek’s social worker worked in partnership with him and his deaf support worker to help achieve goals and outcomes to maximise his potential to remain living in the community independently.

Derek was referred for TeleCare equipment, not knowing a lot about the service he didn’t know what to expect but knew he wanted to continue to live in ‘his home’. He described himself as being frightened all the time, afraid of the dark and anything happening, i.e. a stranger getting into his home, not being aware if there was a fire, nobody knowing if he had fallen and being alone often day and night. His disability now seemed to compound further problems which seemed to prevent him from living alone.

TeleCare services appeared to be a positive way forward to enable Derek to embrace the many challenges knowing

the future appeared a scary prospect.

He couldn’t understand how the TeleCare service would work but was open to any suggestions. TeleCare equipment such as a pendant on a wrist band so that he can move about his home and garden and still be able to call in an emergency, and know help is always at hand was installed. Derek has also benefited with a falls detector which automatically calls the lifeline service and knows if Derek has fallen or is lying on the floor.

Derek says, ‘TeleCare has literally changed my life, it is the equipment for the 21st century, I wish I had known about this sooner.

‘TeleCare equipment has made me feel safe and confident and I now have the freedom to live independently, knowing at the touch of a button I can get help quickly. I feel safe and comfortable living on my own and my extended family and friends know I am safe which gives them peace of mind.’

What is TeleHealth Lincolnshire?

Having your health measurements taken can be stressful and sometimes hard to organise. We also know that you will want to help yourself get as well as possible. Sometimes people forgot to take medication which can really affect their health so a reminder can be really useful. It helps to show your health professional your health measurements between visits.

Telehealth uses technology to provide services that assist in the management of long term health conditions, including Chronic Obstructive Pulmonary Disease (COPD), Chronic Heart Failure (CHF), Diabetes and Epilepsy. Telehealth enables you to have more control over your own health.

Who can use TeleHealth?

All health advisers or social care practitioners can easily get TeleHealth if you believe it might help you. If you do not have the equipment to help you take the

measurements we may be able to loan it to you.

Your nurse or doctor will agree how Telehealth can help you. At the agreed time the system called Flo will send a text to your mobile phone with details of any action you need to take. If you need to provide readings you will send a reply to Flo (for example blood pressure or sugar

levels). Flo will let you know if any further action is required.

Your doctor or nurse can see the information you send to Flo - but they will not check every reading. This system is designed to help you improve your health.

How will this help me?

• It saves you having to make arrangements to visit the nurse/doctor’s surgery just for readings to be taken.

• It allows you to take readings when you are ready and comfortable.

• It doesn’t matter if you are at home. Staying with family, working or on holiday - you can still get a reminder and send a text to Flo.

• Flo will remind you of actions you need to take.

• Flo will record all the details so when you next see your health adviser they can see how you have been managing your health.

How does Flo help health specialists?

Your health specialist - usually a nurse or doctor, wants to know you are getting better and that in between seeing them you are following their advice and it’s helping you.

Sometimes it’s hard for patients to get into a surgery, so if your nurse or doctor can see reports from Flo regularly, it helps them know you are taking care of yourself.

Integrated Community Equipment Service (ICES) In addition to other services that enable you to stay at home, we have an Integrated Community Equipment Service (ICES) which provides short and long term loans of equipment, ranging from simple walking aids, through to larger and more complex items such as pressure relieving mattresses and hoists. Equipment may also be designed to help you or to assist carers with the safer delivery of care. The service can also include installation, servicing and maintenance depending on the type of equipment specified.

Assessments of your equipment needs and the ordering of items are carried out by practitioners such as occupational therapists, social workers, physiotherapists, nurses or other health/social care professionals.

Common items of equipment are held within a central warehouse. We have a targeted delivery timescale to deliver the equipment to the service user within five working days after the order is placed. Technicians are trained to unpack and remove any transit packaging to enable the item to be

used. Occasionally, complex equipment may require setting up and the health or social care professional will provide some training and demonstrate to the service user and carer how to use the equipment.

As the service is a loan service there are facilities in place to return ‘no longer’ required items. To arrange return of items please call 0845 121 2031 to speak to someone who can help you.

Every effort is made to refurbish equipment to allow it to be recycled for re-issue to someone else. This reduces our costs and helps preserve our environment. Whilst every effort is made to collect items within 10 full working days after the request is logged, priority is given to deliveries.

Lincolnshire’s ICES service is a partnership between the County Council and also involves the following partners - NHS Lincolnshire (Primary Care Trust), Lincolnshire Partnership NHS Foundation Trust and the United Lincolnshire Hospital Trust.

Living at home continued Adapting your home Lincolnshire Home Improvement Agency is an awardwinning local charity which helps older people and people with disabilities to live independently and safely in their own homes. The service helps to ensure your home is accessible, secure and comfortable. You may find your mobility decreasing or be having trouble maintaining your property and the Agency may be able to help you

to improve security, ease of access and undertake repairs and improvements.

Occupational therapy Occupational therapists can help you lead a more productive, active, and independent life using a variety of methods, including the use of adaptive equipment. They can assess your home for hazards and point out things that contribute to falls.

Visiting you at home, an occupational therapist will be able to suggest adaptations that will improve your mobility. Perhaps handrails up stairs would be useful?

Major alterations such as fitting a downstairs bathroom or widening doorways could be paid for with a Disabled Facilities Grant (DFG).

An occupational therapist will talk you through any new equipment you may be given, as well as help you apply for funding for major adaptations, if you need them. To start the ball rolling, contact us and explain your circumstances.

An assessment of your needs will determine whether an occupational therapist will be the best person to help and what charges (if any) you can expect.

Getting help in your home You may need some more help to live at home independently. Care and support providers can offer a range of different services which may include: • Home care - help with personal care such as washing and dressing; • Ready-made meals - usually supplied frozen to reheat yourself; • Day services - available in care homes or community centres; • Respite care - residential care or care provided in your own home to give you or your carer a break; • Housing adaptations - we can provide information about equipment and adaptations to make it easier for you to continue to live in your own home; • Direct Payments - this scheme allows you to buy in the care you need for daily independent living; • Supported housing or Extra Care housing - provided by your district or borough council and the independent sector.

Preparing meals Community Meals, often known as ‘meals on wheels’, can help ensure that people receive proper nutrition on a daily basis, whatever their circumstances.

If you are aged 18 and over and unable to prepare and cook one hot meal a day, having your meals delivered could help make life easier. You made need this service due to a permanent situation such as sight loss, a physical or learning disability, illness or a temporary situation such as recovering after a stay in hospital, to help you maintain your independence and quality of life.

A Country Kitchen is a meals on wheels service that provides hot, healthy and nutritious food to your door every day. They already provide meals for Age UK and can provide them for people who need them in the Spalding and South Holland area. The Lincolnshire Kitchen is a meals on wheels service that also provides locally sourced hot food and even a sandwich supper to your door in the North Kesteven area.

Living at home continued Help in your home with maintenance and gardening If you can’t afford the cost of repairs and improvements, the Lincolnshire Home Improvement Agency (LHIA) will help you to look at other options. They may be able to help you source the funding for the work, which may include a grant from a charitable foundation or local authority, an insurance claim, a loan or a lifetime mortgage (often known as ‘equity release’).

Gardening and Handyman Scheme (Lincoln area) – If you are aged 50 and over and need help with gardening and maintenance of your property then contact Age UK Lincoln’s Handy Help Service. They can give you information about local tradesmen who may be able to help you. They also provide a minor repair service, for example help with fitting door bells, locks and safety devices, replace light bulbs, smoke alarm batteries etc.

There is an hourly charge for this service.

Handy Person Scheme – Lincolnshire Home Improvement Agency offers a handy person scheme to tackle small jobs in the home for older people and those with a disability, across the county. This could be anything as small minor plumbing jobs or fitting a grab rail or banister to offer extra support for the mobility impaired. They may be able to offer assistance with costs depending on individual circumstances.

Housing Options Advice Scheme – Sometimes homes can be adapted to improve access and security, but other times it simply isn’t possible due to either the changing circumstances of the resident or the property itself. In this case, the LHIA can advise on finding alternative accommodation with superior accessibility and security and will help you make a reasoned decision about whether moving home may be the best option to retain your independence.

Trusted Tradesman Scheme – It’s important to have trustworthy and reputable people working on your home.

Lincolnshire Home Improvement Agency runs a trusted tradesman scheme where every member is vetted to ensure they operate in a legal, decent, honest and fair way. They share their information with the Police and Lincolnshire Trading Standards.

We’re here to look after you The Lincolnshire Carers and Young Carers Partnership are here to support unpaid carers across Lincolnshire in their caring role.

If you are an unpaid carer living in Lincolnshire there are seven important things you should know about:

But in reality, being a carer can be difficult and lonely at times, and so a good place to start is with a Carers Assessment. This is an opportunity to share your feelings/experience as a carer with a dedicated person who can help you to guide you towards the help and support you are entitled to.

Carers’ Support Groups and Carers’ Partnerships Many support groups, carer organisations and charities are working to support carers in Lincolnshire? The Lincolnshire Carers’ and Young Carers’ Partnerships work together with organisations and carers to improve services and support in the County.

Carer’s Emergency Plan What would happen to your loved one if you were unable to care? Your free Emergency Plan is created by you and provides detailed instruction for others to follow in an emergency situation, giving you peace of mind.

Carer’s Information Pack This is an excellent information source on many subjects relevant to carers, a Carers Journal to record your interactions with professionals.

Carer’s newsletter This bi-monthly newsletter is full of carers’ experiences, stories, news and events. Including advice and fun

Carer Champions Lincolnshire has many Carer Champions who are helping to guide carers to the right support, and are raising awareness of carers. Together we’re shaping training and education for carers and staff.

Getting out and about We know that life is not easy when you are stuck at home all day, particularly if you need a little extra support.

Getting out, taking part in leisure activities and socialising with friends can make all the difference.

Day opportunities and day care Day opportunities By ‘day opportunities’ we mean the things or activities people may want to do during the day. This covers all opportunities for people whether it be in the day, evening or at the weekend. We used to call this day services and/or day care.

We want you to have more choice and control about what you do during the day. Personalisation will enable you to make such choices and control the services you choose to receive.

Whilst the list below focuses on traditional buildingsbased day centres, you may prefer support to do things in your community. You are able to use your personal budget to buy support directly from a care and support provider and helps you to do the things you most enjoy. We may be able to help you to develop such opportunities and tell you what alternatives may be available in your area.

Day care is a popular service offered to people who need some help with personal or practical care and are unable to get out and about by themselves.

Day care is usually available in a resource centre, community building or a residential home and offers more support than an ordinary day centre can provide. Professional caring staff can support you with personal care, such as going to the toilet, reminding you to take medication or help with washing or bathing. You may also be able to have a hot meal and drinks during the day and a chance to socialise and to take part in a number of activities.

Day opportunities often provide valuable help in sustaining independence for people. They are provided in a variety of locations including care homes, specially adapted premises and purpose built centres or in the community.

These services aim to help you retain independence in your own home, live healthily and sometimes give a break to a carer or family member who is helping to support you.

Some services may be specialised, catering for example for people with dementia, or other high dependency needs. In all cases, good day services should provide a stimulating environment able to address individual needs, including physical or mental frailty and still be met through a personal budget. To find out more about whether a direct payment could help you access these services there is more information on page 21.

Access to day opportunities may be arranged privately in many cases by approaching the service provider directly.

Some providers will tailor the support to meet your needs if you want to do something more suited to you as an individual.

The kind of day services we might arrange will depend on your needs. Priority is given to those whose independence is most at risk.

We are currently working with service providers, micro providers and personal assistants to develop day activities.

You will be offered a financial assessment to ensure you are not asked to contribute more than you can afford, and some people pay nothing. However if you have more than £23,250 in savings you will be asked to pay the full or maximum cost of your day services.

What if day care or a day centre isn’t for you?

Practical support is available to help you if you have a learning or physical disability, or a sight and hearing loss problem, to become more involved and use the amenities in your local community. An assessment to work out just what type of practical support is needed to help you get

out and about.

Support can include trained staff helping you learn how to use public transport by yourself and build your independent living skills and to enable you to remain as an active part of your community as much as you should wish.

Transport services There are a number of different bus companies running throughout the county which can help you get out and about. If you are finding it difficult to use public transport due to a disability or mobility problem, community transport offers a number of different ways to get out and about to the shops and other local amenities.

The Blue Badge scheme is a national scheme you can access if you have severe walking difficulties, regularly drive but are unable to turn the steering wheel by hand, or are registered blind. It allows you to park legally, close to places you need to visit such as shops and public buildings.

Care homes and housing options You may feel that you have exhausted all the options to remain living at home, and that it is time to look at moving into more purpose built accommodation with

support. In this section we also describe the different levels of care you, or the person you care for may require, if you decide to move to a residential environment.

Extra Care Extra Care housing supports you to live independently in your own home for as long as possible. Extra Care provides a secure environment with care services available 24 hours a day in your own home. Each apartment has a kitchen, bathroom, up to two bedrooms and a living room. The developments provide communal facilities, such as a restaurant and a hair salon, as well as areas to hold regular social activities.

Although it varies from scheme to scheme there is the opportunity to rent, part purchase or fully own an apartment. Whichever of these options you choose, your apartment will be your home. Should you currently benefit from financial help paying your rent or receiving support, this will still be available subject to your circumstances.

In Lincolnshire there are currently Extra Care schemes in Louth, Lincoln, Bourne, Sleaford, Grantham, Boston and Sutton-on-Sea. We are working closely with the developers to increase the number of schemes, which are managed by registered social landlords.

Extra Care housing enhances the offer to older people when they feel they require a little more support.

Providing peace of mind that someone is always on hand to help if required is at the core of the vision and is something we are seeking to increase the availability of throughout Lincolnshire.

Sheltered housing Sheltered or retirement housing is suitable if you like living independently but want the reassurance of knowing that assistance is on hand if there is an emergency, or if you expect to be away from home for long periods and need to know your home is safe. Often called retirement housing, there are many different types of sheltered schemes, available both to rent and to buy. Schemes usually consist of between 15 and 40 dwellings. These may be bedsitting rooms, self-contained flats, bungalows or luxury apartments.

Some schemes are simply housing designed to meet the needs of older people and linked to a community alarm.

The majority of schemes have a scheme manager/warden and community alarm service. There are often communal facilities such as a lounge, laundry, guest flat and garden.

Meals are not normally provided but a few schemes include a restaurant and some can arrange hot meals.

All services are inspected by the Commission, who report on their findings. These inspection reports are available from the care home or the Care Quality Commission website at www.cqc.org.uk Care homes offering personal care only These are commonly referred to as residential care homes.

If you are reasonably active but would like greater security and care, subject to a needs assessment, a care home only offering personal care may be the option you choose.

Care homes with nursing These are homes previously called ‘nursing homes’. If you think you may need nursing care in a home, you will need

to be visited by Adult Care to access the care you may need. This visit might be in your own home or in hospital if you have been ill. You will be fully involved in planning your care needs.

If a care home providing nursing care is the best solution for you, we will give you information to help you find a home that meets your requirements. The cost of the nursing care part of your fees is paid by the NHS to the home directly. Currently this is a flat rate of £109.79 per week. It is important that you get your nursing assessment done before you go into the home. You can contact the nurse case management teams for your area at the Primary Care Trusts (PCT).

Personal care is not paid for by the NHS and must be paid for by you if your capital/savings exceed £23,250. See page 47 for further information.

Please use in conjunction with the care homes checklist on page 34.

The points below give you a good starting point of things to look for in care homes specialising in dementia care. You can use this checklist with the one for residential care on page 34 to help you feel more confident in the choices you are making for your loved one or the person you are caring for.

RESIDEnTS The best indication of a good home is that the residents appear happy and responsive. Ask for more information on dementia specific activities, i.e. reminiscence or features throughout the home such as specialised signage that makes it easier for people with dementia or memory problems to identify where they are.

ACCESS If the person with dementia or memory problems is likely to need equipment or adaptations you may want to check: • There is adequate signage as described earlier throughout the home, to act as prompts and cues.

• What is the home’s security policy about locking external doors or being supervised and supported in outside areas?

BEDROOMS What room will the person with dementia or memory problems have?

• It can be worthwhile considering how easy it would be for someone to find their way back to their room, if there is one available on the ground floor to avoid the risk of falls.

• Are reminiscence activities available?

• Ask about memory boxes.

• Will staff compile life story books with you and the person you are looking for to give them a better understanding of their life?

STAFF It is important to note whether the staff seem friendly and caring towards residents and whether they treat them with dignity and respect.

• Ask if staff have specific training and experience in caring for people with dementia.

• Who will their key worker be? This should be someone you and they can go to who will have detailed knowledge and particular responsibility of their care.

MAnAGER/HEAD OF HOME A manager who is caring as well as efficient can make all the difference to the atmosphere of a home. It is important for you and the person to have a good relationship so you feel confident in the general running of the home. It is worth talking to the manager and any deputies they may have on how long they have worked in the home, and their general experience. Points to ask about: • Do they have detailed knowledge and training of dementia?

Can they deal with any difficulties that may arise in an understanding way?

• Will they be conducting the initial assessment of the person requiring care in their own home or current place of residence?

• Ask if you can bring any personal items of furniture or personal possessions such as photographs that are particularly important to the person you are looking for.

This can increase familiarity with their surroundings, trigger memories and help them settle in better.

ACTIVITIES In any home there should be a dedicated activities programme that has been designed with all residents’ capabilities in mind, and generally a dedicated activities coordinator is employed to help deliver this with the other care staff in the home. It is important to ask which of these activities have been planned specifically for those who suffer from dementia or who have other memory problems.

Care homes and housing options continued The following sections have been included to help you understand the different terms you may come across when arranging care for you or someone else, depending on your level of need for care and support. If a care home

or care home with nursing is the right option, this will be chosen following an assessment and discussion with you and your family or carer.

Intermediate care Intermediate care differs from respite care. While respite care provides care on a temporary basis, intermediate care is designed to rehabilitate you and enable you to manage by yourself. Intermediate care, including care home fees, provided by the NHS, is free. It can be provided in your own home or in a care home and is usually limited to a maximum of six weeks.

Based around a programme of therapy and treatment, intermediate care is designed to make you as independent as possible. The structured programme of support may mean that you do not have to go into hospital at all, or may enable you to get home faster after you finish hospital treatment, but cannot manage independently at home. Ask your care manager for further details.

Continuing care Continuing care is the name given to the care needed by an adult who because of disability, accidents, illness or the effects of getting older, requires help over a period of time to manage their daily life. It is a package of care that involves services and funding from both the NHS and social services and is available in a range of settings. All NHS healthcare is provided free of charge. Social services care provision is means tested and you pay either a contribution or the full cost depending on your financial situation and the setting in which your care is provided.

An assessment is done by health and/or social care

professionals to identify and prioritise your needs. The assessment will be used to determine who is responsible for providing and funding the services to meet your assessed needs. Decisions on how and where services will be provided will be made with your involvement.

If you have been assessed as requiring care in a registered care home with nursing and you are not eligible for NHS continuing healthcare the NHS will pay a contribution towards the cost of your nursing care. This is known as NHS-funded nursing care and is paid directly to the nursing home. The current amount is £109.79 per week.

nHS continuing healthcare NHS continuing healthcare is a package of continuing care that is arranged and funded solely by the NHS. If your assessed needs identify that you have a ‘prime health need’ you will be eligible for NHS continuing healthcare.

This means that care services to meet your assessed needs will be provided and funded by the NHS. The National Framework for Continuing Healthcare and NHS-funded Nursing Care 2009 (revised) sets out the eligibility criteria and the assessment process for accessing NHS continuing healthcare funding. Following

a multi-professional assessment of your needs the Primary Health Care Trust that your GP is registered with will decide whether or not you are eligible for NHS continuing healthcare.

If you are eligible your care will be provided free of charge to you in any setting where your assessed needs can be met. In your own home this means that the NHS will fund all the care required to meet your assessed needs. In a care home the NHS makes a contract with the home and pays the full fees for both your care and accommodation.

Reviews As your needs can change, regular reviews will be undertaken which may result in changes to the provision and funding of your care.

You can also request a review of your care needs if you feel they have changed.

Dementia care The challenge of living with dementia continues to confront increasing numbers of people as our population ages. This is especially so in Lincolnshire, where in the

last 10 years the increase in our population aged 65 plus is three times that of the national average increase, accounting for approximately 22% of the county’s

Dementia care continued population compared to a 17% national average. Due to this our county continues to show a proportionately higher incidence of dementia than both the regional or national average.

Within the UK as a whole there are estimated to be over 750,000 with dementia. Locally, in Lincolnshire estimates show there are currently around 11,000 people with dementia with this number predicted to grow to over 17,000 in the next decade. In most district council areas this would represent a potential doubling of cases.

As GPs are usually the first point of contact for people with fading memories, the strategy will be working with GPs to increase the number of people getting a formal diagnosis.

Following a diagnosis of dementia, your GP may refer you to a specialist like a Consultant in Old Age Psychiatry, a Community Psychiatric Nurse or an occupational therapist with a special interest in dementia care.

A memory assessment service: diagnosis and early intervention A memory assessment service is comprised of specialist staff skilled in the assessment and treatment of people experiencing dementia, including consultant psychiatrists, mental health nurses, occupational therapists, psychologists, social workers and support workers.

The memory team work together to provide specialist assessment and early intervention for people with Age Care Advice – Dementia Companion Service We provide you with an opportunity to have one Care Coordinator/Companion working alongside you during the dementia journey even post diagnosis. Available to you 7 days a week, 8am to 8pm for as long as you require.

We will ensure your care is appropriate, safe, monitored and adapted as your care needs change. We will help access resources to understand the illness and your loved one is monitored if you take a break/holiday with a strong focus on always supporting the carer.

We provide support for people when attending appointments such as the Memory Clinic, Consultant,

suspected dementia where other common causes have been excluded by the GP. The assessments provided build upon the tests and history started by the GP, and where required request additional investigations such as head scans or further blood tests to further support the process of diagnosis.

Following assessment the memory team will then either signpost on to other appropriate services or discharge back to the GP if a diagnosis of dementia is not the cause of the problems. Where a diagnosis of dementia is made, then information and appropriate early treatment and intervention options and advice are discussed and provided, and an individual care plan detailing the individual’s care needs is completed. The GP will be informed of the findings to ensure continuity of care and signposting to appropriate support networks and organisations for post-diagnosis support provided.

Occupational therapy Occupational therapists can help you lead a more productive, active, and independent life using a variety of methods, including the use of adaptive equipment. They can assess your home for hazards and point out things that contribute to falls.

Visiting you at home, an occupational therapist will be able to suggest adaptations that will improve your mobility.

Perhaps handrails up stairs would be useful? Would a ramp up to your front door help if you are in a wheelchair?

Keeping people safe Safeguarding vulnerable adults Safeguarding and protecting vulnerable adults from abuse or neglect has traditionally been seen as a major role of those working in the social care sector. Now, more than ever given recent cases that have made national news, it is becoming an issue for everyone in society, not just for those people who have a professional responsibility to safeguard.

Far too many vulnerable adults suffer abuse, neglect and worse, often at the hands of relatives and carers, those who you would least expect to treat them in that way. A vulnerable adult is a person aged 18 years or over who may be unable to take care of themselves, or protect themselves from harm or from being exploited.

Some people are more vulnerable than others because they are old and frail, or have a sensory impairment, a disability, a mental health problem, or have some form of illness. This means they may have difficulty in making their wishes and feelings known and this may make them vulnerable to abuse. It may also mean that they are not able to make their own decisions or choices. A person may also be vulnerable because of a temporary illness or difficulty.

What is adult abuse?

Abuse is when somebody or some people do or say things to you or somebody else that makes you or them feel threatened or frightened. It is mistreatment by somebody or some people that violates a person’s human and civil rights.

It may be neglecting a person’s needs, treating somebody disrespectfully or causing physical harm. It may be an isolated incident or it may be repeated incidents. It may be that the abuse is deliberate or it may be unintentional and due to ignorance or lack of training or understanding. It may be that a person is at risk of abuse or being abused in several ways.

It can happen anywhere - in a care home or a care home with nursing, a hospital, in the workplace, at a day centre or educational establishment, in supported housing or in the street.

Forms of abuse could be physical, sexual, psychological, emotional or financial. It could also cover the issues of neglect and institutional abuse – where the abuse affects more than one person within an organisation or a service

and is not addressed by the service’s management. This can be repeated incidents of neglect or poor standards of care that are being ignored. It may be that you are not treated with the dignity and respect that you deserve.

Who might be causing the abuse?

Anyone may be an abuser. It may be somebody you know well or it may be a stranger. It may be more than one person. It might be a family member, a friend, a neighbour, a paid or voluntary carer, a nurse, a social worker, a GP, a visitor to your home, or somebody who you don’t know. If you live in a residential setting or visit a day centre, it may be another user of that service, a visitor, or a carer.

What is adult safeguarding?

Adult safeguarding refers to work aimed at preventing or stopping abuse and neglect of adults who need care and support, and who are more at risk of harm because of those needs.

Safeguarding adults takes place in communities through a number of different factors: • awareness raising and an absence of tolerance of abuse • health and social care services and police responses that are of decent quality effective responses to allegations of harm • providing access to services needed, advocacy, justice and support using learning from safeguarding reviews to improve services.

• Policy and practice around safeguarding adults has developed rapidly over the past12 years as society has become more aware of abuse and neglect in institutions, in people’s homes and in the community.

Who is responsible for adult safeguarding?

Adult safeguarding is everybody’s business. Any person may recognise and report abuse or neglect, and everyone can play a part in building communities where abuse does not happen.

All of the bodies responsible for adult safeguarding need to work effectively with each other, and with local Health and Wellbeing Boards, Children’s Safeguarding Boards, Community Safety Partnerships (set up by the

What should you do if you suspect abuse or are worried about abuse?

Doing nothing is not an option. Lincolnshire County Council operates a zero tolerance of all forms of abuse. All adults should be able to live their lives free from fear and harm.

Your course of action will depend on the situation.

If you or the person you are concerned about is in danger and immediate action is required you should ring the emergency services on 999.

If you or the person you are concerned about is not in immediate danger, you should ring our Customer Services Centre on 01522 782155.

You will be able to speak to a sympathetic adviser who will listen to your concerns and take them seriously. They will offer support and direct your concerns to a specialist worker who will arrange to meet with you.

You may have a relative, friend or independent advocate present if you wish. Together you will discuss the issues and together you will agree a plan to keep you as safe and independent as possible. You will be able to have a copy of this plan. We will agree further contact as is needed.

We may involve other agencies to help support you safely, but we will agree this together. Where the abuse

is a crime, the police need to be involved to prevent other people from suffering as well. We will jointly help to support you.

We will listen to your concerns and take them seriously. We will treat you with dignity and respect at all times. Our main aim will be to help you to live safely whilst maintaining the maximum possible level of independence, choice and control.

If the abuse is also a crime such as assault, racial harassment, rape or theft you should involve the police to prevent someone else from being abused. If the police are involved we will work with them and with you to support you.

If you are worried about contacting the police you can contact us to talk things over first.

Make your voice count There are many ways to get in touch if something is right or you feel unhappy and need further support and advice.

The following section details the different ways you are able to do this.

Advocacy - Someone to speak on your behalf Having someone to speak on your behalf is often known as advocacy.

Advocacy means having someone who will express your views and wishes, secure your rights and represent your interests. It safeguards people who are vulnerable by speaking up for them. It enables people with physical or learning disabilities, older people and those with mental health needs to make informed choices and decisions about their own health and social care.

Advocacy helps you to access information and services and to explore choices and options. Advocacy is completely independent from the organisations that provide social care services. Advocates act only according to the wishes of the person they are speaking for. They do not take their own view of what is best, or try to influence the person to make a different choice. Advocacy services in Lincolnshire are provided by Total Voice Advocacy, an organisation which is independent of the Council.

Total Voice Advocacy services in Lincolnshire Advocacy can help raise the voices of people living in Lincolnshire by helping them express what it is they want and aspire to as members of their community, both locally and wider. We currently provide: • user involvement and participation; and • group advocacy and one to one advocacy to people who have complex needs, learning disabilities, physical disabilities, sensory impairments, mental health, older people and carers of people using services of health and social care.

We also provide statutory advocacy roles in IMCA (Independent Mental Capacity Advocate), IMHA (Independent Mental Health Advocacy) and the DoL (Deprivation of Liberty safeguards).

Our advocacy team is well skilled in ensuring that the advocacy process is meaningful for individuals who use the service. We use different techniques to ensure that our service meets the needs of each individual we meet. We know that ‘one size fits all’ does not work for everybody and our advocates spend time with people developing methods that suit them best. We also signpost people to other organisations that are better placed to help individuals with their enquiries if required.

Make your voice count continued Making a complaint about care services All organisations need to know how they are performing.

They are happy to receive your feedback on their service whether they are compliments or complaints. Feel free to tell them what you think and your comments can be used constructively to improve the service.

If you do need to make a complaint you should feel able to complain about any aspect of your care that affects your happiness or comfort. This could be about the way you are treated by a staff member or the quality of the food you are served. You can also make comments and suggestions about possible improvements to your surroundings and the services provided.

Making a complaint should not be difficult for you. By law, every care home and social care service must have an efficient procedure for dealing with complaints. You should have no fear of raising an issue, nor suffer any detriment to your care or support for doing so. Ask for a copy of the service’s complaints procedure so that you know what you

need to do and how they will deal with your complaint.

If you are concerned about the care that you, a friend or a relative is receiving, it makes sense to speak to the manager before you take any further action. The problem may be resolved quite easily once they are made aware of it.

However, if you need to make a formal complaint, you should initially contact the registered owners of the service. They have a duty to respond to any complaints made. If your complaint is about a breach of regulations, contact the Care Quality Commission on 03000 616161.

Care home contracts for self-funding residents If you are self-funding your care and have a contract with your care provider, it is important to speak to solicitors with experience in this field. Any contract should explain: the cost of care, what services are included in the price and how often this is reviewed; what complaints procedure is in place; what your rights are and the obligations of the care provider under the contract; the

procedure for cancelling a contract and the administering of medication.

Information, legal and financial issues Self-funding advice If you have capital or savings of more than £23,250 you will have to pay for any care services you require but you are still entitled to an assessment of your needs by us.

If you have assets of less than £14,250 you may be entitled to assistance with your funding from us. Between these two figures, a sliding scale operates whereby you must pay £1 per week for each £250 you have above the lower figure.

Before you think about the likely costs of any care and support, it is important that you get information and advice about the options open to you, both regarding the type of services that might suit you, and whether your local council might contribute to the costs. If you don’t

you may make poor decisions about either the care you organise or how you pay for it or both.

This function will allow you to determine exactly how much you may have to contribute towards your care fees in the privacy of your own home. What is a self-funder?

There are many options for help with funding your care.

If you are aware that you will need to fund your own care, there are a number of organisations which can offer advice. These include independent or specialised financial advisors who can explain the options available to you.

If you are paying for the full cost of your support needs, you are known as a ‘self-funder’ which means: • you have chosen not to ask us for support; or • you have been assessed but do not meet our eligibility criteria; or

• you have been assessed as eligible for support but are moving to a care home and will not qualify for help with the fees because your income, savings or capital assets exceed £23,250 (please note this capital limit usually changes every April), or you are eligible for care and support at home but due to your level of capital/ savings or income you have been assessed to pay the full chargeable cost or the maximum charge, and you have therefore decided to fund your care and support yourself.

Why do I need financial advice?

Whatever option you choose, you need to make sure you will have enough resources to cover the cost of your care going forward. If you do not have enough cover there is a risk that you will be unable to fund your own care needs for your lifetime or that you will have to move to a different care provider. This is not a situation anyone would wish to find themselves in, but can be avoided by careful planning and the obtaining of professional advice.

You may decide to apply to your local authority for an assessment of your needs, which will determine the level of funding you may receive from your local authority and what you will have to contribute towards your care costs.

The local authority will consider your previous investments to make sure you haven’t invested as a way to deprive yourself of capital assets or income, this will include previous investments you’ve made towards your care costs.

What options are available to me as a self-funder?

• You can still contact us and ask for an assessment of your needs; they can help you to identify your support and outcomes to help you plan for your support needs.

• You may find that you are entitled to support with the costs or be entitled to benefits you are currently not receiving.

• You can have a financial assessment even if you think that we will not make a contribution to your support needs.

• If you have to pay the full cost of your support, we can offer information, advice and guidance on support and opportunities available to you.

• We can advise you on the cost of your support needs and how much your support needs are likely to cost; our advice could help you save money.

• If you have a carer, your carer is entitled to a carer’s assessment.

• We can offer information, advice and guidance on preventative services; we have a number of services within the community that can support your health and well being – see First Contact, Excellent Ageing.

• Reablement – you could be eligible to time limited support especially after being in hospital, having a fall etc.

It is important to make sure that you find out as much as possible before making care funding decisions.

Assuming you will be moving into care and you meet the eligibility criteria, you may be entitled to some of the following financial assistance and support: Twelve-week property disregard If your former home is included in your financial assessment but your other capital is less than £23,250, and your income is not enough to meet your care home fees, we will help with the cost during the first twelve weeks of permanent care provided we agree that care is needed.

Deferred Payment Agreements After the twelve-week property disregard period, any financial help from us will be charged against the value of your home and recovered once your house has been sold.

A legal charge will be placed on your home, and there will be some legal and administrative costs incurred in the set up of the agreement, it is important to seek independent legal and financial advice in these circumstances. Please see the ‘Deferred Payment’ leaflet available from us for more information.

However, we may limit how much we will pay and it may affect your entitlement to pension credit if your property is not seen to be on the market and becomes treated as capital by the Department of Work and Pensions.

Attendance Allowance This is a non-means tested, non-taxable benefit from the Department of Work and Pensions paid at the lower

rate of £53.00 per week for those needing care by day or night, and at the higher rate of £79.15 per week for those needing care both during the day and night. Everyone can, and should, claim this benefit. Please note that entitlement to this benefit does change upon admission to care so it is important that you contact the Attendance Allowance unit with any change in circumstance to avoid any overpayments in benefit being made. nHS community healthcare You can receive community healthcare services in any setting, including your own home or in a care home. This care is usually provided by a community nurse and is free of charge. Healthcare services may also be provided by therapists and specialist nurses or through organisations such as Marie Curie or your local hospice. Again these services are free of charge. nHS-Funded nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care you will be entitled to a non-means-tested NHS Nursing Care Contribution (currently £109.79 per week for the standard rate) towards the cost of your nursing care. This is paid directly to the home.

Access to NHS continuing healthcare funding is through an assessment process to determine if you have a prime health need as set out in the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care 2009 (revised). If you are eligible for this funding the NHS will fund the cost of your care in any setting where your assessed needs can be met. This funding will cover the cost of your board and accommodation if you live in a care home. It is not means tested. However your eligibility to receive this funding will be reviewed by NHS staff on a regular basis and may be withdrawn following a review and a period of notice.

Running out of money If your capital is likely to reduce to £23,250, you must let us know well in advance as we will step in and help with your care fees. We must conduct an assessment of your situation and will make a contribution - described below.

If the home you have chosen charges more than this, you must find someone to help pay the difference – a ‘top-up’ payment. Whoever does this, whether family or a benevolent charity, they should realise that they may have

Understanding your rights before moving into care

is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before committing yourself.

What will you have to pay for your care?

Do you have more than £23,250 in capital and savings?

What is ‘capital’?

The value of your home is included when assessing capital unless: • Your partner or: • A relative who is over 60 or disabled or: • A child under 16 who you or a former partner maintain still lives there. nO We will contribute financially towards your care costs.

How much?

To work out how much we will pay, you must arrange a Community Care Assessment. This will define your care needs and how they will be best met and your financial situation. Whichever homes you consider must be able to meet these needs.

Your choice of care home will be limited to those that accept our funding level. More expensive homes will expect you to arrange a third party to ‘top-up’ the difference. You are not allowed to do this yourself if your capital is below £23,250.

If your capital is less than £14,250 and the home you choose charges fees that are within our funding rate, your contribution will be assessed only on your income.

With capital between £14,250 and £23,250 you will be expected to contribute £1 per week for every £250 you have above £14,250.

YES Arrange for a Community Care Assessment with us to define your care needs. Whichever homes you consider must be able to meet your assessed care needs.

If, apart from your property, your savings are less than £23,250, we can help with your care costs for the first twelve weeks. After this time, any money from us will be regarded as a loan under the Deferred Payment scheme and will need to be repaid once the scheme is terminated usually by way of the property being sold or the resident’s

demise within the scheme’s payment terms to avoid interest charges. (Please see the Deferred Payments leaflet available from us for more information.)

Whatever your circumstances Remember: • If your partner still lives at home, they will not be means-tested. If you both have a private pension you can opt for only half to be considered when you are assessed.

• Your assessment will be made up of two elements, a ‘care’ part and a ‘financial’ one.

• A nursing home will generally be more expensive than a residential home which only offers a personal care service.

Consider claiming: • Income support • Pension Credit Definitely claim: • Attendance Allowance, worth either £53.00 or £79.15 per week depending on your care needs.

Moving into a care home with nursing?

• You will be eligible for the NHS Nursing Care Contribution – currently £109.79 per week.

Always seek advice: • Independent help is available to guide you through your financial options. There may be a number of solutions to retaining your capital whilst paying for care.

Third party payments If the care home you eventually choose costs more than the rate we usually pay for a person with your needs, someone will have to make up the difference. This extra payment is often referred to as a ‘top-up’ or ‘third party top-up’.

Except in limited circumstances, the law states that you are not allowed to make this additional payment yourself.

The responsibility for this often falls to a member of your family or a benevolent sponsor such as a charity. Once this person or organisation has been confirmed, they must sign an agreement formalising the arrangement.

It is important that whoever agrees to do this for you is made aware that the amount could increase in the future and they must be confident any increases will be met.

Should payments stop for any reason, you should seek help and advice from us immediately.

How solicitors can help A solicitor can give you impartial advice about wills, making gifts, estate planning and Powers of Attorney.

Some can also offer guidance on immediate and longterm care plans, ensuring (if applicable) the NHS has made the correct contribution to your fees.

Lasting Powers of Attorney (LPA) allow you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/or your property and financial affairs. A LPA is only valid once registered with the Office of the Public Guardian. It allows for a person of your choice to make decisions on your behalf at a time when you may be unable.

The Court of Protection can issue Orders directing the management of a person’s property and financial affairs if they are incapable of managing their own affairs should they not have a LPA. The Court procedure is presently very slow and the fees are quite expensive so preparing

a LPA is always advisable, providing you have somebody sufficiently trustworthy to appoint as your attorney.

An ‘advance directive’ allows you to communicate your wishes in respect of future medical treatment but it is not legally binding. You may instead wish to make a living will, properly known as an ‘advance decision’ setting out treatment that you do not want to receive in specified circumstances, which would legally have to be followed, even if you die as a result.

Any proposed gift out of your estate needs careful consideration of the benefits, risks and implications, particularly on any future liability for care costs or tax liability.

Long-term care: whether you remain in your own home or move into sheltered or residential care, you may qualify for financial assistance in the form of social care and NHSfunded care and welfare benefits.

Inspection and registration of care services The Care Quality Commission registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others.

Since October 2010, all care providers have been registered under a new system, introduced by the Health and Social Care Act 2008, which will bring adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety for the first time.

The Care Quality Commission will hold and publish up to date information about the compliance of health and social care providers with essential standards of quality and safely.

Following an inspection, each care home and home care agency is given a report of how it rates against national essential standards of quality and safety.

Each service’s report can be seen on the Care Quality Commission website: www.cqc.org.uk The focus of an inspection is on the standards of care that people who use the service receive and whether they are happy with their care. Virtually all inspections are unannounced.

The guide has been developed through partnership working between ourselves, NHS Lincolnshire, Lincolnshire Police and Age UK (previously called Age Concern).

The first edition of the guide was published in late 2008 with the second edition published in January 2010. The Good Life Guide is funded by the Health & Well-being Fund.

The wide variety of topics covered in the handbook aim to help you stay secure and healthy throughout your retirement. The guide gives handy tips on a range of topics including how to make your home safer and crime prevention.
